Een studie van TAK-103 bij volwassenen met solide tumoren
Een open-label dosisescalatie, eerste fase 1-onderzoek bij mensen van TAK-103 bij volwassen patiënten met gevorderde of gemetastaseerde vaste tumoren die mesotheline tot expressie brengen
In deze studie zullen mensen met gevorderde of uitgezaaide solide tumoren die mesotheline tot expressie brengen, TAK-103 krijgen met hun witte bloedcellen. De belangrijkste doelen van deze studie zijn na te gaan of de deelnemers bijwerkingen krijgen van de behandeling met TAK-103 en na te gaan hoeveel TAK-103 deelnemers kunnen krijgen zonder er bijwerkingen van te krijgen. Onderzoekers kunnen dan de beste dosis TAK-103 bepalen om deelnemers aan toekomstige studies te geven.
Bij het eerste bezoek bekijkt de onderzoeksarts wie er kan deelnemen. Voor degenen die kunnen deelnemen, zullen de onderzoeksartsen bij elke deelnemer witte bloedcellen verzamelen. Deze cellen worden naar het laboratorium gestuurd waar TAK-103 aan de cellen van elke deelnemer wordt toegevoegd. Dit kan wel 4 a 5 weken duren. Deelnemers kunnen specifieke behandelingen krijgen terwijl deelnemers wachten op TAK-103. Vervolgens krijgen de deelnemers TAK-103 met hun cellen langzaam via een ader toegediend (infuus). 4 verschillende kleine groepen deelnemers zullen lagere tot hogere doses TAK-103 krijgen. Elke deelnemer krijgt slechts 1 dosis. De onderzoeksartsen zullen na elke verschillende dosis TAK-103 controleren op bijwerkingen. Op deze manier kunnen onderzoekers bepalen wat de beste dosis TAK-103 is om aan deelnemers aan toekomstige studies te geven.
Deelnemers blijven voor hun behandeling 28 dagen of langer in het ziekenhuis. Daarna bezoeken de deelnemers de kliniek voor regelmatige controles gedurende maximaal 3 jaar.

