Op dit moment ontbreken effectieve behandelingen voor RTT, MECP2 Dup of Rett-gerelateerde aandoeningen. Onderzoekers hebben de afgelopen elf jaar substantiële vooruitgang geboekt in RTT, zodat deze studie een vernauwing van de focus vertegenwoordigt tot mutaties of duplicaties van het MECP2-gen en gerelateerde aandoeningen, inclusief die met fenotypische overlap. Het begrip van RTT is opmerkelijk goed gevorderd door het Rett Syndrome Natural History Clinical Protocol (NHS) en dienovereenkomstig is de vooruitgang op het gebied van basiswetenschap met evenveel succes vooruitgegaan. Zo heeft vooruitgang in de klinische en basiswetenschap geleid tot het opzetten van klinische onderzoeken en andere translationele studies die veelbelovend zijn voor aanvullende klinische onderzoeken in de toekomst. Tijdens het proces werden onderzoekers zich echter bewust van aanvullende MECP2- en RTT-gerelateerde aandoeningen die onbekend waren op het moment dat het oorspronkelijke voorstel werd bedacht en waren verder onder de indruk van de substantiële klinische variabiliteit bij personen met RTT die niet kan worden verklaard door verschillen in mutaties. alleen. In feite heeft de variabiliteit tussen individuen met identieke mutaties ertoe geleid dat onderzoekers op zoek zijn gegaan naar aanvullende verklaringen. Op het moment van de eerste aanvraag (2002), slechts drie jaar na de identificatie van het gen, MECP2, als de moleculaire link naar RTT, waren de onderzoekers niet op de hoogte van de variatie in klinische aandoeningen gerelateerd aan MECP2-mutaties of aan de gerelateerde maar nogal verschillende MECP2 Dup. Elke aandoening wordt gekenmerkt door significante kenmerken van de neurologische ontwikkeling die verband houden met veranderingen in het MECP2-gen of met fenotypes die sterk lijken op die van personen met RTT. Verder was de fenotypische overlap met RTT als gevolg van mutaties in CDKL5 en FOXG1 ook onontgonnen. De onderzoekers stellen voor om in deze nieuwe studie voort te bouwen op de substantiële vooruitgang die is geboekt bij het begrijpen van zowel klassieke als variant RTT en om deze gerelateerde aandoeningen, MECP2 Dup en de Rett-gerelateerde aandoeningen, waaronder CDKL5, FOXG1, en personen met MECP2-mutaties die geen RTT. In combinatie met de longitudinale klinische beoordeling die wordt uitgevoerd via de natuurlijke geschiedeniscomponent, zullen onderzoekers systematisch het bloed van alle bereidwillige deelnemers verzamelen en plasma, DNA en RNA isoleren. Alle deelnemers aan de Natural History Study zullen worden gevraagd om monsters bij te dragen tijdens het eerste bezoek, terwijl er herhaaldelijk monsters zullen worden verzameld bij een subgroep van deelnemers om te zoeken naar veranderingen in de loop van de tijd. Om factoren te identificeren die onderscheid maken tussen getroffen en niet-aangetaste individuen, en om de mogelijkheid te hebben om de erfelijkheidsgraad en mogelijke gevolgen van genetische veranderingen in families te karakteriseren, zullen monsters worden verzameld van niet-aangetaste familieleden. Bovendien zullen bij een subset van personen die zijn gekozen vanwege unieke klinische kenmerken, huidbiopten en/of haarzakjes worden afgenomen om cellijnen vast te stellen. Onderzoekers zullen alle personen die monsters leveren vragen om akkoord te gaan met mogelijke toekomstige sequentiebepaling van het hele genoom om mogelijk te kunnen evalueren op genetische modificatoren van deze ziekten.
Deze materialen zullen worden opgeslagen in een centrale opslagplaats (Greenwood Genetics Laboratory). Het belangrijkste doel van deze monsters is om te dienen als duurzame materialen voor toekomstige analyses, maar er zal een reeks gedefinieerde analyses worden uitgevoerd op alle monsters. Voor de monsters die zijn verzameld in het cohort van het Rett-syndroom, zullen onderzoekers X-chromosoom-inactiveringsonderzoeken uitvoeren en algemene polymorfismen in van de hersenen afgeleide neurotrofe factor (BDNF) evalueren en de bijdrage van deze bekende factoren aan de ernst van de ziekte bepalen. Voor MECP2 Dup zullen cohortonderzoekers ontstekingsmarkers in het plasma karakteriseren en deze correleren met klinische kenmerken. Ook voor MECP2 Dup zullen cohortonderzoekers gedetailleerde genomische breekpunt- en geninhoudanalyses uitvoeren en dit correleren met de ernst van de ziekte. Een vergelijkbare analyse van genomische breekpunten en geninhoud zal worden uitgevoerd bij mensen met FOXG1-duplicaties. Ten slotte zullen onderzoekers in een pilootstudie metabole profilering uitvoeren op mensen met alle aandoeningen en evalueren op metabole kenmerken die verband houden met de ernst van de ziekte, en metabole kenmerken die algemeen of uniek zijn tussen deze aandoeningen. Dit werk zal een duurzame bron vormen voor toekomstige analyse, het begrip van genotype/fenotype-correlaties vergroten, andere biologische factoren identificeren die bijdragen aan de ernst van de ziekte, en het kader bieden voor de ontwikkeling van biomarkers van ziektetoestand en ernst.

Beschrijving
Inclusiecriteria:
- Individuen van beide geslachten en van alle leeftijden, met RTT-, MECP2 Dup- en RTT-gerelateerde aandoeningen, waaronder die met mutaties of deleties in CDKL5- en FOXG1-genen, of die met RTT (atypisch of typisch) die mutatie-negatief zijn. Bovendien komen onaangetaste familieleden van die mensen die voldoen aan de vermelde ziektespecifieke criteria in aanmerking.
Uitsluitingscriteria:
- Personen die niet aan bovenstaande criteria voldoen, worden uitgesloten.
