De voorgestelde studie omvat de rekrutering van de deelnemerspopulatie en het gebruik van gegevens van de bestaande onderzoeksstudiecohorten die zijn gerekruteerd en geïnformeerde toestemming hebben gegeven voor deelname. Deelnemers die aan alle inclusie-/exclusiecriteria voldoen, worden ingeschreven voor het onderzoek. Deelnemers aan de bestaande onderzoeksstudiecohorten ondergaan longfunctietesten, CT-scans van de borstkas en bloedafnames bij elk studiebezoek dat elke 18 maanden plaatsvindt gedurende 3 bezoeken. Sommige van de studieprocedures worden mogelijk niet herhaald als ze zijn uitgevoerd als onderdeel van andere onderzoeksprojecten.
Belangrijkste studiebezoek
De volgende onderzoeksprocedures kunnen in de loop van een of meer bezoeken worden uitgevoerd om aan planningsbehoeften te voldoen. Het studiebezoek vindt plaats in het Montefiore Clinical and Translational Research Center en het University of Pittsburgh Astma Institute in het University of Pittsburgh Medical Center - Montefiore Hospital en kan tot 4 uur in beslag nemen.
- Geïnformeerde toestemming
- Beoordeling van de medische geschiedenis en demografische gegevens
- Beoordeling van opname-/uitsluitingscriteria
- Lichamelijk onderzoek om vitale functies, gewicht en lengte, taille-, heup- en dijomtrek op te nemen.
- Urinezwangerschapstest voor vrouwen in de vruchtbare leeftijd
- Op astma gerichte vragenlijst en vragenlijst over de kwaliteit van leven
- Meting van uitgeademde stikstofmonoxide (eNO).
- spirometrie
- Methacholine-provocatietest - hoeft niet te worden herhaald als de deelnemer dit in de afgelopen 6 maanden heeft gedaan als onderdeel van een ander onderzoek.
- CT-scans van de borst/buik (Als de deelnemer de CT-scan van de borst binnen 6 maanden heeft laten uitvoeren als onderdeel van ander onderzoek, worden deze niet herhaald voor studiedoeleinden. Alleen de Abdominale CT zal worden uitgevoerd)
- Onderzoek bloedafname voor biomarkeranalyses - een deel van dit bloed zal ook worden gebruikt voor pre-bronchoscopietesten in het geval dat de deelnemer in aanmerking komt en geïnteresseerd is in het ondergaan van deel 2, het bronchoscopiebezoek.
- Vetweefselbiopsie (Fat Biopsy): uitgevoerd bij deelnemers met een BMI≥25). Dit wordt herhaald als het eerder is gedaan, omdat het testen op het vet anders zal zijn dan wat eerder op het vet is getest.
- Facultatieve deelstudie
Bij een afzonderlijk bezoek zal slechts een subgroep van 60 geselecteerde deelnemers worden gevraagd om de volgende procedures te ondergaan om aanvullende biologische longmonsters te verkrijgen. Dit bezoek is optioneel voor deelnemers die bereid zijn deel te nemen aan het bronchoscopiegedeelte. De onderzoekers zullen proberen een gelijk aantal (n=15 per groep) obese astmapatiënten, obese niet-astmapatiënten, niet-obese astmapatiënten en niet-obese niet-astmapatiënten te werven. Het bezoek vindt plaats in het Montefiore Clinical Translational Research Center (MUH-CTRC). De bronchoscopieprocedure duurt ongeveer 30-45 minuten, maar de voorbereiding en de hersteltijd maken het bezoek meestal ongeveer 4-8 uur in totaal.
Van deelnemers wordt verwacht dat ze 8 uur voorafgaand aan de bronchoscopieprocedure vasten en op de dag van de procedure geen vloeistoffen drinken. Deelnemers kunnen hun gebruikelijke medicijnen met slokjes water innemen.
• Klinische laboratoriumtesten met volledig bloedbeeld/differentieel/bloedplaatjes, protrombinetijd/partiële tromboplastinetijd, internationale genormaliseerde ratio, bloedureumstikstof en creatinine voorafgaand aan bronchoscopie. Als de patiënt de eerste bloedafname weigert, het monster ontoereikend is of de laboratoria meer dan 30 dagen voorafgaand aan het bronchoscopiebezoek zijn uitgevoerd, kan voorafgaand aan de bronchoscopieprocedure een tweede bloedmonster van maximaal 1 eetlepel worden afgenomen om er zeker van te zijn dat de patiënt is gezond genoeg voor de procedure.
