Cerebrale monitoring en postoperatief delirium en resultaten (Techno-5)
Validatie van cerebrale niet-invasieve monitoring en voorspelling van postoperatief delirium en uitkomsten: een prospectieve observatiestudie

Overzicht van de problematiek: Postoperatief delirium bij hartchirurgie Hersenmonitoring met behulp van NIRS is in 2002 gestart in de operatiekamers van het MHI. Sinds die periode beschreven de onderzoekers onze ervaring met 1613 gemelde patiënten uit gepubliceerde casusrapporten (n=30), cohortstudies (n=1270), gerandomiseerde gecontroleerde onderzoeken (n=313) en overzichtsartikelen.
In de aanwezigheid van hersendesaturatie hebben de onderzoekers eerder een algoritmische benadering beschreven die is gebaseerd op een reeks interventies (zoals positionering van het hoofd, verhoging van de bloeddruk en ingeademde zuurstof enz.) die worden geactiveerd door de etiologie, een voor een of gelijktijdig, idealiter zo vroeg mogelijk, d.w.z. wanneer de cerebrale NIRS-waarde met meer dan 10% begint af te nemen. Het slagingspercentage voor correctie van hersendesaturatie was meer dan 95%.
Het gebruik van zowel cerebrale NIRS als pEEG leidde tot nieuwe inzichten die niet zichtbaar waren wanneer deze technologieën afzonderlijk werden gebruikt. Bij het ontwaken zal bijvoorbeeld hersenverzadiging als gevolg van verhoogde pEEG-activiteit worden waargenomen. Aan de andere kant kan desaturatie van de hersenen geassocieerd met verminderde pEEG-activiteit duiden op hypoperfusie van de hersenen. In mei 2019 werd een nieuw algoritme geïntroduceerd en gepubliceerd om cerebrale NIRS en pEEG te beheren.
Dit algoritme is niet gevalideerd en zou een grote vooruitgang kunnen betekenen in de zorg voor hartchirurgische patiënten. Een van de mogelijke rollen van dit type monitoring zou het vermogen zijn om ontluikend delirium, postoperatief delirium en andere eindorgaancomplicaties te voorspellen. Uiteindelijk zou de vroege identificatie kunnen leiden tot het voorkomen van postoperatieve complicaties zoals delirium.
Hersenoximetrie en delirium Tussen 16% en 30% van de oudere gehospitaliseerde patiënten met hart- en vaatziekten zal een delirium ontwikkelen. Delirium en cognitieve disfunctie komen zeer vaak voor bij gehospitaliseerde patiënten in alle zorgomgevingen, maar vooral in chirurgische en kritieke zorgpopulaties waar de percentages naar verluidt oplopen tot 70%. De mortaliteit is verhoogd bij patiënten met delirium na een hartoperatie.
Delirium wordt door de DSM-5 gedefinieerd als een acute hersendisfunctie geassocieerd met aandachtstekort en veranderd bewustzijn. Er zijn drie soorten delirium beschreven: hypoactief geassocieerd met lethargie, hyperactief met agitatie en een combinatie van beide die elkaar kunnen afwisselen. Er zijn verschillende predisponerende factoren, zoals preoperatieve cognitieve disfunctie. Een recente systematische review heeft de volgende factoren geïdentificeerd die verband houden met het optreden van delirium. Die factoren omvatten gevorderde leeftijd, eerdere psychiatrische aandoening, milde cognitieve stoornissen en cerebrovasculaire aandoeningen. Postoperatief delirium gaat gepaard met langdurige ziekenhuisopname, postoperatieve complicaties en verhoogde mortaliteit.
