Gedetailleerde beschrijving
Dit is een prospectieve, multicenter, multi-cohort, niet-gerandomiseerde fase 2-studie bij maximaal 40 volwassen patiënten met een levend of ledemaatbedreigend vasculair trauma dat chirurgisch herstel vereist. Er zal een ledemaatcohort en een torsocohort zijn. Het ledemaatcohort omvat patiënten die reparatie van een bloedvat in de bovenste of onderste extremiteit nodig hebben. Het torso-cohort omvat patiënten die reparatie van vaten in de thorax (exclusief het hart), de buik en het retroperitoneum nodig hebben. Proefpersonen zullen worden geïmplanteerd met een Humacyte Human Acellular Vessel (HAV) als tussenliggend vat of bypass met behulp van standaard vasculaire chirurgische technieken. Er is geen bedieningsarm.
De actieve studieduur voor elke studiedeelnemer is 36 maanden vanaf HAV-implantatie of tot HAV-falen/verwijdering/overlijden indien eerder. Follow-up na maand 12 omvat het vastleggen van informatie over beoordelingen die zijn uitgevoerd tijdens standaardbezoeken aan de kliniek of telefonische follow-up met de patiënt of zijn/haar arts met lichamelijk onderzoek en echografie in maand 24 en maand 36

Beschrijving
Inclusiecriteria:
- Patiënten met levensbedreigend traumatisch letsel aan een arterieel vat in de ledemaat of torso, anders dan het hart, dat vervanging of reconstructie vereist
- Preoperatieve beeldvorming of klinisch onderzoek geeft aan dat het beschadigde bloedvat een defectlengte heeft van ≤ 38 cm en de juiste maat heeft die overeenkomt met het 6 mm menselijke acellulaire vat (HAV) volgens het oordeel van de behandelend chirurg, rekening houdend met vasoconstrictie en situationele instroom- en uitstroomoverwegingen.
- Autologe veneuze transplantatie is ofwel niet haalbaar naar het oordeel van de behandelend chirurg (bijv. wegens gebrek aan beschikbaarheid van geschikte leiding, aanwezigheid van ernstige veneuze insufficiëntie) of is niet wenselijk vanwege de urgentie van revascularisatie
- In de leeftijd van 18 tot en met 85 jaar
- In staat om zinvol te communiceren met onderzoekspersoneel en in staat om volledige studieprocedures na te leven. Als de patiënt bewusteloos is, geeft informatie van een betrouwbare getuige aan dat de patiënt normaal gesproken in staat zou zijn om te voldoen aan de onderzoeksprocedures
- Patiënt of familielid is in staat, bereid en competent om geïnformeerde toestemming te geven
- Levensverwachting van minimaal 1 jaar
Uitsluitingscriteria:
- Verminkte Extremiteit Severity Score (MESS) van ≥ 7
- Ledematen lopen een hoog risico op amputatie ondanks vasculaire reconstructie (bijvoorbeeld vanwege verbrijzelingsletsel)
- Catastrofale verwondingen die overleven onwaarschijnlijk maken (bijv. Verkorte Injury Scale (AIS) > 5 of Injury Severity Score (ISS) >60)
- HAV mag niet worden gebruikt voor herstel van kransslagaders
- Bekende zwangere vrouwen
- Bekende medische aandoening die langdurige plaatjesaggregatieremmende therapie uitsluit na herstel van acuut letsel
- Elke andere aandoening die naar het oordeel van de onderzoeker een adequate evaluatie van de veiligheid en werkzaamheid van de HAV in de weg zou staan
- Eerdere blootstelling aan HAV
- Bekende deelname aan een onderzoek in de afgelopen 30 dagen
- Werknemers van de sponsor of patiënten die werknemers of familieleden zijn van de onderzoeker

Experimenteel: Acellular Tissue Engineered Vessel (ATEV)
Patients with life or limb threatening traumatic injury to an arterial vessel in the limb or torso, other than the heart, will be implanted with the Humacyte ATEV as an interposition vessel or bypass using standard vascular surgical techniques.
|
The investigational medicinal product (IMP) - the Acellular Tissue Engineered Vessel (ATEV) is a sterile acellular tubular graft composed of human collagen types I and III and other extracellular matrix proteins, including fibronectin and vitronectin which can be used for arterial bypass or reconstruction in patients with life or limb threatening vascular trauma.
The vessel is 6 mm in diameter and approximately 42 cm in length.
The product is supplied on a silicone mandrel immersed in sterile phosphate buffered saline in a sealed and labeled plastic container.
The ATEV is implanted using standard vascular surgical techniques similar to placement of predicate peripheral vascular prostheses.
