Het algemene doel van deze studie is het uitvoeren van een gerandomiseerde klinische studie (RCT) ter evaluatie van een hulpmiddel voor de behandeling van tuberculose (TB-TST), een app voor mobiele telefoons die is ontwikkeld met behulp van gebruikersgerichte ontwerpprincipes en een op papier gebaseerde urineteststrip voor geneesmiddelmetabolieten. aangepast voor thuisgebruik voor het testen van de aanwezigheid van metabolieten van isoniazide-geneesmiddelen in de urine om de therapietrouw direct te controleren, om de behandelingsresultaten te verbeteren voor patiënten met tbc die een zelftoedienende behandeling (SAT) krijgen.
Slechte medicatietrouw aan tbc-regimes, samen met uitdagingen bij het monitoren van patiënten en het weer in behandeling nemen van patiënten, zijn belangrijke factoren die bijdragen aan slechte resultaten en de ontwikkeling van resistentie tegen geneesmiddelen. Met de vooruitgang en verspreiding van mobiele technologieplatforms is er aanzienlijke belangstelling voor het mogelijke gebruik van mobiele gezondheidsinterventies (mHealth) om deze uitdagingen aan te pakken. Van de mHealth-benaderingen die worden onderzocht voor het monitoren van de therapietrouw van tuberculose, is het testen van geneesmiddelmetabolieten geïdentificeerd als de meest veelbelovende, ethische en nauwkeurige, en de minst opdringerige en stigmatiserende strategie in vergelijking met andere mobiele oplossingen, maar het potentieel ervan blijft grotendeels onontgonnen. Bovendien kunnen mobiele applicaties (apps) gepersonaliseerde behandelingssupervisie bieden, het zelfmanagement van patiënten vergroten en de communicatie tussen patiënt en zorgverlener verbeteren door meer geavanceerde functionaliteiten voor patiëntondersteuning en -bewaking aan te bieden.
De bestaande versie van de TB-TST-app biedt onderwijs over tbc en de behandeling ervan, communicatie met een zorgcoördinator, houdt therapietrouw bij (zowel door zelfrapportage als door middel van directe metabolietteststripafbeeldingen), zelfrapportage van bijwerkingen van de behandeling en bewaart de "dagboek" -aantekeningen van de patiënt. Dit voorstel bouwt voort op voorbereidend werk om: 1) de TB-TST-interventie te verfijnen op basis van de bevindingen van pilotstudies en principes van gebruikersgericht ontwerp toe te passen; 2) Evalueer de impact van de TB-TST op behandelresultaten in vergelijking met gebruikelijke zorg; 3) Beoordeel de percepties van patiënten en aanbieders van de facilitators en belemmeringen voor de implementatie van de TB-TST en synthetiseer geleerde lessen met belanghebbenden en beleidsmakers. Het primaire resultaat is het succes van de behandeling. Secundaire uitkomsten zijn onder meer: standaardbehandelingspercentages, zelfgerapporteerde therapietrouw, gebruik van technologie en bruikbaarheid. Bevindingen hebben bredere implicaties, niet alleen voor de therapietrouw van tbc, maar ook voor ziektebeheer in het algemeen, en zullen ons begrip verbeteren van hoe we patiënten kunnen ondersteunen die te maken hebben met uitdagende behandelingsregimes

Inclusiecriteria:
- Deelnemers moeten minimaal 16 jaar oud zijn,
- een nieuwe diagnose van geneesmiddelgevoelige tuberculose hebben,
- Eerste behandeling
- regelmatig toegang hebben tot een smartphone, en
- de telefoon kan bedienen of iemand kan helpen.
Uitsluitingscriteria:
- Kinderen tot 15 jaar
- Herbehandeling (standaard of eerder mislukte behandeling)
- Patiënten die ernstig ziek zijn (d.w.z. die ziekenhuisopname nodig hebben)
- Patiënten die in hetzelfde huishouden wonen als een andere studiedeelnemer
- Onvermogen om een smartphone te bedienen
- Analfabetisme (onvermogen om te lezen en te schrijven)
- Patiënten met bekende geneesmiddelresistentie
- Patiënten met een bekende hiv-co-infectie worden uitgesloten omdat hun zorg afzonderlijk wordt beheerd.
- Van gescreende patiënten die niet voldoen aan de studiegeschiktheid, zullen specifieke screeninggegevens (inclusief geslacht, leeftijd en reden voor uitsluiting) worden ingevoerd in de onderzoeksdatabase om redenen voor uitsluiting en haalbaarheid van inschrijvingscriteria te onderzoeken.
Casusdefinitie: Patiënten van ten minste 16 jaar oud met tuberculose bevestigd door uitstrijkje-positief sputum of diagnose van longtuberculose op basis van radiologische bevindingen en klinische tekenen en symptomen, maar met negatief sputumuitstrijkje. De diagnose kan worden bevestigd met andere methoden, zoals MGIT960, BACTEC 9000 of MB Bact, nucleïnezuuramplificatie (PCR) of ELISA.
