Secondo il National Institute of Diabetes and Digestive and Kidney Diseases, oltre 30 milioni di persone negli Stati Uniti hanno il diabete e poco più di 84 milioni di persone hanno il pre-diabete. Contemporaneamente, 30 stati e il Distretto di Columbia hanno legalizzato la cannabis per uso medico e/o ricreativo e negli ultimi dieci anni il consumo di cannabis tra gli adulti è più che raddoppiato.
La percezione pubblica e alcuni dati scientifici suggeriscono che la cannabis causi un'eccessiva alimentazione, creando preoccupazione che l'accettazione pubblica e legale del consumo di cannabis peggiorerà l'epidemia di obesità negli Stati Uniti, dove più di due terzi degli adulti statunitensi (68,8%) sono attualmente sovrappeso o obeso. Paradossalmente, i dati trasversali dimostrano associazioni tra uso cronico di cannabis e indice di massa corporea inferiore (BMI), prevalenza di obesità, resistenza all'insulina, circonferenza della vita e tassi effettivi di diabete di tipo 2 nonostante i dati supportino un apporto calorico più elevato.
Questo studio esamina gli effetti dei livelli di cannabinoidi nel sangue sull'infiammazione e sulla sensibilità all'insulina sia acuta che cronica in individui di tutto lo spettro di peso. A tal fine, lo studio impiega due disegni osservazionali: 1) Uno studio sugli effetti acuti con consumatori intermittenti di cannabis e 2) Uno studio in cui gli attuali consumatori di cannabis selezioneranno uno dei tre ceppi di cannabis per quattro settimane e verranno confrontati con un gruppo di controllo abbinato. che non usano la cannabis per studiare gli effetti cronici.

Descrizione
Criterio di inclusione:
- In grado di fornire il consenso informato
- I consumatori di cannabis nello Studio A devono aver fumato o svapato cannabis almeno una volta dal 1° gennaio 2014 senza effetti negativi ma NON utilizzata negli ultimi tre mesi
- I consumatori di cannabis nello Studio B devono essere consumatori regolari (almeno settimanalmente) da almeno un anno
- I non utilizzatori dello Studio B non possono aver consumato cannabis nell'anno precedente
- Peso stabile (<5 libbre di fluttuazione negli ultimi sei mesi)
- Ho intenzione di rimanere nell'area di Boulder-Denver per il prossimo mese
- Glicemia a digiuno maggiore o uguale a 55 mg/dl e minore o uguale a 126 mg/dl
- I consumatori di cannabis nello Studio A devono approvare la conoscenza delle procedure per fumare o vaporizzare cannabis
Criteri di esclusione:
- Malattia autoimmune nota
- Rapporto sull'uso di altre droghe (cocaina, oppiacei, metanfetamine) negli ultimi 90 giorni o fallimento dello screening delle urine per una qualsiasi di queste droghe
- Utilizzatore quotidiano di tabacco (sigarette, sigarette elettroniche, senza fumo), dato l'impatto del fumo di tabacco sulla funzione dell'insulina
- Livello di alcol nel sangue superiore a 0 allo screening
- Uso corrente di farmaci per abbassare il glucosio, immunosoppressione o antinfiammatori
- Malattia acuta
- Uso corrente di farmaci psicotropi
- Diagnosi attuale di diabete
- Bere pesante come definito da un test sui disturbi da uso di alcol (AUDIT)
- Le femmine non possono essere incinte o tentare di rimanere incinte
- Le femmine non possono essere madri che allattano

Ai partecipanti viene chiesto di acquistare e utilizzare una delle tre varietà di fiori di cannabis con diversi livelli di THC e CBD: 1) CBD (~14% CBD/0% THC), 2) THC (~14% THC/0% CBD), oppure 3) THC+CBD (~7% THC/7% CBD).

Campione di attuali consumatori e non consumatori di cannabis bilanciati in tutto lo spettro di peso che sono abbinati per età, sesso, indice di massa corporea e attività fisica.

I partecipanti scelgono se utilizzare un prodotto a base di fiori di cannabis ad libitum per quattro settimane o non utilizzare cannabis per quattro settimane.

Calculation of Insulin Sensitivity using changes in fasting plasma glucose (FPG) and fasting plasma insulin (FPI). Measurements are collected on venous blood samples at 0, 30, 60, 90, and 120 minutes after ingestion of 75 grams oral glucose during oral glucose tolerance test (OGTT). Values across the timepoints were combined to create a Matsuda Index score as follows: 10000/sqrt ((FPG X FPI) X (Mean OGTT glucose concentration X Mean OGTT insulin concentration)). Higher scores mean higher sensitive to insulin and that your body more efficiently clears glucose from your bloodstream, thus higher scores are better. |

The SF-12 Health Survey is a 12-item questionnaire used to assess general health and well-being and includes domains of physical functioning, role-physical, pain, general health, vitality, social functioning, role-emotional and mental health.
Only the mental component was analyzed.
Scores are calculated using a weighted algorithm with scores ranging from 0 to 100; scores greater than 50 meaning better than average health and scores worse than 50 meaning worse than average health.

Interviewer administered recall measure developed by the University of Minnesota Nutrition Coordinating Center (NCC), that facilitates the standardized collection of 24-hour dietary recall data.
Healthy Eating Index (HEI) total scores are reported for each participant, which is a measure of healthy eating on a scale of 0 (poor eating) to 100 (very healthy eating).
Scores above 80 indicate a good diet, and scores below 80 indicate a diet in need of improvement.
