Effect van amnio-infusie op externe cephalische versie na initiële mislukking, een prospectieve multicentrische gerandomiseerde studie
Het doel van de studie is om het effect te evalueren van transabdominale amnio-infusie vóór de tweede externe cephalische versie na initiële mislukking. Patiënt met een enkele foetus, op termijn, in stuitligging en na een eerste cephalische versie zijn opgenomen. De randomisatie bepaalt of de patiënt wordt opgenomen in de groep met amnio-infusie vóór de tweede cephalische versie of met alleen de gebruikelijke tweede cephalische versie; De beoordeling van de werkzaamheid is gebaseerd op het aantal cefalische presentaties bij de geboorte. Het slagingspercentage van de cephalische versie met of zonder transabdominale amnio-infusie, het aantal keizersneden in de twee groepen, de morbiditeit van moeder en foetus, de tijd tussen de tweede cephalische versie en de geboorte zullen worden bestudeerd.
Prospectieve interventionele gerandomiseerde sequentiële vergelijkende multicentrische studie. Er worden maximaal 240 patiënten opgenomen (120 in elke groep). Aangezien de studie sequentieel is, zal deze worden beëindigd zodra een significant verschil wordt aangetoond.

Het doel van de studie is om het effect te evalueren van transabdominale amnio-infusie vóór een tweede externe cephalische versie na aanvankelijk falen. Bij succes kan het een alternatief zijn voor een keizersnede. Zonder enige andere techniek is de foetus in cephalische presentatie na een eerste externe cephalische versie in 50%.
Patiënten met een enkele foetus, op termijn, in stuitligging worden voorgesteld om deel te nemen. De randomisatie vindt plaats bij de inclusie en bepaalt of de patiënt wordt opgenomen in de groep met amnio-infusie vóór de tweede cephalische versie of alleen de gebruikelijke tweede cephalische versie. Het enige verschil tussen de twee groepen is de realisatie van een vruchtwaterinfusie, de techniek van de cephalische versie is hetzelfde.
De evaluatie van de werkzaamheid is gebaseerd op de mate van cephalische presentatie bij de geboorte. Het slagingspercentage van de cephalische versie, het aantal keizersnedes, de maternale en foetale morbiditeit in de twee groepen, de tijd tussen de tweede cephalische versie en de geboorte zullen worden bestudeerd.

Inclusiecriteria:
- een enkele foetale zwangerschap
- stuitligging
- op termijn
- initiële mislukking van externe cephalische versie
- structureel normale foetus
Uitsluitingscriteria:
- polyhydramnio's
- anhydramnion
- afwijking van het foetale hartritme
- baarmoeder aangeboren misvorming
- keizersnede bij een vorige bevalling
- onvermogen om studie te begrijpen
