米国では、非経口栄養 (PN) に依存している患者は、大豆油で構成された非経口脂肪乳剤を受け取ります。 脂質はカロリー値が高く、必須脂肪酸が含まれているため、PN 依存患者には必要です。 それらは、PN関連肝疾患の患者の素因に関与しています。 大豆油に含まれる植物ステロールなどは、胆汁分泌に有害な影響を与えると考えられています。
長期の PN を必要とする子供は、PN 関連の肝疾患を発症するリスクがあります。 オメガ 6 脂肪酸エマルジョンは脂肪酸欠乏を防ぎますが、腸カイロミクロンと同様の方法で除去されないため、肝臓に蓄積し、脂肪肝障害を引き起こすと仮定しています。 さらに、Omegaven™ などのオメガ 3 脂肪酸 (すなわち、魚油) で構成される脂肪乳剤は、デノボ脂質生成の阻害、アラキドン酸由来の炎症の減少により、脂肪肝障害の管理に有益であると仮定しています。メディエーター、少量のアラキドン酸の存在による必須脂肪酸欠乏症の予防、および血清からの脂質のクリアランスの改善. 動物研究では、エイコサペンタエン酸やドカスヘキサエン酸を多く含む魚油などの IV 脂肪乳剤 (IFE) が、従来の脂肪乳剤によって引き起こされる胆汁うっ滞で見られる胆汁の流れの障害を軽減することが示されています。 オメガ 3 脂肪酸の静脈内投与は忍容性が高く、長時間の PN 曝露による肝臓の炎症作用を軽減し、PN/IFE の使用による肝機能障害を改善する可能性があります。 従来のフィトステロール/大豆脂肪エマルジョンの代わりにOmegaven™を投与することにより、PN関連の胆汁うっ滞の進行を逆転または防止し、患者が十分な栄養を経腸的に摂取できるようになるまで、適切なPNを維持することができます.

説明
包含基準:
- -患者は非経口栄養に依存し、少なくともさらに30日間はPNが必要になると予想されます
- -患者は、少なくとも2つの連続した直接ビリルビンが2 mg / dlを超えることによって定義される非経口栄養関連肝疾患(PNALD)を持っている必要があります。
除外基準:
- 妊娠
- 肝疾患のその他の原因
- -治験薬を含む他の臨床試験への登録(集学的チームの指定された医師によって承認されない限り)
- 直接ビリルビン < 2 mg/dl
- 魚製品、卵タンパク質に対するアレルギー、および/またはオメガベンに対する以前のアレルギー
- -進行中の出血または恒常性を維持するためのFFPまたはクリオプレシピテートなどの凝固因子置換の緊急の必要性を特徴とする活動性凝固障害
- -Omegavenの開始時の血清Tgレベル> 400によって定義される脂質代謝障害
- 不安定な糖尿病
- 中心静脈カテーテルの一般的な合併症であるカテーテル関連血栓症を除く、最近の脳卒中/塞栓症。
- 崩壊と衝撃
- 不明な昏睡状態
- オメガベン開始時の未治療感染症
