PRIMAIRE DOELEN:
- Fase I: het bepalen van de veilige dosis van alisertib bij toediening in combinatie met abirateron (abirateronacetaat) en prednison bij patiënten met gemetastaseerde castratieresistente prostaatkanker (mCRPC) met ziekteprogressie op abirateron en prednison.
- Fase II: bepalen van het percentage patiënten dat geen ziekteprogressie heeft nadat alisertib is toegevoegd aan abirateron en prednison.
SECUNDAIRE DOELSTELLINGEN:
- Fase II: Om de prostaatspecifieke antigeen (PSA)-kinetiek te bepalen nadat alisertib is toegevoegd aan het abirateron- en prednisonregime (dit omvat het percentage patiënten met PSA-progressievrij na 3 maanden, percentage patiënten met 50% PSA-reductie na studiebehandeling, maximum PSA-daling vanaf baseline gedurende de eerste 12 weken).
- Fase II: om de basislijn van circulerende tumorcellen (CTC's) te vergelijken met de CTC-telling van 12 weken na de therapie.
- Fase II: het vergelijken van de neuro-endocriene markerniveaus (chromogranine A en neuronspecifieke enolase [NSE]) in de uitgangssituatie met de neuro-endocriene markerniveaus 12 weken na de behandeling.
- Fase II: Om de algehele veiligheid van de combinatie van alisertib met abirateron en prednison verder te beoordelen in het fase 1-gedeelte van de studie.
OVERZICHT: Dit is een fase I-studie met dosisescalatie van alisertib, gevolgd door een fase II-studie.
Patiënten krijgen alisertib oraal (PO) tweemaal daags (BID) op dag 1-7, abirateronacetaat PO dagelijks en prednison PO BID. Cursussen worden elke 21 dagen herhaald bij afwezigheid van ziekteprogressie of onaanvaardbare toxiciteit.
Na voltooiing van de studiebehandeling worden patiënten na 30 dagen gevolgd.

Beschrijving
Inclusiecriteria:
- Leeftijd >/= 18 jaar en in staat om geïnformeerde toestemming te geven. Vrijwillige schriftelijke geïnformeerde toestemming voorafgaand aan de uitvoering van een studiegerelateerde procedure die geen deel uitmaakt van de normale medische zorg, met dien verstande dat de toestemming op elk moment door de proefpersoon kan worden ingetrokken zonder afbreuk te doen aan toekomstige medische zorg.
- Patiënten moeten een pathologisch bevestigde diagnose van prostaatadenocarcinoom hebben. Kenmerken van neuro-endocrien fenotype zijn toegestaan.
- Patiënten moeten bewijs hebben van gemetastaseerde ziekte.
- Patiënten worden momenteel behandeld met abirateron en de behandelingsdosis is gedurende ten minste 4 weken stabiel. Er zal geen verdere dosisaanpassing per behandelend arts plaatsvinden.
Patiënten met bewijs van een van de volgende ziekteprogressie op basis van PCWG2-criteria terwijl ze abirateronacetaat en prednison gebruiken:
- Klinische progressie (zoals symptomen die verband houden met prostaatkanker) met PSA-progressie (gedefinieerd als 25% toename ten opzichte van de basislijnwaarde met een toename van de absolute waarde van ten minste 2 ng/ml en een basislijn PSA van 2 ng/ml of hoger).
- Progressie van eendimensionaal meetbaar zacht weefsel (nodale of viscerale metastase) beoordeeld binnen 30 dagen voorafgaand aan registratie door een CT-scan of MRI van de buik en het bekken volgens de RECIST-criteria.
- Progressie van botziekte (evalueerbare ziekte) of (≥ 2 nieuwe botlaesie(s)) door botscan.
- Patiënten moeten een ECOG-prestatiestatus van ≤ 2 hebben.
- Patiënten moeten continu worden behandeld met een LH-RH-agonist of -antagonist of chirurgisch worden gecastreerd met gecastreerde testosteronspiegels (< 20 ng/dl).
- Voor fase I: een willekeurig aantal eerdere chemotherapieregimes is toegestaan, maar de patiënt moet abirateronacetaat gebruiken op het moment van progressie. Chemotherapie-naïeve patiënten worden alleen toegelaten in het fase I-deel van de studie.
- Voor Fase I: Patiënten moeten behandeling met docetaxel hebben gefaald, intolerant zijn of hebben geweigerd.
- Voor fase II: Patiënten moeten 1 maar niet meer dan 2 eerdere chemotherapieschema's voor prostaatkanker hebben gekregen.
- Patiënten hebben mogelijk op androgeenreceptoren gerichte therapie (waaronder antiandrogenen van de tweede en derde lijn) of andere geneesmiddelen in onderzoek gehad. De patiënt moet flutamide of nilutamide of andere anti-androgenen (waaronder enzalutamide) gedurende ten minste 4 weken hebben gestaakt en bicalutamide gedurende ten minste 6 weken voorafgaand aan behandeling op dag 1.
- Patiënten die behandeld worden met bisfosfonaten of denosumab moeten tijdens het onderzoek onder behandeling blijven.
- Patiënten mogen geen gelijktijdige bestraling of andere chemotherapie nodig hebben terwijl ze protocoltherapie krijgen. Patiënten kunnen eerder zijn bestraald, maar moeten de bestraling ten minste 4 weken (8 weken voor bestraling van de hersenen) vóór registratie hebben voltooid.
- Patiënten moeten hersteld zijn tot graad ≤ 1 van alle acute toxiciteit van eerdere bestraling of hormonale of chemotherapie.
