TB-behandlingsstøtteverktøy Interaktiv mobilapp og direkte overvåking av TB-behandlingsresultater (TB-TST)
Støtteverktøy for tuberkulosebehandling: Forfining og evaluering av en interaktiv mobilapp og direkte overvåking av etterlevelse av resultater av tuberkulosebehandling
Det overordnede målet med denne studien er å gjennomføre en randomisert klinisk studie (RCT) for å evaluere et støtteverktøy for tuberkulosebehandling (TB-TST), en mobiltelefonapp utviklet ved bruk av brukersentrerte designprinsipper og en papirbasert urinteststrimmel for medikamentmetabolitt modifisert for hjemmebruk for å teste tilstedeværelsen av isoniazid-medikamentmetabolitter i urin for direkte å overvåke overholdelse av behandling, for å forbedre behandlingsresultater for pasienter med TB som får selvadministrert behandling (SAT).
Dårlig medisinoverholdelse av TB-regimer, sammen med utfordringer med å overvåke pasienter og returnere dem til behandling, er viktige medvirkende faktorer til dårlige resultater og utvikling av medikamentresistens. Med fremskritt og spredning av mobile teknologiplattformer, er det betydelig interesse for mulig bruk av mobile helseintervensjoner (mHealth) for å møte disse utfordringene. Av mHealth-tilnærmingene som undersøkes for overvåking av TB-overholdelse, har medikamentmetabolitttesting blitt identifisert som den mest lovende, etiske og nøyaktige, og den minst påtrengende og stigmatiserende strategien sammenlignet med andre mobile løsninger, men potensialet forblir stort sett uutforsket. I tillegg kan mobilapplikasjoner (apper) gi personlig tilpasset behandlingsovervåking, øke pasientens selvstyring og forbedre pasient-leverandørkommunikasjonen ved å tilby mer avanserte funksjoner for pasientstøtte og overvåking.
Den eksisterende versjonen av TB-TST-appen tilbyr opplæring om tuberkulose og dens behandling, kommunikasjon med en omsorgskoordinator, sporer behandlingsoverholdelse (både ved selvrapportering og direkte metabolittteststrimmelbilder), selvrapporterer behandlingsbivirkninger, og beholder pasientens "dagbok" notater. Dette forslaget bygger på foreløpig arbeid for å: 1) Avgrense TB-TST-intervensjonen basert på pilotstudiefunn og anvende prinsipper for brukersentrert design; 2) Evaluer virkningen av TB-TST på behandlingsresultater sammenlignet med vanlig behandling; 3) Vurder pasientens og leverandørens oppfatning av fasilitatorene og barrierene for implementering av TB-TST og syntetisere erfaringer med interessenter og beslutningstakere. Primært resultat vil være behandlingssuksess. Sekundære utfall vil inkludere: misligholdsrater for behandling, selvrapportert overholdelse, teknologibruk og brukervennlighet. Funn har bredere implikasjoner, ikke bare for TB-overholdelse, men sykdomsbehandling mer generelt og vil forbedre vår forståelse av hvordan vi kan støtte pasienter som står overfor utfordrende behandlingsregimer

Inklusjonskriterier:
- Deltakere må være minst 16 år,
- har en ny diagnose av stoffmottaglig tuberkulose,
- Første behandling
- har regelmessig tilgang til en smarttelefon, og
- kunne betjene telefonen eller ha noen som kan hjelpe.
Ekskluderingskriterier:
- Barn opptil 15 år
- Rebehandling (standard eller tidligere behandlingssvikt)
- Pasienter som er alvorlig syke (dvs. krever sykehusinnleggelse)
- Pasienter som bor i samme husstand med en annen studiedeltaker
- Manglende evne til å betjene en smarttelefon
- Analfabetisme (manglende evne til å lese og skrive)
- Pasienter med kjent medikamentresistens
- Pasienter med kjent HIV co-infeksjon vil bli ekskludert fordi deres omsorg administreres separat.
- Undersøkte pasienter som ikke oppfyller studiekvalifikasjonen, vil ha spesifikke screeningsdata (inkludert kjønn, alder og årsak til eksklusjon) lagt inn i studiedatabasen for å undersøke årsaker til ekskludering og gjennomførbarhet av registreringskriterier.
Kasusdefinisjon: Pasienter minst 16 år gamle med tuberkulose bekreftet av utstryk-positivt oppspytt eller diagnose av lunge-TB basert på radiologiske funn og kliniske tegn og symptomer, men med negativ oppspyttutstryk. Diagnosen kan bekreftes med andre metoder, slik som MGIT960, BACTEC 9000 eller MB Bact, nukleinsyreamplifikasjon (PCR) eller ELISA.
