Beschrijving
Inclusiecriteria:
Klinische diagnose van vermoedelijke auto-immuun type 1 diabetes (T1D) zoals aangegeven door leeftijd van diagnose <10 jaar of gedocumenteerde positieve diabetesgerelateerde auto-antilichamen.
a. Opmerking: voor randomisatie, aanwezigheid van ten minste één van de diabetesgerelateerde auto-antilichamen [insuline auto-antilichamen (IAA) bij diagnose voorafgaand aan de start van insuline, eilandcelantilichamen (ICA), anti-GAD (GAD65), anti-IA2 (IA2) , Zinc Transporter 8 (ZnT8)] moet worden gedocumenteerd uit medische dossiers of nieuwe laboratoriummetingen (IAA en ICA niet gemeten door het centrale laboratorium) die naar het centrale laboratorium worden gestuurd voor deelnemers die ≥10 jaar oud waren bij de diagnose.
- Leeftijd: 12 tot <20 jaar.
- Duur van diabetes type 1: ≥1 jaar.
- Het huidige insulineregime omvat het gebruik van een insulinepomp of meerdere dagelijkse insuline-injecties (minstens 3 injecties per dag) gedurende de laatste drie maanden, zonder plannen om de modaliteit van insulinetoediening in de komende 6 maanden te wijzigen (bijv. overstappen op een pomp, pompgebruiker overstappen op injecties).
- Hemoglobine A1c: 7,5% - <10,0% van meting op het zorgpunt of lokaal laboratorium op de dag van het screeningsbezoek of binnen 1 maand voorafgaand.
- BMI: ≥85e percentiel gecorrigeerd voor leeftijd en geslacht.
- Totale dagelijkse dosis insuline: ≥0,8 eenheden per kg per dag.
- Gemiddelde van ≥3 zelfcontrole-bloedglucosetests (SMBG) per dag voorafgaand aan de start van het onderzoek en vanaf het downloaden van de door het onderzoek verstrekte bloedglucosemeter na het screeningsbezoek.
- Beschikbaar voor ten minste 6 maanden follow-up, heeft een telefoon thuis (of toegang tot de telefoon) en bereid om gecontacteerd te worden door personeel van de klinische locatie.
- Naar verwachting voldoen aan het protocol naar het oordeel van de onderzoeker.
Uitsluitingscriteria:
- Gebruik van niet-insulinemedicatie voor controle van de bloedglucose in de afgelopen 6 maanden of van plan om deze binnen de komende 6 maanden te gebruiken (anders dan het onderzoeksgeneesmiddel).
- Gebruik van medicijnen voor gewichtsvermindering (zoals: Belviq (lorcaserin), Qsymia (Phentermine + topiramaat), Orlistat (xenical)) binnen de voorgaande 6 maanden of van plan om binnen de komende 6 maanden te gebruiken.
- Gebruik van een medicijn zoals stimulerende middelen, psychotrope middelen en orale/geïnhaleerde glucocorticoïden die gewichtstoename of glykemische controle van T1D kunnen beïnvloeden of van plan zijn om het binnen de komende 6 maanden te gebruiken.
- Elke omstandigheid die naar het oordeel van de onderzoeker een negatieve invloed heeft op de voltooiing van het protocol.
Vrouwtjes: zwanger, lacterend of van plan om binnen 34 weken zwanger te worden
- Een negatieve urinezwangerschapstest is vereist voor alle vrouwen. Een effectieve anticonceptiemethode of onthouding is vereist voor alle vrouwen die de menarche hebben doorgemaakt.
- Vereisten met betrekking tot zwangerschapstesten voorafgaand aan inschrijving en controle op zwangerschap in de loop van het onderzoek kunnen verder worden gedefinieerd door elke individuele Institutional Review Board (IRB)
- Klinische diagnose van coeliakie die slecht onder controle is, zoals gedefinieerd door de meest recente weefseltransglutaminase (tTG) die in het abnormale bereik ligt.
- Geschiedenis van ≥1 diabetische ketoacidose-gebeurtenissen in de afgelopen 3 maanden.
- Geschiedenis van ≥1 ernstige hypoglykemische voorvallen (cognitieve stoornis waarvoor hulp nodig was bij de behandeling) in de afgelopen 3 maanden.
- Geschiedenis van bloedarmoede of vitamine B12-tekort in de afgelopen 2 jaar.
- Deelname aan een interventiestudie in de afgelopen 3 maanden.

Experimenteel: Metformine
Metformine 2000 mg per dag
|
De sterkte van elke tablet is 500 mg.
De deelnemers bouwen gedurende vier weken op tot een dagelijkse dosis door gedurende 7 dagen één tablet per dag in te nemen, gedurende 7 dagen tweemaal daags één tablet, gedurende 7 dagen één tablet 's ochtends en 2 tabletten 's avonds, en vervolgens 's ochtends en 's avonds 2 tabletten. 2 tabletten 's avonds, dagelijks gedurende de rest van de studiebehandelingsperiode.
