Veiligheid en werkzaamheid van de maagreactantie (XL) bij de voorspelling van morbimortaliteit bij patiënten na een electieve hartoperatie
Evalueer de veiligheid en effectiviteit van de XL-trend gemeten door Florence (Critical Perfusion Inc, Palo Alto, Californië) bij de voorspelling van morbimortaliteit van Mexicaanse patiënten na een operatie of electieve cardiovasculaire chirurgie.
Hypothese: 1. De maagreactantiemeting (XL) correleert met de morbimortaliteit (postoperatieve shock, overmatig bloeden, vasoplegisch syndroom en overlijden) en met de risicovoorspellers (APACHE II, STS, SOFA en EUROSCORE II) bij patiënten na een electieve hartoperatie. 2. Het is mogelijk om het afkappunt van de waarden van de maagreactantie (XL) te identificeren als een voorspellend instrument voor morbimortaliteit bij patiënten na een operatie of een electieve hartoperatie. 3. De maagreactantie (XL) is een veilige meting voor patiënten die een hartoperatie ondergaan.

Maximaal 35 patiënten met een leeftijd groter dan of gelijk aan 18 jaar met electieve chirurgie (valvulaire chirurgie, revascularisatie of een combinatie van beide) die voldoen aan de opname-/uitsluitingscriteria van dit protocol, zullen worden ingeschreven.
Onderzoeksapparaat: Florence (Gastric Impedance Spectrometer System of ISMO). Vergelijkend met risicoschalen SOFA, APACHE II, STS, Euroscore II, hemodynamische variabelen, lactaat en gemengde veneuze verzadiging.
De Florence-katheter wordt geplaatst aan het begin van de operatie en tot 72 uur op de IC.

Beschrijving
Inclusiecriteria:
- Vrouwen en mannen binnen een leeftijd gelijk aan of ouder dan 18 jaar.
- Proefpersonen die zijn ingepland voor een electieve hartoperatie voor klepchirurgie, revascularisatie of een combinatie van beide.
- Proefpersonen die voldoen aan de indicatie om een drijvende pulmonale arteriekatheter te plaatsen.
- Markeert resultaten voorafgaand aan de operatie: STS-sterfterisico gelijk aan of groter dan 6% van de dag ervoor of Euro Score II met een sterfterisico gelijk aan of groter dan 6% of linkerventrikel-ejectiefractie (LVEF/FEVI) minder dan 45% (patiënten met hartfalen of disfunctie van het rechterventrikel of een combinatie van beide), systolische excursie van het tricuspidalisringvlak (TAPSE) minder dan 17 mm, systolische pieksnelheid van de zijring van de tricuspidalis gemeten met Tissue Doppler (LVOT/TSVI) minder dan 0,1 m/s; patiënt met een electieve operatie van: aorta- en mitralisklepvervanging of aorta- en mitralisklepvervanging + tricuspidalisklepplastiek of aortaklepvervanging + mitralisklepplastiek of mitralisklepvervanging + tricuspidalisklepplastiek of aorta-, mitralis- en tricuspidalisklepvervanging of aortaklepvervanging + tricuspidalisklepplastiek of dubbele klepvervanging (elke combinatie) + revascularisatie of aortaklepvervanging + revascularisatie + mitralisklepplastiek.
- Proefpersoon is niet ingeschreven in een ander onderzoeksprotocol.
- Geïnformeerde toestemming is ondertekend door de proefpersoon voorafgaand aan de studieprocedures.
- Proefpersoon in sinusritme voor de operatie.
Uitsluitingscriteria:
- Proefpersonen met gegevens van recente bloedingen in de spijsverteringsbuis (afgelopen 30 dagen).
- Personen met een dwarslaesie of hemiplegie.
- Proefpersonen met een aangeboren achtergrond (interauriculaire communicatie, ventriculair atriumkanaal, coarctatie van de aorta, aanhoudende arteriële ductus, pulmonale stenose, ventrikelseptumdefect, tetralogie van fallot, totale of gedeeltelijke abnormale pulmonale veneuze drainage, transpositie van de grote vaten, tricuspidalisatresie, arteriële kofferbak, enz.).
- Maxillofaciale misvorming.
- Katheterplaatsing mislukt.
- Bekende zwangerschap of ontdekte zwangerschap na opname (vóór de operatie).
- Vrouw in borstvoedingsperiode.
- Proefpersonen die niet voldoen aan de klinische indicatie voor plaatsing van een neusgastrische katheter of gecontra-indiceerd zijn voor het plaatsen van een neusgastrische katheter.
- Proefpersonen met een geïmplanteerde pacemaker of permanente defibrillator.
- Medicatievoorziening die het contact tussen de Florence-katheter en het maagslijmvlies belemmert (barrière-effect) zoals sucralfaat, aluminiumhydroxide, bismutsubsalicylaat, magaldraat, enz.
