PC4PrEP: PrEP integreren in de eerstelijnszorg (PC4PrEP)
PC4PrEP: integratie van pre-exposure profylaxe (PrEP) in de eerstelijnszorg
Orale pre-exposure profylaxe (PrEP) is bewezen effectief in het verminderen van hiv-infectie bij risicovolle mannen die seks hebben met mannen, heteroseksueel actieve vrouwen en mannen, en injecterende drugsgebruikers. Ondanks de goedkeuring door de FDA in 2012 en de ontwikkeling van klinische richtlijnen van de Centers for Disease Control and Prevention (CDC), is de opname van PrEP beperkt. Belangrijke belemmeringen voor de implementatie van PrEP zijn onder meer: systeembarrières (gebrek aan een medisch "thuis" en aan modellen voor de implementatie van PrEP); belemmeringen voor aanbieders (moeite met het identificeren van degenen die waarschijnlijk baat zullen hebben bij PrEP, onervarenheid met PrEP en zorgen over therapietrouw en risicocompensatie); en gebruikersbelemmeringen (gebrek aan bewustzijn van PrEP, onvermogen om toegang te krijgen tot aanbieders die vertrouwd zijn met het voorschrijven van PrEP, en zorgen over stigmatisering en bijwerkingen). Kosten vormen geen belemmering in de staat New York, waar PrEP wordt gedekt door veel verzekeringsplannen, waaronder Medicaid.
Eerstelijnszorg voor PrEP (PC4PrEP) is een structurele interventie op meerdere niveaus die PrEP zal integreren in de eerstelijnszorgpraktijken die zorgen voor achtergestelde gemeenschappen in de Bronx, NY, een epicentrum van aanhoudende hiv-infectie in de VS. PC4PrEP gaat een organisatieprotocol ontwikkelen voor het voorschrijven van PrEP in de eerste lijn; personen met een hoog risico identificeren in eerstelijnsklinieken en hiv-testlocaties in de gemeenschap met behulp van een nieuwe PrEP-geschiktheidstool; koppelen aan eerstelijnszorgverleners (PCP's) die PrEP kunnen geven; en adviseer potentiële gebruikers over PrEP, zowel voordat ze een recept krijgen (om de ontvankelijkheid te vergroten) als nadat ze met PrEP zijn gestart (om de therapietrouw te verbeteren). In de loop van deze studie zal (zullen) de onderzoeker(s) (1) PC4PrEP ontwikkelen en testen; (2) het implementeren en evalueren in "echte" omgevingen (Federally Qualified Health Centers; FQHC's) op objectieve resultaten, evenals rapporten van zorgverleners en patiënten; en (3) een nieuw model presenteren, de PrEP Cascade, dat - net als bij de HIV Care Cascade voor HIV+-populaties - kan worden gebruikt om de impact van PrEP-programma's in de VS en andere landen te evalueren. PC4PrEP is in overeenstemming met CDC en de richtlijnen van het New York State Department of Health (NYSDOH) en de Affordable Care Act bij het integreren van PrEP in de eerstelijnszorgpraktijken en reageert op recente NYSDOH-aanbevelingen uit 2014 die PrEP nu positioneren als een eerstelijnsinterventie voor MSM en transgender vrouwen die doorlopend anale seks hebben zonder condooms, hiv-partners in sero-discordante relaties en heteroseksuele vrouwen met een hoog risico in gebieden met een hoge seroprevalentie. Fase 2 futiliteitsonderzoek, beoordeel of het veelbelovend is voor het verhogen van het aantal PrEP-recepten in de Bronx, NY; en (2) Identificeer de sterke punten en beperkingen van PC4PrEP op twee manieren: (a) via een procesevaluatie met gemengde methoden PrEP-geschikte patiënten en huisartsen, counselors en navigators; en (b) door "afval" te identificeren bij elke stap van de PrEP-cascade.

Hoewel orale pre-exposure profylaxe (PrEP) in 2012 door de FDA is goedgekeurd en de CDC tussentijdse klinische praktijkrichtlijnen heeft ontwikkeld, is de acceptatie beperkt. De literatuur heeft systeem-, provider- en gebruikersbarrières voor PrEP-implementatie geïdentificeerd. Systeembelemmeringen zijn onder meer het ontbreken van een medisch huis ("de bevoegdheidsparadox") en van organisatiemodellen voor het implementeren van PrEP. Belemmeringen voor aanbieders zijn onder meer moeite met het identificeren van degenen die waarschijnlijk baat zullen hebben bij PrEP, onervarenheid met PrEP en zorgen over therapietrouw en risicocompensatie. Barrières voor gebruikers zijn onder meer een gebrek aan bewustzijn van PrEP, het onvermogen om toegang te krijgen tot aanbieders die het prettig vinden om het voor te schrijven, en zorgen over stigmatisering en bijwerkingen. Kosten vormen geen belemmering in de staat New York (NYS), waar PrEP wordt gedekt door veel verzekeringsplannen, waaronder Medicaid, en wordt verstrekt aan onverzekerden. Tot op heden zijn er geen interventies in de praktijk ontwikkeld en formeel geëvalueerd om de opname van PrEP te bevorderen bij mensen met een hoog risico op hiv-acquisitie. Implementatie Wetenschappelijke onderzoeksdemonstratieprojecten zijn dringend nodig om de meest effectieve strategieën te bepalen voor de integratie van PrEP in de eerstelijnszorg. Het langetermijndoel van PC4PrEP is om het aantal nieuwe hiv-infecties te verminderen door een overdraagbaar model van PrEP-toediening te ontwikkelen in eerstelijnszorginstellingen die zich bevinden in gemeenschappen met een hoge hiv-incidentie. Het doel van deze studie is het ontwikkelen, implementeren en evalueren van de eerstelijnszorg voor PrEP (PC4PrEP), een nieuwe structurele interventie op meerdere niveaus die de belemmeringen van het systeem, de aanbieder en de gebruiker voor de opname van PrEP zal aanpakken.
