Сравнение двух типов шин для лечения вывихов зубов
Сравнение жидкотекучих и упаковываемых композитов при шинировании вывихов: рандомизированное контрольное исследование
Целями этого рандомизированного клинического исследования (РКИ) были:
- Сравнить эффективность шинирования двумя композиционными материалами по ретенции материала.
- Чтобы сравнить размер точки склеивания между двумя группами
- Сравнить время, затрачиваемое на установку и снятие шины.
- Оценить эффективность текучего наполнителя в снижении подвижности вывихнутых зубов.

Подробное описание
Было проведено рандомизированное контрольное исследование продолжительностью 6 месяцев. Пациентам с вывихами в возрасте от 16 до 50 лет было предложено принять участие в исследовании после получения информированного согласия. Затем они были распределены по группам (группа 1: упаковываемый композит (ПК), группа 2: текучий насыпной материал) методом лотереи. Периотест использовался для проверки подвижности зубов перед наложением шины. Выполнено шинирование композитным материалом в соответствии с групповым распределением. Регистрировалось время, необходимое для наложения шины. Фронтальные изображения шинированных зубов были сделаны с помощью цифровой зеркальной фотокамеры и перенесены в Adobe Photoshop версии 22.4.1 для окон, чтобы измерить площадь поверхности реставрационного материала в процентах от общей площади поверхности зуба. Послеоперационные инструкции были даны. Через 2 недели записывали показания Периотеста сразу после снятия шины. Во время обоих посещений было зарегистрировано 3 показания на зуб. Также фиксировалось время, необходимое для снятия шины. Кроме того, также наблюдалось удержание шины.
СТАТИСТИЧЕСКИЙ АНАЛИЗ:
Использовали SPSS версии 26 для окон, среднее время, необходимое для наложения и удаления материала, и подвижность до и после снятия шины сравнивали с помощью Т-критерия для парных образцов. В тесте Манна-Уитни сравнивалась процентная доля занимаемой площади материала. Перекрестные таблицы оценивали удержание шинного материала. Уровень значимости сохранялся на уровне p ≤0,05.

- Пациенты с постоянным прикусом в возрасте от 16 до 50 лет
- Любой пол
- Боковые и экструзивные вывихи
Критерий исключения:
- Пациенты со следующими состояниями были исключены
- Интрузивные травмы
- Перелом корня
- Дентоальвеолярный перелом.
- Больные с системными заболеваниями.
- Пациенты с пародонтитом
- Парафункциональные привычки
- Несогласные пациенты

Удержание будет измеряться следующим образом: Полная фиксация: проволока не подвергалась контакту с композитным материалом, а проволока оставалась приклеенной к зубу. Частично отслоился: частички композита остались на проволоке, а проволока осталась приклеенной к зубу. Без фиксации: проволока находится в полости рта, на ней нет композита, а проволока отсоединена от зуба и свободно перемещается. Результат будет измеряться как процент случаев, сообщаемых с условием интереса в каждой группе. |

Размер точки приклеивания
Временное ограничение: 2 недели
|
Процент площади, занимаемой материалом по сравнению с зубом, будет отмечен как размер точки адгезии. Результат будет измеряться как процент, занимаемый шинирующим материалом, от общей площади зуба, видимой спереди. В нашем случае проволочная шина удерживается либо композитом Bulkfill, либо композитом Packable. Таким образом, интерес представляет процент площади, занятой шинирующим материалом. |

Подвижность зубов
Временное ограничение: 2 недели
|
«Движение зуба в лунке в результате приложенной силы, обычно измеряемое по возрастающей шкале от 1 до 3 или измеряемое величиной горизонтальной и/или вертикальной подвижности в миллиметрах». Значения Periotest для оценки подвижности зубов основаны на шкале, представленной в виде: Степень 0: -0,8- +09 Степень I: +10- +19 Степень II: +20- +29 Степень III: +30- +50 Результат будет измеряться как процент пациентов со степенью подвижности зубов до наложения шины и после ее снятия. |

Время нанесения: количество времени в минутах от начала шинирования, т.е. от нанесения травителя до отверждения последнего блока композита. Время удаления: время в минутах, необходимое для удаления шинирующего материала. Результат будет измеряться как время, затраченное в мм:сс, т. е. минуты:секунды, на наложение и снятие шины в любой группе. |
