Een hydromorfon farmacokinetisch-farmacodynamische vingerafdruk met hoge resolutie als basis voor het identificeren van sekseverschillen in de farmacokinetiek en farmacodynamiek van opioïden

Na 6 uur vasten krijgt elke vrijwilliger een 20G arteriële lijn in de radiale slagader voor vroege bloedafname en een 18 G perifere intraveneuze katheter in de contralaterale onderarm voor medicijntoediening en later bloedafname. Continu bewaakte vitale functies omvatten ECG, invasieve bloeddruk, hemoglobine, O2-verzadiging, end-tidal CO2 en ademhalingsfrequentie (van het capnogram) geregistreerd.
Na het verzamelen van baseline PD-gegevens wordt gedurende 10 seconden een bolus van 0,2 mg/kg hydromorfon toegediend via de vrij stromende perifere infuus (t=0) en worden arteriële bloedmonsters van 3 ml verkregen op 0,25, 0,5, 0,75, 1, 1,25, 1,5, 1,75 en 2 min met behulp van een kraan en verdeelstuksysteem. Daaropvolgende bloedmonsters worden verkregen op 3, 4, 5, 7,5, 10, 15, 20, 25, 30 en 45 min en 1, 1,25, 1,5, 2, 2,25, 2,5, 2,75, 3, 3,5, 4, 4,5 , 5, 6, 7, 8, 10, 12, 16, 20 en 24 uur. Hoewel het EEG continu wordt verkregen, worden de overige farmacologische gegevens geregistreerd op discrete tijdstippen s in de eerste 5 min: pupillometrie op 1, 2 en 5 min; ventilatie op 2 min; temperatuuranalgesie na 3 en 5 minuten en sedatieniveau na 4 minuten. Hierdoor kunnen de ventilatie en pupillometrie in een rusttoestand worden verkregen, waardoor vervorming van deze reacties door stimulatie wordt beperkt. Vervolgens worden alle gegevens verzameld op alle PK-tijdstippen in de volgende volgorde: ventilatie en EEG (gelijktijdig), pupillometrie, gemodificeerde OAA/S-score en temperatuuranalgesie. Na 2 uur, zodra een farmacologische parameter is teruggekeerd naar de basislijn voor 2 opeenvolgende metingen, wordt de opname van die parameter gestopt. Als de vrijwilliger tijdens het onderzoek de apparaattrigger niet kan gebruiken vanwege opioïde-geïnduceerde sedatie, wordt het tolerantieniveau voor verhoogde temperatuur gedefinieerd als de temperatuur waarbij de vrijwilliger een terugtrekkingsbeweging van het geteste ledemaat vertoont. Zodra alle data-acquisitie is voltooid, mag de vrijwilliger heldere vloeistoffen drinken. Vervolgens wordt het dieet voortgezet zoals getolereerd. De vrijwilliger wordt elk uur gecontroleerd (levensfuncties) in de Clinical Research Unit totdat alle bloedmonsters zijn verkregen.

Beschrijving
Inclusiecriteria:
- binnen 20% van hun ideale lichaamsgewicht
- 21-30 jaar oud
- ASA I (geen systemische ziekte)
- Geen geschiedenis van PONV (behalve extractie van verstandskiezen)
- Geen langdurig medicatiegebruik
- Geen voorgeschiedenis van stollingsdefecten (d.w.z. gemakkelijk blauwe plekken krijgen, bloedend tandvlees bij tandenpoetsen, frequente neusbloedingen, in het verleden gedocumenteerde coagulopathie, enz.)
Uitsluitingscriteria:
- Onvermogen om een arteriële lijn te plaatsen
- Een mislukte urine-drugstest bij opname in de CRU
- Een positieve zwangerschapstest bij opname in de CRU
- Een hemoglobinegehalte < 12,5 g/dL bij opname in de CRU
