Un estudio de TAK-103 en adultos con tumores sólidos
Un estudio abierto, de aumento de dosis, fase 1, primero en humanos de TAK-103 en pacientes adultos con tumores sólidos avanzados o metastásicos que expresan mesotelina
En este estudio, las personas con tumores sólidos avanzados o metastásicos que expresan mesotelina recibirán TAK-103 con sus glóbulos blancos. Los objetivos principales de este estudio son verificar si los participantes tienen algún efecto secundario del tratamiento con TAK-103 y verificar cuánto TAK-103 pueden recibir los participantes sin tener efectos secundarios. Luego, los investigadores pueden calcular la mejor dosis de TAK-103 para administrar a los participantes en estudios futuros.
En la primera visita, el médico del estudio comprobará quién puede participar. Para aquellos que puedan participar, los médicos del estudio recolectarán glóbulos blancos de cada participante. Estas células se envían al laboratorio donde se agrega TAK-103 a las células de cada participante. Esto puede tomar hasta 4 o 5 semanas. Los participantes pueden recibir tratamientos específicos mientras esperan el TAK-103. Luego, los participantes recibirán TAK-103 con sus células lentamente a través de una vena (infusión). 4 grupos pequeños diferentes de participantes recibirán dosis más bajas a más altas de TAK-103. Cada participante solo recibirá 1 dosis. Los médicos del estudio verificarán los efectos secundarios después de cada dosis diferente de TAK-103. De esta forma, los investigadores pueden calcular la mejor dosis de TAK-103 para administrar a los participantes en estudios futuros.
Los participantes permanecerán en el hospital durante 28 días o más para su tratamiento. Luego, los participantes visitarán la clínica para chequeos regulares por hasta 3 años.

