글루타민 보충 후 중환자의 전신 염증 반응 조절
이 연구는 위독한 소아 환자의 염증 반응 조절에서 글루타민 보충의 사용을 설명하고 이 감소가 이러한 환자의 이환율과 사망률의 임상적 개선으로 이어지는지 확인하는 것을 목표로 합니다. 따라서, 이 환자들의 식단은 진화를 개선하기 위해 글루타민으로 보충될 수 있습니다.
가설:
문헌 연구에서 얻은 데이터에서 연구자들은 다음과 같이 생각합니다.
중환자는 글루타민 소비가 증가하거나 이용 가능성이 감소하여 글루타민이 부족하므로 혈중 글루타민 수치가 낮습니다.
중환자는 염증 유발 물질(IL-6)의 혈중 농도가 높습니다.
이 환자들에서 혈액 내 조직 병변 억제제(HSP-70)가 감소합니다. 이러한 환자에게 글루타민 보충제를 투여하면 HSP-70의 증가로 인해 산화 스트레스가 감소합니다.
이들 환자에서는 혈중 염증억제물질(IL-10)이 감소한다.
이 환자들에게 글루타민 보충제를 투여하면 IL-10 수치가 증가합니다.
글루타민 보충제는 IL-6 수준의 감소와 함께 염증 반응을 감소시킵니다.

목적
이 연구는 위독한 소아 환자의 염증 반응 조절에서 글루타민 보충의 사용을 설명하고 이 감소가 이러한 환자의 이환율과 사망률의 임상적 개선으로 이어지는지 확인하는 것을 목표로 합니다. 따라서, 이 환자들의 식단은 진화를 개선하기 위해 글루타민으로 보충될 수 있습니다.
최근 몇 년 동안 중환자의 염증 반응을 조절하여 이 반응이 미치는 영향과 다기관 부전으로의 진행을 줄일 수 있는 글루타민을 비롯한 다양한 요인에 대한 수많은 연구가 수행되고 발표되었습니다.
글루타민(Gln)은 체내에서 가장 풍부한 아미노산이며 주로 골격근에서 합성됩니다. 건강한 상태에서 충분히 생산되는 비필수아미노산입니다. 혈장 수치는 0.6mmol/L 이상이며 50%는 혈장1에서 유리 형태로 발견됩니다. 이 아미노산은 에너지원으로 작용할 뿐만 아니라 다른 아미노산, 뉴클레오티드, 핵산, 당, 아민, 단백질 및 다양한 생물학적 활성 분자의 합성에도 관여합니다2. 다른 기능은 다음과 같습니다: 내부 산-염기 항상성 유지, 요소 합성, 당신생합성, 신경전달, 세포 분화 및 증식. 또한 빠르게 증식하는 세포(장세포)와 여러 면역 세포(대식세포, 단핵구, 림프구)의 주요 에너지 기질이기도 합니다. 또한 열충격 단백질3의 발현을 유도하는 세포 및 조직 보호에 참여합니다.
최근 몇 년 동안 장내 및 비경구 경로 모두에서 중환자의 진화에 대한 Gln의 영향을 결정하기 위해 수많은 연구가 수행되었습니다. 이 연구는 동물과 인간, 주로 성인에서 수행되었습니다. 그러나 문헌에는 어린이 연구에 대한 언급이 거의 없습니다. 이 연구는 주로 신생물성 질환 또는 염증성 장 질환 환자에서 식이 보충제로 글루타민을 사용하는 것을 기반으로 합니다. 미숙아의 보충제로도 연구되었지만 중환자에 대한 연구는 거의 없습니다.
작년에 글루타민 사용에 관한 많은 연구가 발표되었습니다. 출혈성 쇼크 상태의 토끼에서 장 점막 장벽에 대한 글루타민 보충의 효과를 연구했습니다. 대퇴 동맥에서 혈액이 빠져나감으로써 쇼크가 유발되었습니다. 토끼는 세 그룹(대조군, 저용량 글루타민 및 고용량 글루타민)으로 무작위 배정되었습니다. diamine oxidase와 IL-8의 혈장 수치를 측정하고 말단 회장의 조직학적 검사를 수행했습니다. 그 결과 Gln 보충을 받은 토끼에서 낮은 염증 및 산화 반응이 나타났습니다37. 쥐를 대상으로 한 또 다른 연구에서는 디펩티드 아르기닌 - Gln이 망막 색소 상피 세포 배양에서 내피 세포 성장 인자 수준에 미치는 영향과 산소 유발 망막병증에서 신생혈관 형성 억제에 미치는 영향을 측정했습니다. 저자는 이 dipeptide38 투여로 감소했다고 결론지었습니다. 중환자에서 연구된 또 다른 요인은 diamine oxidase 활성과 D-lactate 함량으로 측정된 산화 활성이었습니다39. 감염에 대한 보호 및 중환자의 인슐린 저항성 감소는 여전히 연구 중입니다 21,22, 32, 40, 41. 상반된 결과가 보고되었으므로 체계적인 검토에서 새로운 연구가 필요합니다.

제외 기준:
- 법정대리인이 동의하지 않습니다.
- 기저질환(신장장애, 간장애, 염증성 장질환, 류마티스성 질환, 대사성 질환, 면역저하자)이 있는 자.
- 입원 시 경미한 간 장애(간염, 결장정체).
- 체외 순환을 이용한 심장 수술 후.
- 48시간 이상의 임상적 진전이 있는 다른 병원에서 의뢰된 환자.
- 1개월 미만 및 14세 이상의 환자.
