Beschrijving
Inclusiecriteria:
- Leeftijd 18-75
- Histologische of cytologische documentatie van adenocarcinoom van de maag of adenocarcinoom van de gastro-oesofageale overgang;
- Gemetastaseerd adenocarcinoom van de maag of gemetastaseerd adenocarcinoom van de gastro-oesofageale overgang, aanvankelijk gediagnosticeerd of terugkerend.
- Meetbare ziekte volgens Response Evaluation Criteria in Solid Tumors
- ECOG-prestatiestatus van 0 of 1
- Levensverwachting van minimaal 3 maanden
Adequate beenmerg-, lever- en nierfunctie zoals beoordeeld aan de hand van de volgende laboratoriumvereisten:
- Hemoglobine ≥ 9,0 g / dl
- Absoluut aantal neutrofielen (ANC) ≥1.500 / mm3
- Aantal bloedplaatjes ≥ 100.000 / mm3
- Totaal bilirubine < 1,5 x bovengrens van normaal
- ALAT en ASAT < 2,5 x bovengrens van normaal (< 5 x ULN voor patiënten met leverbetrokkenheid van hun kanker)
- Internationaal genormaliseerde verhouding van PT (PT-INR) / PTT < 1,5 x ULN
- Creatinineklaring ≥ 60 ml/min (gebaseerd op formule van Cockcroft en Gault)
- Vermogen om te begrijpen en bereidheid om een schriftelijke geïnformeerde toestemming te ondertekenen. Ondertekende geïnformeerde toestemming moet worden verkregen voorafgaand aan studiespecifieke procedures
Uitsluitingscriteria:
- Patiënten met lokaal-regionaal maag- of gastro-oesofageaal adenocarcinoom (geen para-aortaknopen of viscerale structuur-invasieve primaire [T4]) die mogelijk kandidaat kunnen worden voor een operatie met curatieve intentie na systemische therapie
Geschiedenis van hartaandoeningen:
- Congestief hartfalen >NYHA klasse 2; instabiele angina pectoris (symptomen van angina pectoris aanwezig in rust), nieuw optredende angina pectoris (begon binnen de laatste drie maanden voorafgaand aan randomisatie) of myocardinfarct binnen zes maanden voorafgaand aan randomisatie;
- Ventriculaire aritmieën die anti-aritmische therapie vereisen (bètablokkers of digoxine zijn toegestaan);
- Ongecontroleerde hypertensie (systolische bloeddruk > 150 mmHg of diastolische bloeddruk > 90 mmHg) ondanks optimale medische behandeling
- Voorgeschiedenis van of gelijktijdige neoplasmata < 5 jaar voor aanvang van de studiebehandeling anders dan adenocarcinoom van de maag of adenocarcinoom van de gastro-oesofageale overgang, met uitzondering van curatief behandelde niet-melanoom huidkanker, in situ carcinoom van de cervix uteri of oppervlakkige blaastumoren [Ta niet-invasieve tumor ( Ta), carcinoom in situ (Tis) en T1 (tumor dringt lamina propria binnen)]
- Grote chirurgische ingreep, open biopsie of significant traumatisch letsel binnen 28 dagen voorafgaand aan randomisatie
- Bewijs van gastro-intestinale perforatie of darmobstructie tijdens de screeningperiode
- Bewijs of voorgeschiedenis van bloedingsdiathese of coagulopathie
- Niet-genezende wond, zweer of botbreuk
- Geschiedenis van gastro-intestinale bloedingen > graad 1 CTCAE versie 4.0 binnen 4 weken voorafgaand aan randomisatie
- Voorgeschiedenis van enige andere bloeding > graad 2 volgens CTCAE versie 4.0 binnen 4 weken voorafgaand aan randomisatie
Bekende psychiatrische en neurologische stoornissen waaronder bekende perifere of autonome neuropathie of slechthorendheid > graad 1 volgens CTCAE versie 4.0
- Als de patiënt echter bij aanvang al onomkeerbaar gehoorverlies van graad 4 (> 90 decibel (dB) bilateraal) heeft gekend, komt hij of zij in aanmerking, naar goeddunken van de onderzoeker.
