Ferramentas de suporte ao tratamento da TB: refinamento e avaliação de um aplicativo móvel interativo e monitoramento direto da adesão aos resultados do tratamento da tuberculose
O objetivo geral deste estudo é conduzir um Ensaio Clínico Randomizado (RCT) para avaliar uma ferramenta de suporte ao tratamento da tuberculose (TB-TST), um aplicativo de telefone celular desenvolvido usando princípios de design centrado no usuário e uma tira de teste de urina de metabólito de drogas baseada em papel modificado para uso doméstico para testar a presença de metabólitos da droga isoniazida na urina para monitorar diretamente a adesão ao tratamento, para melhorar os resultados do tratamento para pacientes com TB recebendo tratamento autoadministrado (SAT).
A má adesão à medicação para os regimes de TB, juntamente com os desafios no monitoramento dos pacientes e no retorno deles ao tratamento, são importantes fatores que contribuem para os maus resultados e o desenvolvimento de resistência aos medicamentos. Com os avanços e a proliferação de plataformas de tecnologia móvel, há um interesse substancial no possível uso de intervenções de saúde móvel (mHealth) para enfrentar esses desafios. Das abordagens de mHealth sob investigação para o monitoramento da adesão à TB, o teste de metabólitos de drogas foi identificado como a estratégia mais promissora, ética e precisa, e a menos intrusiva e estigmatizante em comparação com outras soluções móveis, mas seu potencial permanece amplamente inexplorado. Além disso, os aplicativos móveis (apps) podem fornecer supervisão personalizada do tratamento, aumentar o autogerenciamento dos pacientes e melhorar a comunicação paciente-profissional, oferecendo funcionalidades mais avançadas para suporte e monitoramento do paciente.
A versão existente do aplicativo TB-TST oferece educação sobre TB e seu tratamento, comunicação com um coordenador de cuidados, rastreia a adesão ao tratamento (tanto por auto-relato quanto por imagens diretas de tiras de teste de metabólito), auto-relato de efeitos colaterais do tratamento e retém as anotações do "diário" do paciente. Esta proposta baseia-se no trabalho preliminar para: 1) Refinar a intervenção TB-TST com base nos resultados do estudo piloto e aplicar princípios de design centrado no usuário; 2) Avaliar o impacto do TB-TST nos resultados do tratamento em comparação com os cuidados habituais; 3) Avaliar as percepções dos pacientes e provedores sobre os facilitadores e barreiras para a implementação do TB-TST e sintetizar as lições aprendidas com as partes interessadas e formuladores de políticas. O desfecho primário será o sucesso do tratamento. Os resultados secundários incluirão: taxas de inadimplência do tratamento, adesão autorrelatada, uso e usabilidade da tecnologia. As descobertas têm implicações mais amplas não apenas para a adesão à TB, mas também para o gerenciamento da doença de maneira mais geral e melhorarão nossa compreensão de como apoiar pacientes que enfrentam regimes de tratamento desafiadores

Critério de inclusão:
- Os participantes devem ter pelo menos 16 anos de idade,
- tem um novo diagnóstico de TB suscetível a medicamentos,
- primeiro tratamento
- ter acesso regular a um smartphone e
- ser capaz de operar o telefone ou ter alguém capaz de ajudar.
Critério de exclusão:
- Crianças até 15 anos
- Retratamento (falha de tratamento anterior ou padrão)
- Pacientes gravemente doentes (ou seja, que requerem hospitalização)
- Pacientes que residem na mesma casa com outro participante do estudo
- Incapacidade de operar um smartphone
- Analfabetismo (incapacidade de ler e escrever)
- Pacientes com resistência conhecida a medicamentos
- Os pacientes com co-infecção conhecida por HIV serão excluídos porque seus cuidados são gerenciados separadamente.
- Os pacientes selecionados que não atendem à elegibilidade do estudo terão dados de triagem específicos (incluindo sexo, idade e motivo da exclusão) inseridos no banco de dados do estudo para examinar os motivos da exclusão e a viabilidade dos critérios de inscrição.
Definição de caso: Pacientes com pelo menos 16 anos de idade com TB confirmada por baciloscopia positiva ou diagnóstico de TB pulmonar com base em achados radiológicos e sinais e sintomas clínicos, mas com baciloscopia negativa. O diagnóstico pode ser confirmado por outros métodos, como MGIT960, BACTEC 9000 ou MB Bact, amplificação de ácidos nucleicos (PCR) ou ELISA.