Inclusiecriteria
- Histologisch of cytologisch bevestigde gevorderde of uitgezaaide solide tumoren die geen keus hebben of intolerant zijn voor standaardtherapieën met een bewezen klinisch voordeel.
- Mesotheline-expressie (>=50% positief op levensvatbare tumorcellen) moet plaatselijk op de tumor worden bepaald door middel van immunohistochemie met behulp van een gevalideerde assay, scoren en kleuren bevestigd door de sponsor. Voor de geschiktheidsbeoordeling moet een vers biopsiemonster worden gebruikt, tenzij er een gearchiveerd biopsiemonster beschikbaar is dat binnen 6 maanden voorafgaand aan de leukafereseprocedures is verkregen.
- Levensverwachting >=12 weken.
- Eastern Cooperative Oncology Group prestatiestatus van 0 of 1.
Adequate orgaanfunctie zoals bevestigd door klinische laboratoriumwaarden zoals hieronder gespecificeerd:
- Totaal bilirubine =<1,5 × de bovengrens van het normale bereik (ULN) behalve bij deelnemers met het syndroom van Gilbert. Deelnemers met het syndroom van Gilbert kunnen zich inschrijven met direct bilirubine =<3 × ULN van het directe bilirubine. Verhoogd indirect bilirubine als gevolg van hemolyse na transfusie is toegestaan
Alanineaminotransferase (ALT) of aspartaataminotransferase (AST) moet <3 × ULN zijn.
AST en ALT kunnen verhoogd zijn tot 5 × ULN als de verhoging redelijkerwijs kan worden toegeschreven aan de aanwezigheid van metastatische ziekte in de lever.
- Berekende creatinineklaring >50 ml/min (formule Cockcroft-Gault).
- Hemoglobine moet >=9 g/dL zijn.
- Het aantal neutrofielen moet >1000/mm^3 zijn.
- Het absolute aantal lymfocyten moet >500/mm^3 zijn.
- Het aantal bloedplaatjes moet >75.000/mm^3 zijn.
- Deelnemers moeten radiografisch meetbare ziekte hebben zoals gedefinieerd door Response Evaluation Criteria in Solid Tumors, versie 1.1 (RECIST 1.1).
Uitsluitingscriteria
- Actieve systemische infecties.
- Bekende hepatitis B-oppervlakteantigeen (HBsAg)-positieve, of bekende of vermoede actieve infectie met het hepatitis C-virus (HCV). Deelnemers met een positief hepatitis B-kernantilichaam (HBcAb) of hepatitis B-oppervlakte-antilichaam (HBsAb) kunnen worden ingeschreven, maar moeten een niet-detecteerbare virale belasting van het hepatitis B-virus (HBV) hebben. Deelnemers met positieve hepatitis C-virusantistoffen (HCVAb) moeten een niet-detecteerbare HCV-virale belasting hebben.
- Stollingsstoornissen of andere ernstige medische ziekten, waaronder aandoeningen van de luchtwegen of het immuunsysteem.
- Deelnemers met bekende cardiovasculaire en cardiopulmonale aandoeningen gedefinieerd als onstabiele angina, klinisch significante aritmie, myocardinfarct, congestief hartfalen, linkerventrikelejectiefractie (LVEF) <45%, basislijn zuurstofverzadiging <93% op kamerlucht. Een goed gecontroleerde atriale fibrillatie zou geen uitsluiting zijn, terwijl ongecontroleerde atriale fibrillatie een uitsluiting zou zijn.
- Deelnemers met tekenen van lymfoom en/of leukemie.
- Deelnemers die binnen 3 jaar vóór leukaferese-procedures worden gediagnosticeerd met of behandeld voor een andere maligniteit. Deelnemers met niet-melanome huidkanker of carcinoma in situ (bijv. baarmoederhals, blaas, borst) zouden worden opgenomen als ze adequaat werden behandeld.
- Elke ziekte die systemische behandeling met steroïden vereist.
- Elk eerder gebruik van cel- en gentherapie(ën).
- Behandeling met alle onderzoeksproducten (behalve cel- of gentherapie) binnen 14 dagen vóór leukafereseprocedures of 28 dagen vóór behandeling met conditionerende chemotherapie/TAK-103.
- Systemische antikankertherapie (inclusief immuno-oncologische therapieën) en behandeling met radiotherapie binnen 14 dagen vóór leukafereseprocedures of behandeling met conditionerende chemotherapie/TAK-103.
- Behandeling met een grote operatie binnen 28 dagen vóór leukaferese-procedures of behandeling met conditionerende chemotherapie/TAK-103 (kleine chirurgische ingrepen zoals het plaatsen van een katheter zijn geen uitsluitingscriteria).
- Eerdere behandeling met een op mesotheline gerichte therapie.
- Elke onopgeloste toxiciteit van graad 3 of hoger van eerdere antikankertherapie.
- Deelnemers met risico op bloedingen zoals beoordeeld door de onderzoeker.
- Aanwezigheid van metastasen van het centrale zenuwstelsel of andere significante neurologische aandoeningen (Deelnemer met metastasen van het centrale zenuwstelsel die waar nodig effectief zijn behandeld en stabiel zijn, kunnen worden ingeschreven).
- Deelnemers met seropositief humaan immunodeficiëntievirus (HIV) en/of seropositief humaan T-cellymfotroop virus (HTLV).
- Deelnemers met een voorgeschiedenis van orgaantransplantatie of in afwachting van orgaantransplantatie.
- Deelnemers met ernstige onmiddellijke overgevoeligheid voor een van de middelen, waaronder cyclofosfamide, fludarabine of streptomycine.

Experimenteel: TAK-103, dosisescalatie
TAK-103, chimere antigeenreceptor (CAR) (+) cellen, intraveneuze infusie, zullen worden toegediend met een snelheid van ongeveer 5 ml/min.
Er zijn 5 geplande dosisniveaus: 1x10^6, 3x10^6, 1x10^7, 1x10^8 en 5x10^8 CAR (+) cellen/lichaam.
Het niveau van de dosis bij dosisescalatie wordt geleid door het dosisescalatieschema op basis van de waargenomen dosisbeperkende toxiciteiten (DLT) op elk dosisniveau.

DLTs were evaluated according to National Cancer Institute Common Terminology Criteria for Adverse Events (NCI CTCAE) version 5.0.
Any Grade 3 or higher toxicities that were considered by the investigator to be at least possibly related to therapy with TAK-103 during the 28 days immediately after infusion of TAK-103 and, any adverse events (AEs) requiring endotracheal intubation or tracheostomy were considered as DLTs.

In this study, immune effector cell-associated neurotoxicity syndrome (ICANS), cytokine release syndrome (CRS), hemophagocytic lymphohistiocytosis (HLH), macrophage activation syndrome (MAS), and tumor lysis syndrome (TLS) were predefined as AECIs.
CRS and ICANS were evaluated according to American Society for Transplantation and Cellular Therapy (ASTCT) consensus.
For management of HLH and MAS, CARTOX (CAR-T-cell-therapy-associated TOXicity) recommendations were used.
