TB 治療支援ツール: インタラクティブなモバイル アプリの改良と評価、および TB 治療結果の直接遵守モニタリング
この研究の全体的な目標は、無作為化臨床試験 (RCT) を実施して、結核治療支援ツール (TB-TST)、ユーザー中心の設計原則を使用して開発された携帯電話アプリ、および紙ベースの薬物代謝物尿検査ストリップを評価することです。自己投与治療 (SAT) を受けている結核患者の治療結果を改善するために、尿中のイソニアジド薬物代謝物の存在をテストして治療への遵守を直接監視するために家庭用に変更されました。
結核レジメンへの服薬アドヒアランスの低下は、患者のモニタリングと治療への復帰における課題とともに、転帰不良と薬剤耐性の発生の重要な要因です。 モバイル テクノロジー プラットフォームの進歩と普及に伴い、これらの課題に対処するためのモバイル ヘルス (mHealth) 介入の使用の可能性に大きな関心が寄せられています。 結核アドヒアランス モニタリングのために調査中の mHealth アプローチの中で、薬物代謝物検査は、他のモバイル ソリューションと比較して、最も有望で、倫理的で、正確であり、最も侵入的で汚名を着せない戦略であると特定されていますが、その可能性はまだほとんど調査されていません。 さらに、モバイル アプリケーション (アプリ) は、患者のサポートと監視のためのより高度な機能を提供することで、パーソナライズされた治療の監視を提供し、患者の自己管理を強化し、患者とプロバイダーのコミュニケーションを改善する可能性があります。

- 参加者は 16 歳以上である必要があります。
- 薬剤感受性結核の新たな診断を受けている、
- 最初の治療
- スマートフォンへの定期的なアクセスがあり、
- 電話を操作できるか、誰かに手伝ってもらうことができます。
除外基準:
- 15歳までのお子様
- 再治療(デフォルトまたは以前の治療失敗)
- 重篤な患者(入院が必要な患者)
- 別の研究参加者と同じ世帯に住んでいる患者
- スマートフォンの操作ができない
- 非識字(読み書きができない)
- 薬剤耐性が判明している患者
- HIV 重複感染が判明している患者は、ケアが個別に管理されているため、除外されます。
- 研究の適格性を満たさないスクリーニングされた患者は、特定のスクリーニングデータ(性別、年齢、除外の理由を含む)を研究データベースに入力して、除外の理由と登録基準の実現可能性を調べます。
症例定義:16 歳以上の患者で、喀痰塗抹陽性または X 線所見と臨床徴候および症状に基づいて肺結核と診断されたが、喀痰塗抹陰性である。 診断は、MGIT960、BACTEC 9000またはMB Bact、核酸増幅(PCR)またはELISAなどの他の方法によって確認され得る。
