Le atassie spinocerebellari (SCA) sono malattie neurologiche genetiche che causano squilibrio, scarsa coordinazione e difficoltà di linguaggio. Esistono diversi tipi di SCA e questo studio si concentrerà sui tipi 1, 2,3 e 6 (SCA 1, SCA 2, SCA 3, nota anche come malattia di Machado-Joseph e SCA 6). Le malattie sono rare, lentamente progressive, causano difficoltà neurologiche sempre più gravi e sono variabili tra e all'interno dei genotipi. Lo scopo di questo studio di ricerca è riunire un gruppo di esperti nel campo dell'SCA allo scopo di saperne di più sulla malattia.

Le domande di ricerca sono:

  1. Come progredisce la tua malattia nel tempo?
  2. Quali sono i modi migliori per misurare la progressione?
  3. Alcuni geni, diversi dal gene che è anormale nella tua malattia, hanno qualche effetto sul modo in cui si comporta la malattia?

Questo è uno studio a livello nazionale e prevediamo che 800 pazienti parteciperanno in tutti gli Stati Uniti. I partecipanti rimarranno nello studio per un periodo di tempo indeterminato. Le visite di studio verranno effettuate ogni 6 o 12 mesi a seconda del sito partecipante.

Descrizione dettagliata

Se decidi di partecipare a questo studio, raccoglieremo 1 cucchiaio (15 millilitri) di sangue durante la prima visita di screening per estrarre il tuo DNA. Il campione verrà inviato al laboratorio di ricerca del dottor Stefan Pulst presso l'Università dello Utah per lo studio dei fattori genetici che modificano il decorso della tua malattia.

Come parte di questo studio, vorremmo mettere un po' del suo sangue in un deposito di tessuti. L'invio del campione al deposito può fornire agli scienziati prezioso materiale di ricerca che può aiutarli a sviluppare nuovi test diagnostici, nuovi trattamenti e nuovi modi per prevenire le malattie. Gli scienziati non useranno il tuo campione, o materiale isolato da esso, per prodotti o servizi commerciali. Il tuo sangue sarà custodito dal dottor Stefan Pulst.

Il tuo campione non avrà il tuo nome o altre informazioni personali ad esso collegate. Il tuo campione può essere condiviso con i ricercatori dell'Università dello Utah e di altre istituzioni. Le uniche informazioni che conserveremo con il campione sono la tua età, quale malattia hai, l'età di insorgenza della tua malattia e la durata della malattia. Il Clinical Research Consortium for Spinocerebellar Ataxias (CRC-SCA) sta cercando soggetti per partecipare a uno studio di ricerca clinica su pazienti con SCA 1, 2 3 e 6.

I potenziali partecipanti dovrebbero avere sintomi di atassia con una diagnosi di SCA 1,2,3 o 6 stabilita dai test del DNA sul paziente stesso o su un altro membro della famiglia affetto e avere un'età compresa tra 6 e 80 anni.   • Atassie recessive, legate all'X e mitocondriali note
  • Esclusione di SCA 1, 2, 3 e 6 da precedenti test del DNA,

Participants with Spinocerebellar Ataxias (Main Study)
Individuals aged 6 or older with the spinocerebellar ataxias 1, 2, 3, 6, 7, 8, 10, 27b, or RFC1-Ataxia will be enrolled for genetic testing, blood collection, assessments, and questionnaires.
About two teaspoons (10 milliliters) of blood will be collected during the first/screening visit to determine SCA type.
Up to 50 milliliters of total blood (whole blood, plasma, serum) may be collected at each visit to measure markers of neurological disease.
Participants will complete various motor function and cognitive assessments and self-report questionnaires.
(Optional) About 1 1/2 tablespoon (25ml) of CSF collected in adults.
Participants with Spinocerebellar Ataxias (MRI Sub-study)
Adults with the spinocerebellar ataxias 1, 2, and 3 will be enrolled for genetic testing, blood collection, assessments, questionnaires, and magnetic resonance imaging (MRI).
About two teaspoons (10 milliliters) of blood will be collected during the first/screening visit to determine SCA type.
Up to 50 milliliters of total blood (whole blood, plasma, serum) may be collected at each visit to measure markers of neurological disease.
Participants will complete various motor function and cognitive assessments and self-report questionnaires.
Participants in the sub-study will undergo an MRI scan of head and spine lasting up to 90 minutes at 3 Tesla strength.
Healthy Controls (MRI Sub-study)
Adults without spinocerebellar ataxias or other neurological conditions will be enrolled for genetic testing, blood collection, assessments, questionnaires, and magnetic resonance imaging (MRI).
About two teaspoons (10 milliliters) of blood will be collected during the first/screening visit to determine SCA type.
Up to 50 milliliters of total blood (whole blood, plasma, serum) may be collected at each visit to measure markers of neurological disease.
Participants will complete various motor function and cognitive assessments and self-report questionnaires.
Participants in the sub-study will undergo an MRI scan of head and spine lasting up to 90 minutes at 3 Tesla strength.

The modified Friedreich Ataxia Rating Scale - Part E (mFARS-E) is a 7-item comprehensive neurological assessment of upright stability. Item E1 (Sitting) is scored 0-4, Items E2-E5 (Stance) are scored 0-4 based on how many seconds a patient can maintain position without support, and Item E6 (Tandem Walk) is scored 0-3, and Item E7 (Gait) is scored 0-5. Participants may be given up to three attempts (trials) for Items E2-E5, with each attempt recording a score 0-4, and then averaging trial scores for the total Item score. Total score is a sum of item scores on a scale of 0 to 36 points, with higher scores indicating greater impairment and loss of balance.
