Это проспективное, многоцентровое, многогрупповое, нерандомизированное исследование 2 фазы с участием до 40 взрослых пациентов с угрожающей жизни или конечностям сосудистой травмой, требующей хирургического вмешательства. Будет когорта конечностей и когорта туловища. Когорта конечностей будет включать пациентов, которым требуется восстановление сосуда верхней или нижней конечности. Когорта туловища включает пациентов, которым требуется восстановление сосудов грудной клетки (за исключением сердца), брюшной полости и забрюшинного пространства. Субъектам будет имплантирован человеческий бесклеточный сосуд (HAV) Humacyte в качестве промежуточного сосуда или шунта с использованием стандартных сосудистых хирургических методов. Нет рычага управления.
Активная продолжительность исследования для каждого участника исследования будет составлять 36 месяцев с момента имплантации ВГА или до отказа/удаления/смерти ВГА, если ранее. Последующее наблюдение после 12-го месяца будет включать в себя сбор информации об оценках, проведенных во время обычных визитов в клинику «стандартного лечения» или по телефону с пациентом или его / ее врачом с физическим осмотром и ультразвуковым исследованием на 24-м и 36-м месяцах.
Общая ожидаемая продолжительность клинического исследования составляет 61 месяц (24 месяца регистрации и 36 месяцев наблюдения).

Описание
Критерии включения:
- Пациенты с угрожающим жизни или конечности травматическим повреждением артериального сосуда конечности или туловища, кроме сердца, которое требует замены или реконструкции

- В возрасте от 18 до 85 лет включительно

- Пациент или родственник может, желает и компетентен дать информированное согласие
- Ожидаемая продолжительность жизни не менее 1 года
Критерий исключения:
- Оценка тяжести искривленных конечностей (MESS) ≥ 7
- Конечность с высоким риском ампутации, несмотря на сосудистую реконструкцию (например, из-за размозжения)
- Катастрофические травмы, делающие выживание маловероятным (например, Сокращенная шкала травм (AIS) > 5 или оценка тяжести травм (ISS) > 60)
- HAV нельзя использовать для восстановления коронарных артерий.

Экспериментальный: Acellular Tissue Engineered Vessel (ATEV)
Patients with life or limb threatening traumatic injury to an arterial vessel in the limb or torso, other than the heart, will be implanted with the Humacyte ATEV as an interposition vessel or bypass using standard vascular surgical techniques.
|
The investigational medicinal product (IMP) - the Acellular Tissue Engineered Vessel (ATEV) is a sterile acellular tubular graft composed of human collagen types I and III and other extracellular matrix proteins, including fibronectin and vitronectin which can be used for arterial bypass or reconstruction in patients with life or limb threatening vascular trauma.
The vessel is 6 mm in diameter and approximately 42 cm in length.
The product is supplied on a silicone mandrel immersed in sterile phosphate buffered saline in a sealed and labeled plastic container.
The ATEV is implanted using standard vascular surgical techniques similar to placement of predicate peripheral vascular prostheses.
