상세 설명
하지 절단 후 성인의 최대 48%가 다가오는 해에 사망할 것입니다. 신체 활동(운동)의 부족은 높은 사망률에 기여합니다. 만성 질환의 위험을 줄이기 위해 건강 상태가 없는 젊은(및 중년) 성인은 하루 10,000보를 걷는 것이 권장되며, 노인과 만성 질환이 있는 사람은 하루 7,100보를 권장합니다. 하지 절단이 있는 성인은 하루 권장 걸음 수인 7,100보의 약 21~43%를 걷습니다. StepWatch(연구 등급 가속도계) 및 FitBit(시중에서 판매되는 모니터)과 같은 걸음 활동 모니터는 신체 활동을 평가하는 수단을 제공하며, 이는 환자가 자신의 신체 활동 수준을 과대 평가하는 경향이 있기 때문에 중요합니다. 그러나 활동 모니터 사용은 모니터 비용이 $100-$600일 수 있고 누군가 모니터에서 데이터를 제거하고 데이터를 해석하고 데이터를 환자의 의료 기록에 입력해야 하기 때문에 모든 의료 진료 환경에서 가능하지 않을 수 있습니다. 따라서 환자가 자신의 신체 활동을 자가 보고하는 신체 활동 설문지는 임상 환경에서 이상적입니다. 불행하게도 지금까지 하지 절단 환자의 신체 활동 설문지의 정확성과 이러한 설문지가 걸음 수 활동 모니터에서 얻은 데이터까지 어떻게 측정되는지 조사한 연구는 거의 없었습니다.
하루 7,100걸음 외에도 이동 제한이 있는 만성 질환이 있는 성인(하지 절단 환자 포함)은 주당 150분 이상의 중간 강도 신체 활동에 참여해야 하며 한 번에 10분 이상 활동해야 합니다. 중등도 활동은 3가지 대사 당량(MET)으로 정의됩니다. 1MET는 안정 시 소비하는 산소량과 같으므로 3MET는 안정 시 소비하는 산소량의 3배를 소비한다는 의미입니다. 의학적 상태가 없는 건강한 성인의 경우 이것은 2.6-2.7 mph 또는 100보/분에 해당하는 반면, 무릎 아래 다리가 절단된(즉 경골절단) 성인의 경우 1.47 mph 또는 86보/분에 해당합니다. 3MET와 동일한 것으로 보고되었습니다. 하지 절단이 있는 성인, 특히 혈액 순환이 좋지 않아 사지를 잃은 성인의 최대 보행 속도는 ≤1.67mph일 수 있으므로 이러한 환자가 장시간 동안 1.47mph를 걷는 것이 가능한지 질문해야 합니다. 수사관들은 당뇨병 및 말초 혈관 질환과 같은 다른 의학적 문제가 있고 신체 활동에 참여하지 않고 신체 활동에 참여하지 않는 하지 절단이 있는 성인의 경우 1.47mph가 3 MET보다 크다고 생각합니다.
하지 절단이 있고 현재 비활성 상태인 다른 의학적 상태가 있는 성인의 속도(mph) 및 케이던스(걸음/분)가 3 MET와 같은지 아는 것이 중요합니다. 조사관은 이러한 환자가 의료 서비스 제공자가 접하고 신체 활동 수준을 높이는 것과 관련하여 협의해야 하는 환자의 대다수를 대표한다고 생각합니다. 제공자는 다른 의학적 상태로 사지 손실이 있는 비활성 환자의 경우 3 MET와 동일한 속도와 케이던스를 알아야 제공자가 환자에게 적절하게 조언하고 운동을 처방할 수 있습니다.
걷기 운동은 지상 또는 런닝머신에서 할 수 있습니다. 하지 절단이 있는 개인이 지상을 걸을 때 오른쪽에서 왼쪽으로 걷는 패턴은 런닝머신에서 걸을 때와 비교할 때 더 비대칭(불균일)합니다. 걷는 것이 좌우 대칭(균등)일 때 환자는 더 많은 에너지를 소비해야 할 수 있습니다. 에너지 소비량은 걷는 동안 참가자의 호흡을 평가하는 마스크를 착용하는 동안 얻을 수 있는 소비된 산소의 양으로 평가할 수 있습니다. 하지 절단이 있는 성인의 에너지 소비 연구에서는 일반적으로 에너지 소비를 살펴보기 위해 러닝머신을 사용했습니다. 그러나 지상 대 러닝머신 조건은 다르므로 각 보행 조건에서 3 MET(속도, 케이던스)에 해당하는 값이 다를 수 있습니다.
또한 하지 절단으로 비활동적인 성인의 걷기 에너지 비용을 절단하지 않은 성인과 연령, 성별 및 체질량 지수(신장 및 체중에서 계산)와 일치하는 성인과 비교한 연구는 없습니다. 연구에서 절단된 사지의 노화와 사지 길이가 에너지 소비에 미치는 영향을 평가했지만, 하지 절단이 있는 성인의 에너지 소비에 영향을 줄 수 있는 수정 가능한 요인을 평가한 연구는 거의 없습니다. 마찬가지로, 신체 활동 수준에 영향을 줄 수 있는 수정 가능한 요인을 조사한 연구는 거의 없습니다.
이 프로젝트의 목표는 단일 무릎 아래 절단(예: 경골 절단).
프로젝트의 성공적인 완료는 의료 제공자에게 (1) 하지 절단 환자의 임상 실습에서 사용할 수 있는 신체 활동 자가 보고 측정, (2) 중강도 이상 처방을 위한 보행 속도(mph)를 제공할 수 있습니다. -하지 절단 환자의 재활 중 그라운드 및 러닝머신 걷기 및 (3) 하지 절단이 있는 비활성 성인을 위한 단계 활동 모니터를 통해 중간 강도 신체 활동을 평가 및 모니터링하기 위한 케이던스(걸음/분) .
조사관은 건강한 성인과 비교하여 짧은 거리를 걷기 위해 보철물을 사용하는 하지 절단이 있는 비활성 성인에게 필요한 추가 에너지 소비를 보는 첫 번째 객관적 데이터를 제공할 것입니다. 이 데이터는 에너지 소비를 줄이는 미래의 보철 구성 요소 개발에 사용될 수 있습니다. 조사관은 하지 절단이 있거나 없는 성인의 에너지 소비 및 신체 활동과 관련될 수 있는 요인을 조사할 것입니다.