- Zwangere vrouwen of vrouwen die borstvoeding geven, vrouwen die zwanger kunnen worden en geen adequate anticonceptie gebruiken [Bij vrouwen die zwanger kunnen worden moet binnen zeven dagen voor aanvang van de behandeling een negatieve serumzwangerschapstest worden uitgevoerd. Van belang is dat zowel mannen als vrouwen die deelnemen aan dit onderzoek adequate barrière-anticonceptiemaatregelen moeten nemen tijdens het verloop van het onderzoek en vier weken na voltooiing van het onderzoek of 6 maanden na de laatste dosis cisplatine (afhankelijk van welke groter is). De definitie van effectieve anticonceptie zal gebaseerd zijn op het klinische oordeel van de hoofdonderzoeker of een aangewezen medewerker.]
- Bewijs van infectie (> graad 2)
- Geschiedenis van HIV-infectie of chronische / actieve hepatitis B of C
- Bewijs van hersenmetastasen. Patiënten met onverklaarbare neurologische symptomen ondergaan een CT-scan of MRI van de hersenen om metastasen uit te sluiten.
- Convulsiestoornis die behandeling vereist met medicijnen die CYP 3A4 beïnvloeden
- Geschiedenis van orgaantransplantaat
- Bekende of vermoede allergie of overgevoeligheid voor een van de onderzoeksgeneesmiddelen, klassen van onderzoeksgeneesmiddelen of hulpstoffen in de formulering die in de loop van dit onderzoek is gegeven
- Elke toestand die onstabiel is of de veiligheid van de patiënt en zijn/haar medewerking aan het onderzoek in gevaar kan brengen
- Onvermogen om orale medicatie in te slikken of vast te houden
- Elke aandoening van malabsorptie waarvan de onderzoeker meent dat deze de absorptie of farmacokinetiek van de onderzoeksmedicatie in gevaar zou kunnen brengen
- Ongecorrigeerde uitdroging
- Bekende dihydropyrimidinedehydrogenasedeficiëntie
- Middelenmisbruik, medische, psychologische of sociale omstandigheden die de deelname van de patiënt aan het onderzoek of de evaluatie van de onderzoeksresultaten kunnen belemmeren
- Bewijs van trombotische of embolische ziekte, inclusief cerebrovasculair accident, voorbijgaande ischemische aanvallen of longembolie in de afgelopen 6 maanden
- Elke tumor met kenmerken die de onderzoeker ongeschikt acht voor mogelijk cytoreductieve therapie vanwege de kans op ernstige bloedingen of perforaties zoals zweren of bloedingen. Uitgesloten therapieën en medicijnen
- Eerdere of gelijktijdige systemische antikankertherapie inclusief cytotoxische therapie, gerichte middelen of experimentele therapie voor maagkanker. (neo)-adjuvante cytotoxische therapie is echter toegestaan als de laatste dosis > 6 maanden (12 maanden voor op platina gebaseerde therapie) werd toegediend vóór aanvang van de studiemedicatie in deze studie.
- Radiotherapie voorafgaand aan of tijdens het onderzoek (palliatieve radiotherapie is toegestaan zoals beschreven in de rubriek 'voorafgaande en gelijktijdige therapie', 4.3.7)
- Gebruik van biologische responsmodificatoren, zoals granulocyt G-CSF, binnen 3 weken na aanvang van het onderzoek en tijdens het onderzoek.
- Medicamenteuze onderzoeksbehandeling buiten deze studie tijdens of binnen 4 weken voorafgaand aan randomisatie
- Eerdere blootstelling aan een Ras-routeremmer zoals MEK- of Raf-remmers of farnesyltransferaseremmers
Therapeutische antistolling met vitamine K-antagonisten zoals warfarine, of met heparines of heparinoïden
- Lage dosis warfarine (1 mg p.o. q.d.) is toegestaan als de internationaal genormaliseerde ratio < 1,5 is
- Lage dosis aspirine is toegestaan (≤ 100 mg per dag)
- Profylactische doses heparine zijn toegestaan
- Voor patiënten die warfarine krijgen, zal de INR worden gemeten voordat met sorafenib wordt begonnen, en patiënten zullen regelmatig worden gecontroleerd op veranderingen in de protrombinetijd, INR of klinische bloedingsepisoden, aangezien zeldzame bloedingen of verhogingen van de INR zijn gemeld bij sommige patiënten die warfarine gebruikten terwijl ze warfarine gebruikten. op sorafenib-therapie.
