Comparaison de deux types d'attelles pour traiter les blessures de luxation dentaire
Comparaison des composites fluides et compressibles en vrac dans l'attelle des lésions de luxation : un essai contrôlé randomisé
Les objectifs de cet essai clinique randomisé (ECR) étaient :
- Comparer l'efficacité de l'attelle par deux matériaux composites en termes de rétention de matière
- Pour comparer la dimension du point adhésif entre les deux groupes
- Pour comparer le temps nécessaire pour placer et retirer l'attelle
- Évaluer l'efficacité de bulkfill flowable dans la réduction de la mobilité des dents luxées.

Description détaillée
Un essai contrôlé randomisé d'une durée de 6 mois a été réalisé. Les patients souffrant de lésions de luxation âgés de 16 à 50 ans ont été invités à participer à l'étude après avoir obtenu leur consentement éclairé. Ils ont ensuite été attribués à l'un ou l'autre des groupes (Groupe 1 : Packable Composite (PC), Groupe 2 : Bulkfill Flowable) par tirage au sort. Periotest a été utilisé pour vérifier la mobilité des dents avant la pose de l'attelle. L'attelle a été réalisée avec un matériau composite selon l'attribution du groupe. Le temps nécessaire pour placer l'attelle a été enregistré. Des images de vue frontale de dents attelles ont été prises avec DSLR et transférées vers Adobe Photoshop version 22.4.1 pour fenêtres afin de mesurer la surface du matériau de restauration en pourcentage par rapport à la surface totale de la dent. Des consignes post-opératoires ont été données. Après 2 semaines, les lectures du Periotest ont été enregistrées, immédiatement après le retrait de l'attelle. Lors des deux visites, 3 lectures par dent ont été enregistrées. Le temps nécessaire pour retirer l'attelle a également été enregistré. De plus, la rétention de l'attelle a également été observée.
ANALYSES STATISTIQUES:
La version SPSS 26 pour fenêtres a été utilisée, le temps moyen nécessaire pour appliquer et retirer le matériau et la mobilité avant et après le retrait de l'attelle ont été comparés à l'aide du test T pour échantillons appariés. Le test de Mann-Whitney a comparé le pourcentage de surface du matériau occupé. Des tableaux croisés ont évalué la rétention du matériau de l'attelle. Le niveau de signification a été maintenu à p ≤ 0,05.

- Patients en denture permanente âgés de 16 à 50 ans
- Soit le sexe
- Lésions de luxation latérale et extrusive
Critère d'exclusion:
- Les patients avec les conditions suivantes ont été exclus
- Blessures par intrusion
- Fracture radiculaire
- Fracture dento-alvéolaire.
- Patients atteints de maladies systémiques.
- Patients atteints de parodontite
- Habitudes para-fonctionnelles
- Patients non consentants

Rétention d'attelle

|
La rétention sera mesurée comme suit : Entièrement collé : Aucune exposition du fil sur le matériau composite et le fil est resté collé à la dent. Partiellement décollé : les caractéristiques du composite sont restées sur le fil et le fil est resté collé à la dent. Non collé : le fil est exposé à la cavité buccale sans composite et le fil est détaché de la dent et se déplace librement. Le résultat sera mesuré en pourcentage de cas signalés avec une condition d'intérêt dans chaque groupe. |

Dimension du point adhésif
Délai: 2 semaines
|
Le pourcentage de surface occupée par le matériau par rapport à la dent sera noté comme dimension du point adhésif. Le résultat sera mesuré en pourcentage occupé par le matériau d'attelle de la surface totale de la dent telle qu'elle est visible de face. Dans notre cas, l'attelle métallique est retenue par le composite Bulkfill ou le composite Packable. Par conséquent, le pourcentage de surface occupée par le matériau d'attelle est le résultat d'intérêt. |

Mobilité dentaire

|
"Mouvement de la dent dans son alvéole résultant d'une force appliquée, généralement mesuré sur une échelle croissante de 1 à 3, ou mesuré par la quantité de mobilité horizontale et/ou verticale en millimètres" Les valeurs de Periotest dans l'évaluation de la mobilité dentaire sont basées sur l'échelle donnée comme suit : Niveau 0 : -0,8- +09 Niveau I : +10- +19 Niveau II : +20- +29 Niveau III : +30- +50 Le résultat sera mesuré en pourcentage de patients présentant un degré de mobilité dentaire avant l'application de l'attelle et après le retrait de l'attelle |

Temps nécessaire pour appliquer et retirer l'attelle

Temps d'application : temps nécessaire en minutes depuis le début de l'attelle, à partir de l'application du mordançage jusqu'au durcissement du dernier bloc de composite. Temps de retrait : Le temps nécessaire en minutes pour retirer le matériau d'attelle Le résultat sera mesuré en tant que temps pris en mm: ss, c'est-à-dire minute: seconde pour appliquer et retirer l'attelle dans l'un ou l'autre groupe. |
