Statinetherapie om cognitieve disfunctie na hartchirurgie te beperken

Het primaire doel van onze prospectieve, gerandomiseerde, dubbelblinde interventionele klinische studie is het bepalen van de effectiviteit van een hoge dosis atorvastatinetherapie om postoperatieve cognitieve disfunctie (POCD) te verminderen bij patiënten die een hartklepoperatie ondergaan.
Onze hypothese is dat therapie met hoge doses atorvastatine de incidentie en/of ernst van POCD aanzienlijk zal verminderen.

Deelname Criteria
Onderzoekers zoeken naar mensen die aan een bepaalde beschrijving voldoen, de zogenaamde geschiktheidscriteria. Enkele voorbeelden van deze criteria zijn iemands algemene gezondheidstoestand of eerdere behandelingen.

Inclusiecriteria:
- Vijftig geïnformeerde en instemmende patiënten voor hartklepchirurgie met CPB, ouder dan 50 jaar en onder de hoede van een of meer van de onderzoekers of adviseurs zullen prospectief worden ingeschreven over een periode van een jaar (plus een jaar voor follow-up ).
Uitsluitingscriteria:
Patiënten met een voorgeschiedenis van de volgende ziekte-entiteiten worden uitgesloten:
- symptomatische cerebrovasculaire ziekte met substantieel residueel tekort, alcoholmisbruik, psychiatrische ziekte, nierfalen (creatinine>2.0),
- leverdisfunctie (ALAT of ASAT > 1,5 x ULN), voorgeschiedenis van allergie of myopathie bij statinetherapie, zwangere vrouwen of vrouwen die borstvoeding geven of andere comorbiditeiten die naar de mening van de onderzoeker de deelname van patiënten kunnen beperken. Uitsluitingscriteria zijn ontworpen om confounding te beperken en de differentiatie van tekorten gerelateerd aan chirurgie en/of behandeling te verbeteren.
- Ook patiënten die niet kunnen lezen en dus niet in staat zijn om de neurocognitieve test af te ronden, worden uitgesloten.
- Patiënten die gelijktijdig een carotis-endarteriëctomie of andere vasculaire chirurgie ondergaan, zullen ook worden uitgesloten om een meer homogeen monster ter vergelijking te bieden.
