Szczegółowy opis
Jest to prospektywne, wieloośrodkowe, wielokohortowe, nierandomizowane badanie fazy 2 z udziałem maksymalnie 40 dorosłych pacjentów z zagrażającymi życiu lub kończynami urazami naczyniowymi wymagającymi leczenia chirurgicznego. Będzie kohorta kończyn i kohorta tułowia. Kohorta kończyn będzie obejmować pacjentów, którzy wymagają naprawy naczynia znajdującego się w kończynie górnej lub dolnej. Kohorta tułowia obejmuje pacjentów wymagających naprawy naczyń w obrębie klatki piersiowej (z wyłączeniem serca), jamy brzusznej i przestrzeni zaotrzewnowej. Osobnikom zostanie wszczepiony ludzki naczynie bezkomórkowe (HAV) Humacyte jako naczynie pośrednie lub obejście przy użyciu standardowych technik chirurgii naczyniowej. Nie ma ramienia sterującego.
Czas trwania aktywnego badania dla każdego uczestnika badania będzie wynosił 36 miesięcy od wszczepienia HAV lub do niepowodzenia/usunięcia/śmierci HAV, jeśli nastąpi wcześniej. Obserwacja po 12 miesiącu będzie obejmowała zebranie informacji na temat ocen przeprowadzonych podczas rutynowych wizyt w klinice „standardowej opieki” lub telefonicznej wizyty kontrolnej u pacjenta lub jego/jej lekarza z badaniem fizykalnym i USG w 24 i 36 miesiącu
Całkowity przewidywany czas trwania badania klinicznego wynosi 61 miesięcy (24 miesiące rejestracji i 36 miesięcy obserwacji).

Kryteria przyjęcia:
- Pacjenci z zagrażającym życiu lub kończyną urazowym uszkodzeniem naczynia tętniczego kończyny lub tułowia, innym niż serce, wymagającym wymiany lub rekonstrukcji
- Przedoperacyjne obrazowanie lub badanie kliniczne wskazuje, że uszkodzone naczynie ma ubytek o długości ≤ 38 cm i jest odpowiednio dopasowane rozmiarem do ludzkiego naczynia bezkomórkowego (HAV) o średnicy 6 mm, zgodnie z oceną chirurga prowadzącego, biorąc pod uwagę zwężenie naczyń oraz okoliczności związane z dopływem i odpływem.
- Autologiczny przeszczep żyły jest albo niewykonalny w ocenie chirurga prowadzącego (np. z powodu braku odpowiedniego przewodu, obecności ciężkiej niewydolności żylnej) albo nie jest pożądany ze względu na pilną potrzebę rewaskularyzacji
- Wiek od 18 do 85 lat włącznie
- Zdolny do porozumiewania się w znaczący sposób z personelem dochodzeniowym i zdolny do przestrzegania całych procedur badawczych. Jeśli pacjent jest nieprzytomny, informacje pochodzące od wiarygodnego świadka wskazują, że w normalnych warunkach pacjent byłby w stanie zastosować się do procedur badania

Kryteria wyłączenia:
- Skala ciężkości zniekształconych kończyn (MESS) ≥ 7
- Kończyna o wysokim ryzyku amputacji pomimo rekonstrukcji naczyń (np. z powodu urazu zmiażdżeniowego)
- Katastrofalne obrażenia, które sprawiają, że przeżycie jest mało prawdopodobne (np. Skrócona Skala Urazów (AIS) > 5 lub Skala Ciężkości Urazów (ISS) > 60)
- HAV nie może być stosowany do naprawy tętnic wieńcowych
- Znane kobiety w ciąży

Eksperymentalny: Acellular Tissue Engineered Vessel (ATEV)
Patients with life or limb threatening traumatic injury to an arterial vessel in the limb or torso, other than the heart, will be implanted with the Humacyte ATEV as an interposition vessel or bypass using standard vascular surgical techniques.
|
The investigational medicinal product (IMP) - the Acellular Tissue Engineered Vessel (ATEV) is a sterile acellular tubular graft composed of human collagen types I and III and other extracellular matrix proteins, including fibronectin and vitronectin which can be used for arterial bypass or reconstruction in patients with life or limb threatening vascular trauma.
The vessel is 6 mm in diameter and approximately 42 cm in length.
The product is supplied on a silicone mandrel immersed in sterile phosphate buffered saline in a sealed and labeled plastic container.
The ATEV is implanted using standard vascular surgical techniques similar to placement of predicate peripheral vascular prostheses.
