경도인지장애 노인의 전산화 인지훈련 효과: 임상적 결과와 자기뇌파영상
경도 인지 장애(MCI)는 알츠하이머병 또는 기타 치매가 나타나기 전에 인지 기능이 감소하는 전조 증상으로 언어 장애, 시공간 관계 장애, 주의력 장애, 기억력 장애, 일상 생활의 도구적 활동(IADL)이 영향을 받습니다. MCI는 정상적인 노화에서 경도 치매로 넘어가는 과도기로 간주되며, MCI 환자는 정상 인지에서 MCI로 또는 치매로 발전하는 등 일정 기간 동안 인지 기능이 다르게 변동합니다. MCI로 발달한 정상적인 인지를 가진 노인의 연간 전환율(ACR)은 임상 환경에서 30%, 지역사회에서 각각 5%입니다. MCI가 있는 모든 환자가 알츠하이머병으로 발전하는 것은 아니며, MCI가 있는 환자의 정상 인지로의 복귀가 존재합니다. 그러나 MCI는 중요한 위험 요소입니다. 정상인지 또는 치매로 진행된 MCI를 가진 노인의 ACR은 각각 1-2% 및 5-15%입니다. 또한 MCI 환자의 약 절반이 5년 내에 치매로 발전했습니다.
인지 훈련(CT)은 기억력, 주의력 또는 문제 해결과 같은 특정 인지 기능의 표준화된 인지 작업을 반복적으로 연습하여 인지 기능을 향상시킵니다. CT는 인지 운동을 포함하는 전략 훈련, 인지 장애를 줄이고 성능을 향상시키기 위한 전략을 표시하고 실행하는 것을 포함하여 널리 정의되었습니다. CT는 MCI에 더 효과적입니다. 최근에는 많은 장점을 가진 컴퓨터 기반 CT(CCT)가 전통적인 종이-연필 형태를 점차 대체하고 있습니다. 간략한 체계적 검토는 컴퓨터 기반 개입이 MCI 및/또는 치매 환자의 우울증 및 불안과 같은 행동 증상에 긍정적인 영향을 미쳤다는 것을 보여주었습니다. 이전 연구에서는 컴퓨터 기반 개입이 MCI 환자의 전반적인 인지 기능에 중간 정도의 치료 효과를 보였고 학습, 단기 기억 및 행동 증상에도 긍정적인 영향을 미쳤음을 입증했습니다.
인지 장애가 있는 노인은 세계적인 고령화로 인해 증가할 것으로 예상됩니다. MCI가 있는 노인의 인지 기능을 개선하거나 유지하는 데 중요합니다. MCI가 있는 노인의 인지 기능, 신경 정신병 증상, 일상 기능 및 뇌자기 뇌파 검사(MEG)의 뇌 활성화 영상에 대한 CCT의 효능은 바쁜 임상 실습을 위해 탐구할 가치가 있습니다.
연구 개요
상세 설명
연구 설계는 전향적이고 단일 맹검 무작위 통제 시험이었습니다. MCI 참가자 36명을 모집하고 인구 통계학적 데이터(연령, 성별, 교육 수준/교육 기간, 결혼 상태 등)도 수집했습니다. MCI 참가자는 신경심리학적 평가 및 자기 공명 영상 또는 전산화 단층 촬영을 포함하여 기준선에서 포괄적인 검토를 받았습니다. 무작위 치료 할당은 난수 테이블에 의해 생성되고 그에 따라 환자의 개입 그룹을 할당합니다. CCT 그룹이 포함된 봉인된 불투명 봉투 또는 용량이 일치하는 대조군 시트가 준비되어 치료사에게 제공됩니다.
지수 d 1.00의 t-검정 효과 크기를 기반으로 0.05의 양면 제1종 오류가 있는 0.80의 검정력에 대해 각 그룹의 약 17명의 참가자가 필요합니다. 5%(q) 드롭률을 고려하여 각 그룹에 대해 18명의 참가자(N/1-q)를 모집해야 합니다. 참가자들은 CCT 그룹(참가자 18명) 또는 복용량이 일치하는 대조군(참가자 18명)에 무작위로 할당되었으며, 하루 30분 동안, 주 3회, 연속 4주 동안 개별 중재를 받았습니다. 추가 분석을 위해 치료 전, 치료 후 및 1개월 추적 관찰 시 임상 결과 측정 및 MEG 영상화를 실시했습니다.
통계 분석은 SPSS(Statistical Package for the Social Sciences) 버전 19.0을 사용하여 수행되었습니다. 통계 테스트에서 양측 p 값 ≤ 0.05는 통계적으로 유의한 것으로 간주되었습니다. 연속형 변수의 분포 특성은 평균±표준편차(SD)로, 범주형 변수는 빈도와 백분율로 나타내었다. 처리군과 대조군의 범주형 변수인 연속변수 분포의 차이는 two-sample t-test, Wilcoxon rank-sum test(또는 Mann-Whitney U test), 카이제곱 검정을 이용하여 조사하였다. 또한 최소 규범 추정치(MNE), 소스 기반 시간-주파수 분석, 교차 주파수 결합 및 기능적 연결성을 사용하여 서로 다른 치료 그룹 간의 참가자의 뇌 기능 활성화 차이를 탐색했습니다. t-test는 MEG의 자극 반응 검사에서 반응 시간, 교정 속도 및 물리적 신호의 차이를 다른 치료 그룹 간의 참가자 간에 탐색하는 데 사용되었습니다. 상관 통계는 결과 측정 점수와 물리적 신호 간의 관계를 탐색하는 데 사용되었습니다.

- (2) 국립노화연구소-알츠하이머 협회 워크그룹의 알츠하이머병 진단 지침을 사용하여 기억소거 유형 MCI를 만들었으며, 임상 치매 평가 척도(CDR) 글로벌 점수는 0.5입니다.
- (3) 명령을 따르고, 평가 내용을 이해하고, 구두 의사소통을 통해 치료 개입에 협력할 수 있습니다.
제외 기준:
- (1) 우울증 상태를 나타내는 GDS-SF(Geriatric Depression Scale-Short Form) > 7의 점수를 가짐
- (2) Barthel 지수(BI)≠100의 점수를 가지고, 의존적 기초 일상생활 활동을 나타냄
- (3) 전형적으로 다른 MCI 증후군을 나타내는 전측두엽 치매 또는 루이소체를 포함한 다른 MCI 하위 유형으로 진단되었습니다.
- (4) 기타 신경정신병적 질환이 있는 경우
- (5) MEG로 관리할 수 없음
- (6) 심각한 건강 문제로 인해 참가할 수 없었습니다.

두 개의 자극(S1, S2) 중 동일한 것이 500ms 간격 내에서 나타나며 두 번째 자극(S2)의 유도 반응의 크기는 정상적인 신체 조건에서 억제됩니다.