- Patiënt stemt ermee in een aanvaardbare anticonceptiemethode te gebruiken gedurende de gehele behandelperiode van het onderzoek tot 4 maanden na de laatste dosis Alisertib. Adequate nierfunctie zoals gedefinieerd door serumcreatinine ≤ 1,5 x ULN. Als creatinine >1,5 x ULN, moet de berekende of gemeten creatinineklaring ≥ 40 ml/minuut zijn (Cockcroft-Gault).
- ANC > 1500/mm³, bloedplaatjes > 100.000/mm³, Hgb > 9 g/dL. Waarden moeten binnen 14 dagen worden verkregen zonder ondersteuning van myeloïde groeifactor of bloedplaatjestransfusie, maar erytrocytengroeifactor is toegestaan volgens de gepubliceerde ASCO-richtlijnen.
- Totaal bilirubine ≤ ULN, SGOT (AST) en SGPT (ALT) < 1,5 x ULN. AST en/of ALT kunnen oplopen tot 5X ULN indien met bekende leveraandoeningen
Uitsluitingscriteria:
- Systemische infectie die intraveneuze antibioticatherapie vereist binnen 14 dagen voorafgaand aan de eerste dosis van het onderzoeksgeneesmiddel, of andere ernstige infectie.
- Grote operatie binnen 28 dagen of ernstige infectie waarvoor intraveneuze antibiotica nodig zijn binnen 14 dagen voorafgaand aan de eerste dosis van de onderzoeksbehandeling.
- Patiënt heeft binnen 14 dagen vóór inschrijving andere geneesmiddelen voor onderzoek gekregen.
- Bekende gastro-intestinale ziekte of gastro-intestinale procedure die de opname van geneesmiddelen in de bovenste darm of tolerantie voor Alisertib kan beïnvloeden. Voorbeelden omvatten, maar zijn niet beperkt tot, partiële gastrectomie, resectie van de dunne darm, pancreatectomie, malabsorptie of coeliakie.
- De patiënt heeft een constante toediening van protonpompremmers, H2-antagonisten of pancreasenzymen nodig. Intermitterend gebruik van maagzuurremmers of H2-antagonisten is toegestaan (om maagzuur of reflux te beheersen) tijdens de studiedag 8 - 20. [Histamine-2 (H2)-receptorantagonisten zijn niet toegestaan vanaf de dag ervoor (dag -1) tot het einde van Alisertib-dosering (dag 7)]
- Aanhoudende misselijkheid of braken van welke ernst dan ook zonder verbetering na geschikte behandeling.
- > Graad 1 diarree, niet onder controle met de juiste behandeling.
- Geschiedenis van ongecontroleerd slaapapneusyndroom en andere aandoeningen die kunnen leiden tot overmatige slaperigheid overdag, zoals ernstige chronische obstructieve longziekte die aanvullende zuurstof vereist.
- Klinisch en/of radiografisch bewijs van hersenmetastasen. Patiënten met een voorgeschiedenis van metastasen van het centrale zenuwstelsel (CZS) maar die geen radiografisch of klinisch bewijs hebben van een resterende tumor (bijv. na volledige chirurgische resectie of stereotactische radiochirurgie) worden niet uitgesloten van deelname aan dit onderzoek.
- Bestralingstherapie tot meer dan 25% van het actieve beenmerg. Totale bekkenbestraling wordt beschouwd als meer dan 25%.
- Myocardinfarct binnen 6 maanden voorafgaand aan inschrijving of heeft New York Heart Association (NYHA) Klasse III of IV hartfalen, ongecontroleerde angina, ernstige ongecontroleerde ventriculaire aritmieën, of elektrocardiografisch bewijs van acute ischemie of afwijkingen van het actieve geleidingssysteem. Voorafgaand aan deelname aan het onderzoek moet elke ECG-afwijking bij de screening door de onderzoeker worden gedocumenteerd als niet medisch relevant.
- Ernstige medische of psychiatrische ziekte of laboratoriumafwijking die het risico kan verhogen dat gepaard gaat met deelname aan het onderzoek of toediening van onderzoeksproducten of die de interpretatie van onderzoeksresultaten kan verstoren en, naar het oordeel van de onderzoeker, de patiënt ongeschikt zou maken voor deelname aan dit onderzoek.
- Momenteel actieve andere maligniteit exclusief gecontroleerde niet-melanoom huidkanker. Patiënten worden geacht GEEN "momenteel actieve" maligniteit te hebben als ze de noodzakelijke therapie hebben voltooid en volgens hun arts een risico van minder dan 30% op terugval hebben.
- Behandeling met klinisch significante enzyminductoren, zoals de enzyminducerende anti-epileptica fenytoïne, carbamazepine of fenobarbital, of rifampicine, rifabutine, rifapentine of sint-janskruid binnen 14 dagen voorafgaand aan de eerste dosis Alisertib en tijdens het onderzoek.
- Bekende voorgeschiedenis van infectie met het humaan immunodeficiëntievirus (HIV), hepatitis B of hepatitis C. Testen is niet vereist bij afwezigheid van klinische bevindingen of vermoedens.

De Kaplan-Meier-productlimietmethode zal worden gebruikt om de waarschijnlijkheidsverdeling van progressievrije overleving (PFS) te schatten.
Het percentage patiënten dat ten minste een afname van 50% ten opzichte van de uitgangswaarde bereikt, wordt gerapporteerd met een betrouwbaarheidsinterval van 95%.
De resultaten worden grafisch gepresenteerd met behulp van een watervalplot.
