Terapia com estatina para limitar a disfunção cognitiva após cirurgia cardíaca

O objetivo principal de nosso ensaio clínico prospectivo, randomizado e duplo-cego intervencionista é determinar a eficácia da terapia com altas doses de atorvastatina para reduzir a disfunção cognitiva pós-operatória (DCPO) em pacientes submetidos à cirurgia de válvula cardíaca.
Nossa hipótese é que a terapia com altas doses de atorvastatina reduzirá significativamente a incidência e/ou a gravidade da DCPO.

Critério de inclusão:
- Cinquenta pacientes informados e consentidos para cirurgia de válvula cardíaca com CEC, com mais de 50 anos de idade e sob os cuidados de um ou mais dos investigadores ou consultores serão incluídos prospectivamente ao longo de um período de um ano (mais um ano para acompanhamento ).
Critério de exclusão:
Serão excluídos pacientes com histórico das seguintes entidades patológicas:
- doença cerebrovascular sintomática com déficit residual substancial, abuso de álcool, doença psiquiátrica, insuficiência renal (creatinina>2,0),
- disfunção hepática (ALT ou AST > 1,5 x LSN), história de alergia ou miopatia com terapia com estatina, mulheres grávidas ou lactantes ou outras comorbidades que, na opinião do investigador, poderiam limitar a participação do paciente. Os critérios de exclusão são projetados para limitar a confusão e aumentar a diferenciação de déficits relacionados à cirurgia e/ou tratamento.
- Também serão excluídos pacientes incapazes de ler e, portanto, incapazes de concluir o teste neurocognitivo.
- Pacientes com endarterectomia carotídea concomitante ou outra cirurgia vascular também serão excluídos para fornecer uma amostra mais homogênea para comparação.