Als de methacholine-uitdaging meer dan 30 dagen voorafgaand aan het bezoek twee datum is gedaan, zullen longfunctietesten worden herhaald.
- Oral Wash: deelnemers wordt gevraagd om gedurende 1 minuut met zout water te gorgelen en vervolgens in een bakje te spugen om een monster van ongeveer 10 ml (2 theelepels) te verkrijgen tijdens dit studiebezoek.
- Tongschrapen: er wordt een tongblad of een soortgelijk voorwerp gebruikt om de tong van de deelnemer te schrapen. Dit monster wordt gebruikt om te bepalen welke bacteriën zich op de tong van de deelnemer bevinden. Deze procedure duurt slechts enkele minuten en is niet pijnlijk.
- Daarna spoelen de deelnemers met 10cc Listerine. Dit is om ervoor te zorgen dat er voorafgaand aan de bronchoscopie geen bacteriën in de mond van de deelnemer zijn achtergebleven.
- Bronchoscopie met bronchoalveolaire lavage (BAL), endobronchiaal poetsen en biopsie
Onder plaatselijke verdoving ondergaan deelnemers bronchoscopie met BAL voor microbiologische monsters. Een lichte intraveneuze verdoving wordt getitreerd om de deelnemer indien nodig comfort te bieden. De vitale functies worden continu gecontroleerd voor, tijdens en na de procedure. Deze procedure bestaat uit 3 fasen.
- Longspoeling: tijdens longspoeling worden vijf wasbeurten van 50 ml steriele zoutwateroplossing door de bronchoscoop geïnjecteerd. Vloeistof zal worden opgevangen met zachte afzuiging voor onderzoeksstudies.
- Bronchiaal poetsen: een kleine borstel wordt door de bronchoscoop ingebracht en zachtjes over een heel klein deel van het longoppervlak gewreven. Er worden ongeveer 10 borstels uitgevoerd om epitheelcellen te verzamelen.
- Endobronchiale biopsie: terwijl de bronchoscoop wordt ingebracht, wordt een ander klein instrument door de bronchoscoop gehaald om kleine weefselmonsters (biopten) van de wanden van de luchtwegen te nemen. Tijdens deze procedure kunnen maximaal 5 monsters worden genomen.
Na de bronchoscopieprocedure worden deelnemers nauwlettend gevolgd in de herstelruimte van de bronchoscopie of bij terugkeer naar het CTRC. In zeldzame gevallen kan de astma van de deelnemer verergeren tijdens of na de procedure. Passende medische behandelingen of nachtelijke observatie zullen indien nodig worden verstrekt. Na de bronchoscopie mogen deelnemers niet naar huis rijden. Dit komt omdat de sedativa die tijdens de procedure worden gebruikt, de rijvaardigheid kunnen verminderen. De onderzoekers zullen de naam en het nummer van de contactpersoon vragen van de persoon die de deelnemer na de bronchoscopie zal ophalen

Inclusiecriteria:
- 18 jaar of ouder
- Proefpersoon waarvan eerder is vastgesteld dat ze hiv-positief zijn door middel van testen
Uitsluitingscriteria:
- Zwangerschap of borstvoeding
- Contra-indicatie voor longfunctietesten (d.w.z. buik- of staaroperatie binnen 3 maanden, recent myocardinfarct, enz.)
- Hartaandoening zoals tachycardie, angina pectoris of aritmieën
- Acute luchtweginfectie: toenemende luchtwegsymptomen of koorts (temperatuur >100,4°F [38°C]) binnen 4 weken na aanvang van het onderzoek
- Ziekenhuisopname binnen 4 weken na aanvang van de studie
- Ongecontroleerde hypertensie bij screeningbezoek (systolisch > 160 mm Hg of diastolisch > 100 mm Hg) van gemiddeld twee of meer metingen (deelnemer kan terugkomen voor screening nadat de bloeddruk onder controle is)
- Actieve kanker die systemische chemotherapie of bestraling vereist
- Actieve infectie van longen, hersenen of buik; of intraveneus drugsgebruik of alcoholgebruik dat naar de mening van de onderzoeker het vermogen om onderzoeksonderzoeken af te ronden zal aantasten
- Geschiedenis van eerdere longresectie