Er zijn verschillende rapporten geweest die een verband leggen tussen verminderde cerebrale NIRS-metingen, delirium en postoperatieve cognitieve disfunctie bij chirurgische populaties. Het mechanisme waardoor verminderde hersenverzadiging en delirium naast elkaar bestaan, kan cerebrale perfusie en veneuze congestie door disfunctie van het rechterhart omvatten. Deze factoren zijn onlangs onderzocht en hun associatie met delirium en postoperatieve cognitieve stoornissen is zowel op de IC als in de operatiekamer gemeld. Aangezien disfunctie van het rechterhart kan worden geassocieerd met een langdurige periode van veneuze hypertensie, kan dit leiden tot interstitieel oedeem dat de perfusie van eindorganen, zoals de hersenen, verder kan aantasten. In de context van cardiopulmonale bypass (CPB) kan dit fenomeen worden verergerd door een wijziging in de endotheliale glycocalyx, wat resulteert in een verhoogde permeabiliteit van het capillaire endotheel. Aangenomen wordt dat het resulterende interstitiële oedeem een bemiddelaar is van nadelige resultaten bij ernstig zieke patiënten met vochtoverbelasting. Naast veranderde NIRS-waarden waargenomen bij delirium na hartchirurgie, is abnormale elektrische hersenactiviteit beschreven.
Elektro-encefalografische monitoring van de hersenen en delirium Elektrische activiteit van de hersenen kan worden gecontroleerd door middel van elektro-encefalografie (EEG) en recentelijk maakt een bedmonitor van pEEG, zoals de BIS (Medtronic, VS) of Sedline (Masimo Irvine CA) metingen aan het bed van frontale elektrische activiteit mogelijk. Delirium wordt geassocieerd met EEG-veranderingen, meestal een wereldwijde vertraging van de EEG-activiteit. Significante verschillen in EEG zijn waargenomen bij ouderen met of zonder delirium. Het alfa-, theta- en deltagolfvermogen en de theta-ratio in relatie tot de andere EEG-golven zijn verschillend bij patiënten met of zonder delirium. Bovendien is een significante vermindering van de EEG-activiteit die leidt tot "burst-onderdrukking" in verband gebracht met het optreden van delirium, verlengde opnameduur in het ziekenhuis en mortaliteit. Er zijn controversiële resultaten gerapporteerd uit onderzoeken en meta-analyses waarin werd gekeken naar interventie om die factoren te corrigeren en de uitkomst te verbeteren. Deze waarnemingen hebben echter nooit informatie verkregen uit frontale EEG gecombineerd met hersenoximetrie. Een recente prospectieve studie uit België rapporteerde een associatie met postoperatief delirium en de informatie verkregen uit het gecombineerde gebruik van hersen-NIRS en pEEG.
Eerder werk van delirium en hersenoximetrie en elektro-encefalografische monitoring In 2011 rapporteerden de onderzoekers onze eerste ervaring met het gebruik van NIRS bij 61 patiënten die off-pump bypass-hartchirurgie ondergingen als voorspeller van postoperatieve cognitieve disfunctie. In 2016 werd een observationele studie uitgevoerd met 30 opeenvolgende volwassenen met delirium na een hartoperatie. De gemiddelde oximetriewaarde nam af van (gemiddelde ± SD) 66,4 ± 6,7 tot 50,8 ± 6,8 op de eerste dag na het begin van het delirium en nam toe bij patiënten bij wie het delirium in de loop van de 3 dagen resorbeerde. De relatie tussen oximetrie, deliriumdiagnose en ernst werd geanalyseerd met een marginaal model en lineaire gemengde modellen. Cerebrale oximetrie was gerelateerd aan delierdiagnose (p≤.0001) en ernst (p≤.0001). In die studie waren de somatische oximetriewaarden normaal bij patiënten met delirium.
De tweede studie, in 2017, was een case-control retrospectieve studie met 346 patiënten, van wie 39 (11%), 104 (30%) en 142 (41%) patiënten na 24, 48 en 72 uur delirium vertoonden. post-IC-aankomst, respectievelijk. De cumulatieve vochtbalans was geassocieerd met het optreden van delirium (OR 1,20, 95% BI: 1,066-1,355, p=.003). Geschiedenis van neurologische aandoening, met zowel gehoor- als visuele beperking, type procedure, peri-operatieve cerebrale oximetrie, gemiddelde longslagaderdruk vóór CPB en gemiddelde arteriële druk na CPB droegen ook bij aan delirium in het model.