PC4PrEP heeft vier componenten. Het zal: (1) een "thuis" aanwijzen voor PrEP in eerstelijnsklinieken; (2) eerstelijnszorgverleners (PCP's) trainen en ondersteunen om PrEP voor te schrijven en te beheren, en normen voor zorgverleners te creëren die de waarde van PrEP promoten; (3) personen met een hoog risico identificeren in eerstelijnsklinieken met behulp van een nieuwe PrEP-geschiktheidstool ontwikkeld door ons team; en (4) personen met een hoog risico buiten de zorg identificeren (in samenwerking met een gemeenschapsorganisatie die outreach en hiv-tests uitvoert bij hiv-risicogroepen) en hen koppelen aan huisartsen die PrEP kunnen bieden. Onderzoeker(s) zullen potentiële gebruikers over PrEP adviseren, zowel voordat ze een recept krijgen (om de ontvankelijkheid te vergroten) als nadat ze met PrEP zijn gestart (om de therapietrouw te verbeteren).
Onderzoeker(s) zullen een fase 2 klinische gerandomiseerde futiliteitsstudie van PC4PrEP uitvoeren in zes Montefiore Medical Center Federally Qualified Health Centers (FQHC's) en een niet-gerandomiseerde Bronx Community Based Organization (CBO) om te bepalen of PC4PrEP veelbelovend lijkt voor het verhogen van het percentage van nieuwe PrEP-voorschriften, het effect ervan op de resultaten beoordelen in elke fase van onze conceptualisering van de "PrEP-cascade", en de uitdagingen voor de acceptatie van PrEP beoordelen.
De specifieke doelstellingen zijn:
- Beoordeel effecten van PC4PrEP gemeten als het aantal nieuwe PrEP-voorschriften per kliniek per jaar;
- Bepaal via een gefaseerde implementatie van de vier componenten van PC4PrEP de incrementele bijdrage van elk component;
- Evalueer de effecten van PC4PrEP op maatregelen van de "PrEP Cascade";
- Beoordeel uitdagingen voor PC4PrEP-acceptatie door (1) diepte-interviews met PCP's en ondersteunend personeel over PC4PrEP; (2) diepte-interviews met geselecteerde patiënt-aanbieder dyades om hun communicatie over PrEP te begrijpen; (3) een genest longitudinaal kwalitatief onderzoek van patiënten die met PrEP beginnen, waaronder onderzoek naar risicocompensatie en zelfgerapporteerde therapietrouw aangevuld met een objectieve maatstaf voor therapietrouw, gedroogde bloedvlektesten voor tenofovirdifosfaat.
Onderzoeker(s) zullen PC4PrEP ontwikkelen, testen en implementeren; evalueer het in 'real-world'-instellingen (FQHC's) via objectieve resultaten en rapporten van zorgverleners en patiënten. PC4PrEP zal worden ontwikkeld door ons multidisciplinaire academische gemeenschapsteam van het Montefiore Medical Center, het Albert Einstein College of Medicine, het HIV Center for Clinical and Behavioral Studies aan het NYS Psychiatric Institute en Columbia University, en Bronx CBO. PC4PrEP wordt aangestuurd door het beleid van het Montefiore Medical Center (dat huisartsen PrEP promoten); ontworpen door degenen die het daadwerkelijk zullen gebruiken voor naadloze implementatie, efficiëntie en duurzaamheid; en maakt gebruik van bestaand personeel in hun huidige functietitels en heersende gezondheidszorgpraktijken [bijv. elektronische medische dossiers (EMR) "best practice"-waarschuwingen, screeningtools, hiv-consulenten, permanente medische educatie].

Interventieklinieken krijgen de PC4PrEP-interventie.
De interventie omvat bewustwording van het Montefiore PrEP-beleid, voorlichting aan aanbieders en patiënten, pre-screening om PrEP-geschikte patiënten te identificeren, en het identificeren van personen met een hoog risico in de gemeenschap en het verstrekken van doorverwijzing en koppeling naar eerstelijnszorg.
