せん妄は、入院中の多くの患者、特に集中治療室に影響を与える急性の錯乱状態です。 せん妄の現在の診断は、集中治療室 (CAM-ICU) タスクベースの質問票における混乱評価法を使用して行われます。 CAM-ICU の中核となる原則は不注意です; これは、患者にタスクを覚えてもらい、必要に応じて実行するように求めることでテストされます。 文字 A を言うたびにオペレーターの手を握り締め、次に CASABLANCA のスペルを入力します。
この研究の目的は、不注意との相関関係を見つけることです。 視線データは、環境に注意を払うために目が動くため、このタスクに最適です。
ベッドの端 (ヘッドカメラ) と患者の後ろ (シーンカメラ) にあるビデオカメラベースのアイトラッカーが開発されました。 ヘッドカメラは機械学習を使用して患者の目の視線を測定し、シーンカメラは患者が見ているものを見つけます。 次に、シーン カメラからシミュレーションが実行され、患者の視線が比較されて、シミュレートされたものに患者が注意を払っているかどうかが確認されます。
1 日 1 回、地元の研究チームのメンバーが、MacMurchy らの研究に基づいた検証されていない質問票に記入します。

- -ICU患者におけるDELIRiumのEarly PREdictionによって定義される予測せん妄(E-PREDELIRIC)スコア≧20%
- 予想される滞在期間は2日以上
除外基準:
- 一致しない目
- 視力障害
- 認知症
- 顔認識と視線追跡が確実に実行できない
- 署名済みの同意書/指名された相談者フォームの欠如
- CAM-ICUを確実に実施できない

患者の通常の臨床ケアは、この研究によって実施される評価の影響を受けません。
私たちの機関では、CAM-ICU は少なくとも 1 日 2 回実施されており、精神状態に変化がある場合はそれ以上の頻度で実施されています。
CAM-ICU を実行する 5 分前にシステムの電源を入れ、10 分間記録します。その後、システムの電源を切り、カメラを覆います。
この手順は、CAM-ICU が実行されるたびに発生します。
