Criteri di inclusione:
- Sono visti nel centro scleroderma UT Houston (UTHSC)
- Incontra il 2013 American College of Rheumatology (ACR)/European League Against Rheumatist (Eular) Criteri o Crest Criteria per SSC
- Soddisfare i criteri di Roma IV per la costipazione
- Soggetto disposto a firmare un modulo di consenso informato
- Soggetto ritenuto probabile che sopravviva per ≥ 1 anno dopo l'iscrizione
- In grado di seguire la procedura di studio e il follow-up
- I soggetti maschili e femminili sessualmente attivi del potenziale di gravidanza devono accettare di utilizzare un metodo efficace di controllo delle nascite durante il trattamento e il periodo di follow-up
- I soggetti femminili del potenziale di gravidanza devono avere un test di gravidanza negativo nelle 72 ore prima della procedura
- Soggetti che accettano di aderire a una dieta stabile durante lo studio
- I soggetti devono avere un medico curante che fornirà cure non trapianti per l'argomento
Criteri di esclusione:
- Soggetti con costipazione da cause non attribuite a SSC come determinate dal medico curante (ad es. Malattia infiammatoria intestinale, farmaci, diabete significativo o ipotiroidismo, ecc.)
- Soggetti non disposti a smettere di assumere integratori probiotici durante la durata dello studio
- Soggetti che hanno diarrea definito come sgabelli sciolti e acquosi tre o più volte al giorno
- Soggetti che hanno post-totale o emicolectomia o la presenza di una colostomia
- Soggetti incapaci di (a) tollerare il trapianto di microbioma tramite clistere per qualsiasi motivo o (b) le capsule di deglutizione
- Soggetti che richiedono terapia antibiotica sistemica 4 settimane prima dello studio
- Se i soggetti sono in immunosoppressione, la dose di immunosoppressione dovrebbe essere stabile per almeno 4 settimane prima dell'iscrizione allo studio

Il placebo sarà somministrato come capsule dalla settimana 2-10 e come clistere alla linea di base e alla settimana 11.

Per la somministrazione orale, ogni dose conterrà il numero di capsule necessarie per consegnare la polvere di batteri intestinali liofilizzati derivati dopo liofilizzazione di 60 g di feci filtrate in 300 ml 0,85% di NaCl e mannitolo 2%. I partecipanti prendono le capsule (5-6 capsule due volte alla settimana) dalla settimana 2 alla settimana 10. Le capsule saranno prese per un periodo fino a un'ora per massimizzare la tolleranza e assicurarsi che le capsule non vengano espulse dal vomito. Per la somministrazione tramite clistere di ritenzione, che verrà effettuato al basale e alla settimana 11, 50 g di feci filtrate e congelate in 250 mL di cloruro di sodio 0,85% (NACL). |

This consists of 34 questions covering domains such as diarrhea, constipation, abdominal pain, nausea, bloating, gastroesophageal reflux, fecal incontinence, and emotional well-being related to GI symptoms.
Each item is scored on a Likert scale ranging from 0 to 3 (0 = none, 1 = mild, 2 = moderate, 3 = severe), with higher scores indicating greater symptom severity.
Domain scores are calculated as the sum of responses to items within the domain, and a total score is calculated by summing all domain scores, representing the overall GI symptom burden.
Total score will be reported, and the total score ranges from 0-102.

The Pelvic Floor Distress Inventory Questionnaire-20 (PFDI-20) is composed of 3 subscales: the Urinary Distress Inventory-6 (UDI-6), the Pelvic Organ Prolapse Distress Inventory-6 (POPDI-6), and the Colorectal-Anal Distress Inventory-8 (CRADI-8).
For each question, the participant must indicate how much bowl, bladder, or pelvic symptoms have been bothering them in the past 3 months on a 4-point scale that ranges from "not at all" (0) to "quite a bit" (4).
Total score for each subscale ranges from 0 to 100, with a higher score indicating worse distress.
The sum of the 3 subscales are added together to get the PFDI-20 summary score, which ranges from 0 to 300, with a higher score indicating worse distress.
