Volgens het National Institute of Diabetes and Digestive and Kidney Diseases hebben meer dan 30 miljoen mensen in de VS diabetes en iets meer dan 84 miljoen mensen pre-diabetes. Tegelijkertijd hebben 30 staten en het District of Columbia cannabis voor medisch en/of recreatief gebruik gelegaliseerd en in de afgelopen tien jaar is het cannabisgebruik onder volwassenen meer dan verdubbeld.
De perceptie van het publiek en sommige wetenschappelijke gegevens suggereren dat cannabis acuut overeten veroorzaakt, waardoor de bezorgdheid ontstaat dat de publieke en legale acceptatie van cannabisgebruik de zwaarlijvigheidsepidemie in de Verenigde Staten zal verergeren, waar meer dan tweederde van de Amerikaanse volwassenen (68,8%) momenteel overgewicht of obesitas. Paradoxaal genoeg laten cross-sectionele gegevens associaties zien tussen chronisch cannabisgebruik en een lagere body mass index (BMI), de prevalentie van obesitas, insulineresistentie, tailleomtrek en werkelijke percentages diabetes type 2, ondanks gegevens die acuut een hogere calorie-inname ondersteunen.
Deze studie onderzoekt de effecten van cannabinoïde-niveaus in het bloed op ontsteking en insulinegevoeligheid, zowel acuut als chronisch bij individuen over het hele gewichtsspectrum. Daartoe maakt het onderzoek gebruik van twee observatieontwerpen: 1) een onderzoek naar acute effecten bij intermitterende cannabisgebruikers en 2) een onderzoek waarin huidige cannabisgebruikers gedurende vier weken een van de drie cannabissoorten selecteren en worden vergeleken met een gematchte controlegroep die geen cannabis gebruiken om chronische effecten te bestuderen.

Beschrijving
Inclusiecriteria:
- In staat om geïnformeerde toestemming te geven
- Cannabisgebruikers in onderzoek A moeten sinds 1 januari 2014 minstens één keer cannabis hebben gerookt of verdampt zonder negatieve effecten, maar NIET gebruikt in de afgelopen drie maanden
- Cannabisgebruikers in onderzoek B moeten gedurende minimaal een jaar een regelmatige (minstens wekelijkse) gebruiker zijn geweest
- Niet-gebruikers in onderzoek B mogen het voorgaande jaar geen cannabis hebben gebruikt
- Gewicht stabiel (fluctuatie <5 pond in de afgelopen zes maanden)
- Ik ben van plan om de komende maand in het Boulder-Denver gebied te blijven
- Nuchtere bloedglucose hoger dan of gelijk aan 55 mg/dl en lager dan of gelijk aan 126 mg/dl
- Cannabisgebruikers in Studie A moeten kennis van de procedure(s) voor het roken of vapen van cannabis onderschrijven
Uitsluitingscriteria:
- Bekende auto-immuunziekte
- Melding van ander drugsgebruik (cocaïne, opiaten, methamfetamine) in de afgelopen 90 dagen of urinescreening mislukt voor een van deze drugs
- Dagelijkse gebruiker van tabak (sigaretten, e-cigs, rookloos), gezien de invloed van het roken van tabak op de insulinefunctie
- Bloedalcoholgehalte hoger dan 0 bij screening
- Huidig gebruik van medicijnen voor glucoseverlaging, immunosuppressie of ontstekingsremmers
- Acute ziekte
- Huidig gebruik van psychotrope medicijnen
- Huidige diagnose diabetes
- Zwaar drinken zoals gedefinieerd door een Alcohol Use Disorders Test (AUDIT)
- Vrouwtjes kunnen niet zwanger zijn of proberen zwanger te worden
- Vrouwtjes kunnen geen zogende moeders zijn
- Bloed hebt gedoneerd in de 8 weken voor het onderzoek of van plan bent om bloed te doneren in de 8 weken na het onderzoek.

Studie B
Steekproef van huidige cannabisgebruikers en niet-gebruikers, gebalanceerd over het gewichtsspectrum en gematcht op leeftijd, geslacht, BMI en fysieke activiteit.

Deelnemers kiezen of ze een cannabisbloemproduct vier weken ad libitum willen gebruiken of vier weken geen cannabis willen gebruiken.
Deelnemers die ervoor kiezen cannabis te gebruiken, wordt gevraagd een van de drie cannabistoppen met verschillende THC- en CBD-niveaus te kopen en te gebruiken: 1) CBD (~14% CBD/0% THC), 2) THC (~14% THC/0). % CBD), of 3) THC+CBD (~7% THC/7% CBD).

Calculation of Insulin Sensitivity using changes in fasting plasma glucose (FPG) and fasting plasma insulin (FPI). Measurements are collected on venous blood samples at 0, 30, 60, 90, and 120 minutes after ingestion of 75 grams oral glucose during oral glucose tolerance test (OGTT). Values across the timepoints were combined to create a Matsuda Index score as follows: 10000/sqrt ((FPG X FPI) X (Mean OGTT glucose concentration X Mean OGTT insulin concentration)). Higher scores mean higher sensitive to insulin and that your body more efficiently clears glucose from your bloodstream, thus higher scores are better. |

The SF-12 Health Survey is a 12-item questionnaire used to assess general health and well-being and includes domains of physical functioning, role-physical, pain, general health, vitality, social functioning, role-emotional and mental health.
Only the mental component was analyzed.
Scores are calculated using a weighted algorithm with scores ranging from 0 to 100; scores greater than 50 meaning better than average health and scores worse than 50 meaning worse than average health.

Scores above 80 indicate a good diet, and scores below 80 indicate a diet in need of improvement.
