Multipel myeloom is een neoplastische aandoening met meerdere organen die wordt veroorzaakt door de klonale proliferatie van plasmacellen. Het heeft een incidentie van ongeveer 4,5/100.000 per jaar in de VS, waardoor het de tweede meest voorkomende hematologische maligniteit is. Al vele jaren vormen alkylerende middelen de ruggengraat van de behandeling. De combinatie van melfalan en prednison was jarenlang de standaardbehandeling voor patiënten die niet in aanmerking kwamen voor autologe transplantatie. Melphalan blijft het belangrijkste conditioneringsmiddel voor autologe transplantatie en cyclofosfamide heeft ook voet aan de grond gekregen bij de behandeling van deze ziekte.
De introductie van nieuwe middelen heeft het landschap van de behandeling van deze ziekte fundamenteel veranderd, hoewel de werkelijke effecten op de overleving nog niet bekend zijn. Immunomodulerende middelen en proteosoomremmers, waaronder thalidomide, lenalidomide en bortezomib, zijn gebruikt bij zowel nieuw gediagnosticeerde als recidiverende patiënten. Momenteel wordt intensief klinisch onderzoek gedaan naar de optimale manier om deze nieuwe middelen te combineren met de traditionele ruggengraat van de behandeling - inclusief alkylatoren - met elkaar en uiteindelijk met de daaropvolgende iteraties van deze klassen geneesmiddelen. Ondanks de therapeutische opwinding die deze ziekte omringt, zullen de meeste patiënten terugvallen en blijft genezing een ongrijpbaar doel.

Beschrijving
Inclusiecriteria:
- Volwassenen met recidiverend en/of refractair myeloom die 1 tot 4 eerdere behandelingslijnen hebben gekregen
- Moet een adequate lever- en nierfunctie hebben
- Zubrod Performance Status (ZPS) van 2 of beter
- Moet een meetbare ziekte hebben
Uitsluitingscriteria:
- Perifere neuropathie van graad II of hoger
- Trombocytopenie (bloedplaatjes minder dan 50.000/uL)
- Neutropenie (ANC<1000/uL)
- Aspartaataminotransferase (AST) of alanineaminotransferase (ALAT) >2,4 X ULN
- Totaal bilirubine >1,5 x bovengrens van normaal (ULN)
- Creatinineklaring van minder dan 45 milliliter per minuut (ml/min)
- Patiënten met hiv
- Patiënten met actieve hepatitis
- Zwangere of zogende vrouwen
- Personen in de vruchtbare leeftijd die geen adequate anticonceptie gebruiken
- Personen die geen geïnformeerde toestemming kunnen geven

Experimenteel: Bendamustine
Bendamustine wordt gecombineerd met standaardchemotherapie.

De eerste groep van drie patiënten die aan het onderzoek deelnemen, krijgt een dosis bendamustine van 25 mg/m^2.
Als deze dosis veilig blijkt te zijn, krijgen de volgende drie patiënten 50 mg/m^2.
Met behulp van een aangepast Fibonacci-dosis-escalatieontwerp zal de dosis blijven toenemen met een snelheid van 25 mg/m^2 totdat de hoogste veilige dosis bendamustine is gevonden.
De maximale dosis is 125 mg/m^2.
Bendamustine en bortezomib worden elke 21 dagen tweemaal per week via een katheter toegediend.
Dexamethason en lenalidomide worden oraal toegediend.
Over het algemeen duurt een cyclus van chemotherapie 21 dagen.

Het aantal patiënten dat ten minste één toxiciteit ervaart bij de laagste dosis bendamustine.
Een toxiciteit wordt gedefinieerd als een of meer van de volgende: bovenste luchtweginfectie; Bloedarmoede; trombocytopenie; neutropenie; kortademigheid bij inspanning; verminderde eetlust; misselijkheid; neuropathie; spanning; artritis; en hypercalciëmie.
