Gepegyleerd liposomaal doxorubicine versus pirarubicine plus ifosfamide, dacarbazine bij lokaal gevorderd, inoperabel of gemetastaseerd wekedelensarcoom (PDVPSTS)
Vergelijking van de effectiviteit en toxiciteit voor lokaal gevorderde, inoperabele of gemetastaseerde patiënten met wekedelensarcoom die een totale dosis anthracycline-antibiotica van meer dan 300 mg/m2 hadden gekregen met gepegyleerd liposomaal doxorubicine versus pirarubicine plus ifosfamide, dacarbazine, een multicenter, open-label, gerandomiseerde fase 2 proef

Gedetailleerde beschrijving
We ontwerpen deze multicenter, open-label, gerandomiseerde, fase 2-studie in 5 academische ziekenhuizen in China. Geschikte patiënten moeten 16 jaar of ouder zijn met een diagnose van een gevorderd inoperabel of gemetastaseerd wekedelensarcoom, van intermediaire of hoge graad, waarvoor geen standaard curatieve therapie beschikbaar zal zijn, een prestatiestatus van de Eastern Cooperative Oncology Group van 0- 1, en meetbare ziekte volgens responsevaluatiecriteria in solide tumoren versie 1.1.
Deelnemers worden willekeurig (1:1) toegewezen aan gepegyleerde liposomale doxorubicine (60 mg/m² via continue intraveneuze infusie gedurende 4-6 uur op dag 1 van elke cyclus van 21 dagen gedurende maximaal zes cycli) of pirarubicine (30 mg/ m2/d continue intraveneuze infusie gedurende 3 uur op dag 1 en 2 van elke cyclus van 21 dagen gedurende maximaal zes cycli) plus ifosfamide (2 g/m²/d intraveneus gedurende 2 uur op dag 2,3 en 4 van elke cyclus van 21 dagen gedurende maximaal zes cycli), dacarbazine (300 mg/m²/d intraveneus gedurende 2 uur op dag 2, 3 en 4 van elke cyclus van 21 dagen gedurende maximaal zes cycli). Na zes behandelingscycli zullen patiënten afwachtend worden gevolgd terwijl patiënten met een stabiele of responsieve ziekte door kunnen gaan met ifosfamide en dacarbzine tot gedocumenteerde ziekteprogressie. Een op het web gebaseerde centrale randomisatie met blokgroottes van twee en vier werd gestratificeerd op basis van de ernst van de ziekte, de wijze van toediening van het geneesmiddel en eerdere systemische therapie. Patiënten en onderzoekers worden niet gemaskeerd voor de behandelingsopdracht. Het primaire eindpunt is progressie-overleving, geanalyseerd in de intention-to-treat-populatie. Er zullen veiligheidsanalyses worden uitgevoerd bij alle patiënten die een hoeveelheid onderzoeksgeneesmiddel krijgen.

Beschrijving
Inclusiecriteria:
- 16 jaar of ouder;
- diagnose van een gevorderd inoperabel of gemetastaseerd wekedelensarcoom, van middelmatige of hoge graad, waarvoor geen standaard curatieve therapie beschikbaar is;
- cumulatieve dosis anthracycline-antibioticum ≥ 300 mg/m2;
- stabiel of responsief op doxorubicine, potentiële begunstigde;
- een Eastern Cooperative Oncology Group (ECOG) performance status van 0-1, een levensverwachting van minimaal 3 maanden;
- meetbare ziekte volgens RECIST 1.1;
- adequate eindorgaan- en hemopoëtische functie.
Uitsluitingscriteria:
- vooruitgang ten opzichte van doxorubicine;
- eerdere mediastinale of cardiale radiotherapie;
- een laaggradige tumor volgens standaard beoordelingssystemen (bijv. American Joint Committee on Cancer graad 1 en 2 of Fédération Nationale des Centres de Lutte Contre le Cancer graad 1);
- significante cardiale disfunctie;
- ernstige chronische obstructieve longziekte;
- een bekende infectie met HIV of actieve infectie met hepatitis B of hepatitis C;
- bekende hersenmetastasen tenzij eerder behandeld en goed onder controle gehouden gedurende een periode van 3 maanden of langer;
- combinatie met andere antitumortherapie;
- zwanger of borstvoeding geven.

Experimenteel: Arm A
deelnemers krijgen gepegyleerde liposomale doxorubicine (50 mg/m²d1) plus ifosfamide (2 g/m²/d d2-3), dacarbazine (300 mg/m²/d d2-3).

Er zijn verschillende pogingen gedaan om het toxiciteitsprofiel van conventionele anthracyclines te verbeteren, waaronder het gebruik van liposomale inkapselingstechnologie, zoals gepegyleerde liposomale doxorubicine.

Placebo-vergelijker: Arm B
deelnemers krijgen pirarubicine (60 mg/m²d1) plus ifosfamide (2 g/m²/d d2-3), dacarbazine (300 mg/m²/d d2-3).

Een andere poging om het toxiciteitsprofiel van conventionele anthracyclines te verbeteren, is de ontwikkeling van nieuwe anthracycline-analogen, zoals pirarubicine.
