Questo è uno studio non randomizzato e non interventistico che fa parte dell'iniziativa Randomized Controlled Trials Duplicated Using Prospective Longitudinal Insurance Claims: Applying Techniques of Epidemiology (RCT-DUPLICATE) (www.rctduplicate.org) del Brigham and Women's Hospital, Harvard Medical School. Gli studi randomizzati controllati (RCT) hanno dimostrato i benefici cardiovascolari delle moderne terapie incretiniche semaglutide e tirzepatide in popolazioni selezionate. SUSTAIN-6 (NCT01720446) e SURPASS-CVOT (NCT04255433) hanno mostrato riduzioni degli eventi cardiovascolari con semaglutide e tirzepatide tra i pazienti con T2DM ad alto rischio cardiovascolare, risultati che sono stati replicati anche in contesti di pratica clinica (NCT06659744, NCT07088718).1-3 Lo studio REWIND (NCT01394952) ha dimostrato un'efficacia cardiovascolare simile per dulaglutide e ha suggerito un beneficio sia nei pazienti con che senza precedenti malattie cardiovascolari.4 Questi risultati sollevano la questione più ampia se i benefici cardiovascolari delle moderne terapie incretiniche si estendano a individui senza malattia cardiovascolare aterosclerotica (ASCVD) consolidata quando utilizzate nella pratica clinica di routine.
Per affrontare questa questione, questo studio di efficacia comparativa che utilizza una struttura di emulazione di trial target valuterà le terapie incretiniche dulaglutide, semaglutide e tirzepatide rispetto a sitagliptin (utilizzato come proxy di placebo comparatore attivo) sugli eventi avversi cardiovascolari maggiori (MACE) tra individui con diabete di tipo 2 (T2DM) e sovrappeso con o senza ASCVD.
Sebbene molte caratteristiche del trial target non possano essere replicate direttamente nei dati delle assicurazioni sanitarie, le misurazioni delle caratteristiche chiave del disegno, inclusi esiti, esposizioni e criteri di inclusione/esclusione, sono state progettate per rappresentare quelle caratteristiche del trial target. La randomizzazione non può essere ottenuta nei dati delle assicurazioni sanitarie ma è stata rappresentata attraverso un bilanciamento statistico delle covariate misurate secondo la pratica standard.
Le analisi del database saranno studi comparativi attivi su nuovi utenti, condotti utilizzando 3 database nazionali di assicurazioni statunitensi, dove confrontiamo l'effetto di dulaglutide, semaglutide e tirzepatide rispetto a sitagliptin sulla prevenzione degli eventi cardiovascolari aterosclerotici.

Le analisi del database saranno studi attivi comparativi su nuovi utenti, condotti utilizzando 3 database nazionali di richieste di risarcimento degli Stati Uniti, dove confrontiamo l'effetto di dulaglutide, semaglutide e tirzepatide rispetto a sitagliptin nella prevenzione di eventi cardiovascolari aterosclerotici.
Optum: Periodo di ammissione alla coorte idoneo dal 18 settembre 2014 al 31 agosto 2025. Marketscan: Ammissione alla coorte idonea dal 1 ottobre 2016 al 31 ottobre 2023. Medicare: Ammissione alla coorte idonea dal 18 settembre 2014 al 31 ottobre 2020.
POPOLAZIONE CON ASCVD
Criteri di inclusione:
- Storia di ASCVD (definita come IM, SCA, angina stabile/instabile, procedura di rivascolarizzazione coronarica/arteriale chirurgica o percutanea, ictus ischemico, TIA, aneurisma aortico, malattia arteriosa periferica)
- BMI >= 25,0 kg/m²
- Diabete di tipo 2
Criteri di esclusione:
- Carcinoma midollare della tiroide
- Sindrome MEN di tipo 2
- Malignità
- Diabete di tipo 1 o diabete secondario
- Malattia renale allo stadio terminale o dialisi
- Retinopatia o maculopatia diabetica non controllata
- Gravidanza
- Chirurgia bariatrica
- Uso precedente di pramlintide, o qualsiasi GLP-1RA tranne semaglutide/tirzepatide/dulaglutide iniettabile, o qualsiasi DPP4i tranne sitagliptin
- Evento o intervento cardiovascolare
- Uso concomitante di entrambi i farmaci dello studio
