Multimodale medicijninfiltratie bij totale knieartroplastiek: is posterieure capsulaire infiltratie het risico waard? Een prospectieve, dubbelblinde, gerandomiseerde, gecontroleerde studie
Multimodale lokale anesthesie-infiltratie (LAI) zorgt voor effectieve pijnbestrijding bij patiënten die een totale knieartroplastiek (TKA) ondergaan. Sommige chirurgen vermijden posterieure capsulaire infiltratie (PCI) uit angst posterieure neurovasculaire structuren te beschadigen. Er zijn beperkte gegevens over de toegevoegde voordelen van PCI met verschillende combinaties van lokale anesthetica. Daarom wilde de onderzoeker weten wat de effectiviteit is van pijnbestrijding bij LAI met en zonder PCI.
De helft van de deelnemers kreeg LAI met PCI, terwijl de andere helft LAI zonder PCI kreeg tijdens een totale knieartroplastiek.

- Geneesmiddel: 0,5% bupivacaïne 100 mg, morfinesulfaat 5 mg, 0,1% epinefrine 0,6 mg en ketorolac 30 mg gemengde NSS tot 100 ml
- Procedure: Geneesmiddelinjectie in het voorste zachte weefsel (25 ml) + mediaal gootgebied (25 ml) + lateraal gootgebied (25 ml) + posterieure capsulaire infiltratie (25 ml)

Beschrijving
Inclusiecriteria:
- Primaire artrose van de knie, jonger dan 80 jaar, en in staat de onderzoeksprocedures te begrijpen en na te leven.
Uitsluitingscriteria:
- Eerdere drugsverslaving
- Onvermogen om een ruggenmergblokkade te ondergaan
- Allergie om drugs te bestuderen
- Nierinsufficiëntie
- Abnormale leverfunctie
- Geschiedenis van een beroerte
- Geschiedenis van coronaire hartziekte
