Vergelijking van twee soorten spalken om tandluxatieletsels te behandelen
Vergelijking van bulkvulbaar, vloeibaar en verpakbaar composiet bij het spalken van luxatieverwondingen: een gerandomiseerde controleproef
De doelstellingen van deze gerandomiseerde klinische studie (RCT) waren:
- Vergelijken van de efficiëntie van spalken door twee composietmaterialen in termen van retentie van materiaal
- Om lijmpuntdimensie tussen de twee groepen te vergelijken
- Om de tijd te vergelijken die nodig is om de spalk te plaatsen en te verwijderen
- Om de werkzaamheid van bulkfill flowable te beoordelen bij het verminderen van de mobiliteit van geluxeerde tanden.

Er werd een gerandomiseerde controleproef met een periode van 6 maanden uitgevoerd. Patiënten met luxaties in de leeftijd van 16 tot 50 jaar werden na geïnformeerde toestemming gevraagd om deel te nemen aan het onderzoek. Ze werden vervolgens toegewezen aan een van beide groepen (Groep 1: Packable Composite (PC), Groep 2: Bulkfill Flowable) door middel van een loterijmethode. Periotest werd gebruikt om de mobiliteit van tanden te controleren voordat de spalk werd geplaatst. Het spalken werd uitgevoerd met composietmateriaal volgens groepstoewijzing. De tijd die nodig was om de spalk te plaatsen werd geregistreerd. Vooraanzichtbeelden van gespalkte tanden werden gemaakt met DSLR en overgebracht naar Adobe Photoshop versie 22.4.1 voor vensters om het oppervlak van het restauratiemateriaal te meten als percentage van het totale tandoppervlak. Postoperatieve instructies werden gegeven. Na 2 weken werden de periotestwaarden geregistreerd, onmiddellijk na verwijdering van de spalk. Bij beide bezoeken werden 3 metingen per tand geregistreerd. De tijd die nodig was om de spalk te verwijderen werd ook geregistreerd. Bovendien werd ook de retentie van de spalk waargenomen.
STATISTISCHE ANALYSE:
SPSS versie 26 voor ramen werd gebruikt, de gemiddelde tijd die nodig was om het materiaal aan te brengen en te verwijderen en de mobiliteit voor en na het verwijderen van de spalk werd vergeleken met behulp van de paired samples T-test. De Mann-Whitney-test vergeleek het procentuele oppervlak van het bezette materiaal. Kruistabellen evalueerden de retentie van spalkmateriaal. Het significantieniveau werd op p ≤0,05 gehouden.

Inclusiecriteria:
- Patiënten met een blijvend gebit in de leeftijd van 16-50 jaar
- Ofwel geslacht
- Laterale en extrusieve luxatieletsels
Uitsluitingscriteria:
- Patiënten met de volgende aandoeningen werden uitgesloten
- Verwondingen door inbraak
- Wortel breuk
- Dentoalveolaire fractuur.
- Patiënten met systemische ziekten.
- Patiënten met parodontitis
- Parafunctionele gewoonten
- Niet-toestemmende patiënten

De retentie wordt als volgt gemeten: Volledig verlijmd: geen blootliggende draad op composietmateriaal en de draad bleef aan de tand vastzitten. Gedeeltelijk onthecht: er bleven stukjes composiet achter op de draad en de draad bleef aan de tand vastzitten. Ongebonden: de draad is blootgesteld aan de mondholte zonder composiet erop en de draad is losgemaakt van de tand en vrij beweegbaar. Het resultaat zal worden gemeten als percentage van gevallen die in elke groep worden gemeld met de voorwaarde van belang. |

Zelfklevende puntafmeting

Percentage van het oppervlak dat wordt ingenomen door het materiaal in vergelijking met de tand wordt genoteerd als adhesieve puntafmeting. Het resultaat wordt gemeten als percentage dat wordt ingenomen door het spalkmateriaal van het totale tandoppervlak, zoals zichtbaar vanaf de voorkant. In ons geval wordt de draadspalk vastgehouden door Bulkfill-composiet of Packable-composiet. Daarom is het percentage van het gebied dat wordt ingenomen door het spalkmateriaal de uitkomst van belang. |

Tand mobiliteit

'Beweging van tand in zijn kom als gevolg van een uitgeoefende kracht, meestal gemeten op een schaal van 1-3, of gemeten door de hoeveelheid horizontale en/of verticale mobiliteit in millimeters' De waarden van Periotest bij de beoordeling van tandmobiliteit zijn gebaseerd op de schaal die wordt gegeven als: Graad 0: -0,8- +09 Graad I: +10- +19 Graad II: +20- +29 Graad III: +30- +50 Het resultaat wordt gemeten als percentage van de patiënten met graad van tandmobiliteit vóór het aanbrengen van de spalk en na het verwijderen van de spalk |