Descripción
Criterios de inclusión
- Tumores sólidos avanzados o metastásicos confirmados histológica o citológicamente que no tienen opción o son intolerantes a las terapias estándar con un beneficio clínico comprobado.
- La expresión de mesotelina (>=50% positivo en células tumorales viables) debe determinarse localmente en el tumor mediante inmunohistoquímica utilizando un ensayo validado, puntuación y tinción confirmadas por el patrocinador. Se debe usar una muestra de biopsia fresca para la evaluación de elegibilidad, a menos que esté disponible una muestra de biopsia archivada obtenida dentro de los 6 meses anteriores a los procedimientos de leucoaféresis.
- Esperanza de vida >=12 semanas.
- Estado funcional del Eastern Cooperative Oncology Group de 0 o 1.
Función adecuada de los órganos confirmada por los valores de laboratorio clínico como se especifica a continuación:
- Bilirrubina total = <1,5 × el límite superior del rango normal (ULN), excepto en los participantes con síndrome de Gilbert. Los participantes con síndrome de Gilbert pueden inscribirse con bilirrubina directa = <3 × ULN de la bilirrubina directa. Se permite la bilirrubina indirecta elevada debido a la hemólisis posterior a la transfusión
La alanina aminotransferasa (ALT) o la aspartato aminotransferasa (AST) debe ser <3 × LSN.
La AST y la ALT pueden estar elevadas hasta 5 × ULN si la elevación puede atribuirse razonablemente a la presencia de enfermedad metastásica en el hígado.
- Depuración de creatinina calculada > 50 ml/min (fórmula de Cockcroft-Gault).
- La hemoglobina debe ser >=9 g/dL.
- El recuento de neutrófilos debe ser >1000/mm^3.
- El recuento absoluto de linfocitos debe ser >500/mm^3.
- El recuento de plaquetas debe ser >75.000/mm^3.
- Los participantes deben tener una enfermedad medible radiográficamente según lo definido por los Criterios de Evaluación de Respuesta en Tumores Sólidos, Versión 1.1 (RECIST 1.1).
Criterio de exclusión
- Infecciones sistémicas activas.
- Antígeno de superficie de hepatitis B conocido (HBsAg) positivo, o infección activa conocida o sospechada por el virus de la hepatitis C (VHC). Los participantes que tengan un anticuerpo central de la hepatitis B (HBcAb) positivo o un anticuerpo de superficie de la hepatitis B (HBsAb) pueden inscribirse, pero deben tener una carga viral del virus de la hepatitis B (VHB) indetectable. Los participantes que tengan anticuerpos contra el virus de la hepatitis C (HCVAb) positivos deben tener una carga viral del VHC indetectable.
- Trastornos de la coagulación u otras enfermedades médicas importantes, incluidas enfermedades respiratorias o del sistema inmunitario.
- Participantes con enfermedad cardiovascular y cardiopulmonar conocida definida como angina inestable, arritmia clínicamente significativa, infarto de miocardio, insuficiencia cardíaca congestiva, fracción de eyección del ventrículo izquierdo (FEVI) <45 %, saturación de oxígeno inicial <93 % con aire ambiente. Una fibrilación auricular bien controlada no sería una exclusión, mientras que una fibrilación auricular no controlada sería una exclusión.
- Participantes con cualquier signo de linfoma y/o leucemia.
- Participantes a los que se les diagnostica o se les trata otra neoplasia maligna dentro de los 3 años anteriores a los procedimientos de leucaféresis. Se incluirían participantes con cáncer de piel no melanoma o carcinoma in situ (p. ej., de cuello uterino, vejiga, mama) si recibieron el tratamiento adecuado.
- Cualquier enfermedad que requiera tratamiento con esteroides sistémicos.
- Cualquier uso anterior de terapias celulares y génicas.
- Tratamiento con cualquier producto en investigación (excepto terapia celular o génica) dentro de los 14 días anteriores a los procedimientos de leucoféresis o 28 días antes del tratamiento con quimioterapia de acondicionamiento/TAK-103.
- Terapia anticancerosa sistémica (incluidas las terapias de inmunooncología) y tratamiento con radioterapia dentro de los 14 días anteriores a los procedimientos de leucoaféresis o tratamiento con quimioterapia de acondicionamiento/TAK-103.
- Tratamiento con cirugía mayor dentro de los 28 días anteriores a los procedimientos de leucoaféresis o tratamiento con quimioterapia de acondicionamiento/TAK-103 (los procedimientos quirúrgicos menores como la colocación de catéteres no son criterios de exclusión).
- Tratamiento previo con cualquier terapia dirigida a la mesotelina.
- Cualquier toxicidad no resuelta de grado 3 o superior de una terapia anticancerígena anterior.
- Participantes con riesgo de hemorragia a juicio del investigador.
- Presencia de metástasis en el sistema nervioso central u otras afecciones neurológicas significativas (se puede inscribir a un participante con metástasis en el sistema nervioso central que se haya tratado de manera efectiva cuando sea necesario y estable).
- Participantes con virus de la inmunodeficiencia humana (VIH) seropositivos y/o virus linfotrópico de células T humanas (HTLV) seropositivos.
- Participantes con antecedentes de trasplante de órganos o en espera de trasplante de órganos.
- Participantes con hipersensibilidad inmediata grave a cualquiera de los agentes, incluidos ciclofosfamida, fludarabina o estreptomicina.

Experimental: TAK-103, Escalada de dosis
TAK-103, células receptoras de antígeno quimérico (CAR) (+), infusión intravenosa, se administrarán a aproximadamente 5 ml/min.
Hay 5 niveles de dosis planificados: 1x10^6, 3x10^6, 1x10^7, 1x10^8 y 5x10^8 CAR (+) células/cuerpo.
El nivel de dosis en el aumento de dosis se guiará por un esquema de aumento de dosis basado en la tasa de toxicidad limitante de dosis (DLT) observada en cada nivel de dosis.

DLTs were evaluated according to National Cancer Institute Common Terminology Criteria for Adverse Events (NCI CTCAE) version 5.0.
Any Grade 3 or higher toxicities that were considered by the investigator to be at least possibly related to therapy with TAK-103 during the 28 days immediately after infusion of TAK-103 and, any adverse events (AEs) requiring endotracheal intubation or tracheostomy were considered as DLTs.

In this study, immune effector cell-associated neurotoxicity syndrome (ICANS), cytokine release syndrome (CRS), hemophagocytic lymphohistiocytosis (HLH), macrophage activation syndrome (MAS), and tumor lysis syndrome (TLS) were predefined as AECIs.
CRS and ICANS were evaluated according to American Society for Transplantation and Cellular Therapy (ASTCT) consensus.
For management of HLH and MAS, CARTOX (CAR-T-cell-therapy-associated TOXicity) recommendations were used.

Partial response (PR): At least a 30 percent (%) decrease in the sum of the longest diameter (LD) of TL, taking as reference the baseline sum LD.

Immune Stable Disease (iSD), which is to be determined if the criteria of iCR or iPR are not met and no tumor progression is present.
