Gebruik van point-of-care longechografie om de etiologie van acuut respiratoir falen op een pediatrische intensive care-afdeling te diagnosticeren.

Gedetailleerde beschrijving
Ademhalingsfalen is een van de meest voorkomende aandoeningen die opname op de pediatrische intensive care (PICU) vereisen. Als zodanig is thoraxradiografie naar voren gekomen als het meest gebruikte hulpmiddel bij de beoordeling van longpathologie, ondanks het feit dat het misschien niet het meest nauwkeurige is. Sinds het baanbrekende artikel van Lichtenstein in 2008 is echografie van de longen naar voren gekomen als een alternatief voor thoraxradiografie bij de beoordeling van ernstig zieke volwassenen. Evenzo heeft pediatrische longechografie een groeiend aantal onderzoeken ter ondersteuning van het gebruik ervan bij veel voorkomende longpathologieën, waaronder longontsteking, astma, bronchiolitis, acuut chest syndroom, pleurale effusies en pneumothorax. Ondanks het snel groeiende bewijsmateriaal over het lichaam, blijft er weinig literatuur over om het gebruik ervan voor de diagnose en behandeling van acuut respiratoir falen op de PICU te ondersteunen.
De voorgestelde studie zal evalueren of point-of-care long-echografie nauwkeurig is bij het bepalen van de etiologie van acuut respiratoir falen bij opname op de PICU.
Specifieke doelen zijn onder andere:
- Doel 1: Vaststellen van de sensitiviteit en specificiteit van point-of-care longechografie bij het identificeren van de etiologie van acuut respiratoir falen bij kinderen bij opname op de PICU.
- Doel 2: Vaststellen van de interobserverbetrouwbaarheid van de bevindingen van point-of-care longechografie bij acute pediatrische respiratoire insufficiëntie bij stagiaire en deskundige sonografen.
- Doel 3: Vergelijk point-of-care longechografie met thoraxradiografie wat betreft de detectiesnelheid van consolidatie, interstitieel oedeem, pneumothorax en pleurale effusie.
- Doel 4: Beschrijf de POC-LUS bevindingen bij patiënten opgenomen op de PICU met acute respiratoire insufficiëntie

Inclusiecriteria:
- Leeftijd langer dan 37 weken zwangerschapsduur en minder dan 18 jaar
- Opgenomen op de PICU
- Klinische diagnose van acuut ademhalingsfalen
- Vereiste van niet-invasieve of invasieve ademhalingsondersteuning (zoals gedefinieerd door een klinische behoefte aan high-flow neuscanule >1L/kg/min; RAM-canule, neus- of volgelaatsmasker leverde continue positieve luchtwegdrukventilatie; RAM-canule, nasaal of volledig gezicht masker toegediend bi-level positieve luchtwegdrukventilatie of invasieve mechanische ventilatie); behoefte aan aanvullende zuurstof met FiO2 > 0,35 terwijl op high flow neuscanule <1L/kg/min om saturaties > 90% te behouden; continue verneveltherapie; of chronisch gebruik van thuiszuurstof of beademingsondersteuning EN elke toename van de thuissituatie
Uitsluitingscriteria:
- Hemodynamisch onstabiel zoals gedefinieerd door de initiatie van of de behoefte aan verhoogde vasopressorondersteuning binnen de voorgaande 30 minuten
- Bekende chronische ademhalingsziekte zoals primaire ciliaire dyskinesie, cystische fibrose of aangeboren longmisvormingen.

Kinderen met acute respiratoire insufficiëntie
Point-of-care long-echo bij opname op de PICU om de oorzaak van respiratoire insufficiëntie vast te stellen.
|
Bij opname (niet later dan 14 uur na opname, rekening houdend met stabilisatie en zorg van de deelnemer) op de PICU en binnen 6 uur na eventuele escalatie van mechanische beademingsondersteuning, wordt een point-of-care long-echoonderzoek uitgevoerd door onderzoeksonderzoekers die blind zijn voor de geschiedenis van de deelnemer, lichamelijk onderzoek en diagnostische testen/beeldvorming en die niet betrokken zijn bij de klinische behandeling van de deelnemer.
Onderzoekers zullen een gestandaardiseerd scanprotocol met zes zones en patroonanalyse gebruiken om een diagnose te stellen voor de oorzaak van de ademhalingsinsufficiëntie van de deelnemer.
Afbeeldingen zullen worden verkregen door stagiaires voor pediatrische intensive care en online worden opgeslagen; nauwkeurigheid van diagnoses op basis van het echografisch onderzoek zal worden geëvalueerd door een deskundige echoscopist offline.
Na de ICU-rondes in de ochtend wordt de PICU-medewerker of behandelend arts die voor de deelnemer zorgt, gevraagd naar zijn/haar interpretatie van de meest recente thoraxfoto en zijn/haar diagnose met betrekking tot de etiologie van de acute respiratoire insufficiëntie van de deelnemer.
Deze klinische diagnose zal worden vergeleken met echografische bevindingen.
Resultaten van thoraxfoto's worden verzameld ter vergelijking met bevindingen van echografie.

De specificiteit van de longechografie bij het identificeren van de etiologie van acuut respiratoir falen bij kinderen zal worden bepaald door vergelijking met het uiteindelijke criterium of de "gouden standaard"-diagnose die wordt gegenereerd door een geblindeerde beoordeling van de kaart na ontslag door een onderzoeksonderzoeker die blind is voor de casus.

Echografiebeelden verkregen door een intensive care-stagiair worden online opgeslagen voor beoordeling door een deskundige echoscopist.
De deskundige echoscopist leest de beelden over; bevindingen zullen op alle echografiepunten worden vergeleken om het percentage overeenstemming te bepalen bij het interpreteren van de bevindingen van echografisch onderzoek tussen de stagiair en de deskundige echoscopist.
Bovendien zal de definitieve echografie-diagnose worden vergeleken tussen stagiaire en deskundige echoscopisten.
Interpretatie van onderzoeksbevindingen en bepaling van onderzoeksdiagnose zullen worden vergeleken om de interobserver-betrouwbaarheid van point-of-care long-echo-onderzoeken bij acute pediatrische respiratoire insufficiëntie te bepalen.
De hypothese is dat de interobserverbetrouwbaarheid tussen leerling- en deskundige sonografen voor point-of-care longecho-bevindingen >80% zal zijn.
