Terapia con statine per limitare la disfunzione cognitiva dopo cardiochirurgia

L'obiettivo principale del nostro studio clinico interventistico prospettico, randomizzato, in doppio cieco è determinare l'efficacia della terapia con atorvastatina ad alte dosi per ridurre la disfunzione cognitiva postoperatoria (POCD) nei pazienti sottoposti a chirurgia valvolare cardiaca.
Ipotizziamo che la terapia con alte dosi di atorvastatina ridurrà significativamente l'incidenza e/o la gravità della POCD.

Descrizione
Criterio di inclusione:
- Cinquanta pazienti informati e consenzienti per chirurgia valvolare cardiaca con CPB, di età superiore a 50 anni e sotto la cura di uno o più ricercatori o consulenti saranno arruolati in modo prospettico per un periodo di un anno (più un anno per il follow-up ).
Criteri di esclusione:
Saranno esclusi i pazienti con una storia delle seguenti entità patologiche:
- malattia cerebrovascolare sintomatica con sostanziale deficit residuo, abuso di alcol, malattia psichiatrica, insufficienza renale (creatinina> 2,0),
- disfunzione epatica (ALT o AST> 1,5 x ULN), anamnesi di allergia o miopatia con terapia con statine, donne in gravidanza o in allattamento o altre comorbilità che, secondo l'opinione dello sperimentatore, potrebbero limitare la partecipazione del paziente. I criteri di esclusione sono progettati per limitare il confondimento e migliorare la differenziazione dei deficit correlati alla chirurgia e/o al trattamento.
- Saranno esclusi anche i pazienti incapaci di leggere e quindi impossibilitati a completare il test neurocognitivo.
- Saranno esclusi anche i pazienti sottoposti a concomitante endoarterectomia carotidea o altra chirurgia vascolare per fornire un campione più omogeneo per il confronto.
