Beheer van plaatjesaggregatieremmers tijdens chirurgische ingrepen (MARS-register)
Er zijn tegenwoordig belangrijke verschillen in de klinische praktijk met betrekking tot het perioperatieve beheer van post-PCI-patiënten op DAPT die NCS ondergaan. Bovendien zijn er aanzienlijke verschillen tussen overbruggingsmiddelen en verbeterde veiligheid van DES van de huidige generatie. Gezien de aanzienlijke beperkingen van de huidige studies, zal een goed ontworpen register om de huidige praktijkpatronen in de VS en de bloedings- of ischemische risico's die samenhangen met de verschillende peri-operatieve DAPT-beheerstrategieën, waaronder overbrugging en geen overbrugging, aanzienlijk bijdragen aan het begrip van het huidige probleem en de bijbehorende risico's. van patiënten. Deze gegevens zouden de basis kunnen vormen voor het uitvoeren van toekomstige gerandomiseerde klinische onderzoeken (RCT's) op dit gebied. NCS bij post-PCI-patiënten met chronische DAPT vertoont een complexe interactie van zowel ischemische als bloedingsrisico's.
Het MARS-register zal het gebied van peri-operatieve plaatjesaggregatieremmers bestuderen door middel van een multicenter observationeel nationaal register dat is ontworpen om pre-operatieve, intra-operatieve en postoperatieve klinische strategieën, therapeutische interventies en resultaatgegevens te verzamelen. Dit voorstel schetst de rol van een zeer ervaren klinisch onderzoeksteam om een multicenter prospectieve registratie uit te voeren. Deze vraag is zeer relevant voor een diverse groep medische zorgverleners en miljoenen patiënten in de VS en de rest van de wereld; dit zal het eerste Amerikaanse register zijn dat dit uiterst belangrijke volksgezondheidsprobleem aanpakt.
Studiedoelen:
i. Gegevens verzamelen over de hedendaagse klinische praktijk in de VS over het beheer van DAPT na percutane coronaire interventie (PCI) bij patiënten die NCS en CS moeten ondergaan.
ii. Om ischemische en bloedingseindpunten in deze groep patiënten tijdens de studieperiode te beoordelen.
iii. Om initiële gegevens te genereren om plannen te maken voor toekomstige grootschalige gerandomiseerde klinische onderzoeken naar perioperatief beheer van DAPT.

- Registratie-infrastructuur: het voorgestelde register is een multicenter prospectief register dat is ontworpen om peri-operatieve DAPT-beheerstrategieën vast te leggen bij patiënten die NCS of CS post-PCI ondergaan. Het zal worden beoordeeld en gecontroleerd op juistheid en volledigheid van de gegevens. Het register wordt geanonimiseerd en voldoet aan de HIPPA-regelgeving en bevindt zich op beveiligde servers van het University of Texas Southwestern Medical Center (UTSW). Fulltime medewerkers, waaronder een IT-functionaris en een dataprogrammeur, beheren de registerservers professioneel. Er wordt dagelijks een back-up van de servers gemaakt en ze bieden veilige online toegang aan site-onderzoekers en coördinatoren. De huidige registerstructuur is al in gebruik voor een nationaal register voor perifere arterieziekte en kan worden bekeken op www.XLPAD.org. Het MARS-register zal twee afzonderlijke registerarmen bevatten om gegevens over twee verschillende cohorten vast te leggen: (i) NCS en (ii) CS.
- Hypothese: Dit observationele register is ontworpen om te testen of 30-daagse NACE geassocieerd met IV APT-overbrugging ≥6% is.
- Studieperiode: periode van 14 dagen voor de operatie tot 30 dagen na de operatie.

- Perifere arteriële revascularisatie (bijv. aorta-aneurysmareparatie*, vasculaire bypass*, halsslagaderoperatie)
- Urologische chirurgie (bijv. prostatectomie*, blaastumorresectie);
- Grote ingreep (bijv. nier-, long- of levertransplantatie*, longbiopsie, hoofd-halsoperatie*)
- Borstoperaties en andere operaties of procedures die DAPT-onderbreking vereisen volgens het klinische team *NCS met hoog risico, blijft geclassificeerd als laag-gemiddeld

- Studiepopulatie: Post-PCI-patiënten op DAPT die NCS en CS ondergaan. Patiënten zullen worden gevraagd om een formulier voor geïnformeerde toestemming (ICF) of ICF-uitstel te ondertekenen op basis van het mandaat van hun respectievelijke institutionele beoordelingsraden (IRB).
- Verzameling van registergegevens: Het register zal worden gebouwd op het REDCap-gegevensverzamelingsportaal van de NHLBI dat beschikbaar is voor UTSW onder een Academic Information Systems NIH-subsidie UL1-RR024982. Het voorgestelde MARS-registerportaal voor gegevensverzameling. Het portaal voor gegevensverzameling is ontworpen om uitgebreide gegevensverzameling mogelijk te maken vanaf 2 weken voorafgaand aan de operatie tot 30 ± 7 dagen na de operatie en omvat demografische gegevens, laboratoriumgegevens, coronaire revascularisatie, medicatie en klinische uitkomstinformatie. Baseline-informatie omvat demografische, relevante medische geschiedenis, preoperatieve medicatie en laboratoriumwaarden en alle preoperatieve klinische gebeurtenissen tijdens de onderzoeksperiode. Proceduregerelateerde gegevens omvatten details van de chirurgische procedure, intraoperatieve medicatie en medicijnen, laboratoriumwaarden en klinische resultaten vanaf de dag van de operatie tot ontslag uit het ziekenhuis. Postoperatieve gegevensverzameling omvat medicijnen, laboratoriumwaarden en klinische gebeurtenissen tussen ontslag uit het ziekenhuis en 30 dagen na ontslag. Gegevens worden ingevoerd in een veilig en HIPPA-compatibel online REDCap-portaal voor gegevensverzameling. Er zijn geen studiegerelateerde procedures, interventies of follow-ups nodig. Elk locatiepersoneel voert gegevens in op basis van elektronische medische dossiers (EHR), die zullen dienen als de primaire gegevensbron. Medische dossiers buiten het ziekenhuis moeten worden opgenomen of gedocumenteerd als onderdeel van de brongegevens van een bepaalde deelnemer. Alle deelnemers krijgen de gelegenheid om hun beslissingen met betrekking tot deelname aan het MARS-register opnieuw te evalueren. Het MARS-register zal twee afzonderlijke takken van het register bevatten om gegevens over twee verschillende cohorten vast te leggen: (i) NCS en (ii) CS.

- Gepland voor NCS of CS.
- Bereid en in staat om geïnformeerde toestemming te geven (indien nodig op basis van institutionele IRB-vereisten).
Uitsluitingscriteria:
- Opkomende (6 uur) operatie post-PCI op DAPT die een grondig geïnformeerd toestemmingsproces uitsluit.