In de derde studie werd zowel een retrospectief als een prospectief cohort gerapporteerd met in totaal 382 patiënten. Volwassen patiënten die een portale veneuze pulsed-wave Doppler ondergingen door de behandelend arts tijdens de gebruikelijke zorg op de IC, werden opgenomen in het retrospectieve cohort (n=237). Voor het prospectieve cohort (n=145) ondergingen patiënten een cognitieve en echocardiografische evaluatie de dag vóór de operatie en dagelijks gedurende drie dagen na de operatie. Delirium werd onafhankelijk beoordeeld door het verplegend personeel in het prospectieve cohort. In het retrospectieve cohort werd een verband gevonden tussen delirium en pulsatiliteit van de poortader, een marker van rechterhartfalen (OR:2.69). BI:1.47-4.90 p=0,001). In het prospectieve cohort werden significante associaties bevestigd tussen de aanwezigheid van pulsatiliteit van de poortader en de ontwikkeling van cognitieve disfunctie en asterixis beoordeeld door de onderzoekers (OR:2.10 BI:1.25-3.53 p=0,005 en OR:5,19 [BI:2,27; 11,88]p<0,001) Maar ook het door het verplegend personeel geconstateerde verband met delirium werd bevestigd (HR:2.63 CI:1.13-6.11 p=0,025). Hogere NT-pro-BNP metingen (OR:4.03 [BI: 1.78-9.15] p=0,001) en cerebrale desaturaties (OR:2,54 [CI:1.12-5.76] p=0,03) waren geassocieerd met cognitieve disfunctie. In deze derde studie werden dagelijkse baseline pEEG dagelijks verkregen in combinatie met NIRS-waarden. Er werd echter geen verband waargenomen tussen delirium en voorlopige golfvormanalyse.
Samenvattend ondersteunen die onderzoeken een verband tussen episodes van cerebrale desaturatie gemeten door hersenoximetrie en het optreden van delirium. Veneuze cerebrale congestie als gevolg van rechterventrikeldisfunctie, leidend tot portale hypertensie en mogelijk cytokine-afgifte via een cardio-intestinaal syndroom, lijkt een potentieel mechanisme te zijn. Zoals ondersteund door andere auteurs, kunnen verminderde cerebrale NIRS-waarden secundair zijn aan verhoogde veneuze druk, vochtophoping en de laatste dragen bij aan een soort congestief delirium. Het kan ook een weerspiegeling zijn van een verhoogd cerebraal metabolisme, zoals kan worden waargenomen wanneer de patiënt na een hartoperatie uit algehele anesthesie komt. In die onderzoeken werd echter slechts één enkele cerebrale NIRS en pEEG-waarde verkregen. De combinatie van beide modaliteiten met het gebruik van bilaterale cerebrale NIRS is zowel in de operatiekamer als op de IC niet prospectief onderzocht. Momenteel is het niet bekend of de informatie die door deze monitoren wordt verkregen, anders is bij patiënten met een deliriumepisode dan bij patiënten die geen enkele episode van postoperatief delirium vertonen.

- Mannelijke of vrouwelijke patiënten van 18 jaar en ouder die een hartoperatie ondergaan met CPB of off-pump CABG
Uitsluitingscriteria:
- Patiënt met een kritieke preoperatieve toestand gedefinieerd als een van de volgende: vasopressorbehoefte, mechanische ondersteuning van de bloedsomloop, dialyse, mechanische beademing, hartstilstand die reanimatie noodzakelijk maakt, afgebroken plotseling overlijden, preoperatieve hartmassage, preoperatieve intra-aortale ballonpomp.
