Implanteerbare hartmonitor om boezemfibrilleren te detecteren bij patiënten met MINOCA (MINOCA)
Myocardinfarct met niet-obstructieve kransslagaders (MINOCA) (d.w.z. <50% stenoses) op coronaire angiografie) is een ondergewaardeerde klinische entiteit waar 5-6% van de patiënten met een acuut myocardinfarct mee te maken heeft. Ongeveer 50% van deze patiënten blijft achter zonder passende diagnose en behandeling.
De MINOCA-studie is gericht op het systematisch beoordelen van de frequentie van onderliggende pathologieën van MINOCA en de resultaten met een multidisciplinaire etiologische work-up en follow-up van 5 jaar, inclusief, voor de eerste keer, een implanteerbare cardiale monitor (ICM) om de frequentie van atriale fibrillatie als onderliggende oorzaak voor MINOCA.

Ongeveer 5-6% van de patiënten met een acuut myocardinfarct (AMI) heeft een myocardinfarct met niet-obstructieve kransslagaders (MINOCA) (d.w.z. <50% stenosen) op coronaire angiografie en tot 50% van deze patiënten blijft zonder passende diagnose en behandeling . Een multidisciplinair etiologisch onderzoek van MINOCA is onlangs voorgesteld in internationale consensusdocumenten. De huidige studie streeft naar een gestructureerde wetenschappelijke gegevensverzameling van een volledig op richtlijnen gebaseerd onderzoek na MINOCA en een follow-up van 5 jaar om de klinische resultaten te beoordelen.
Onbehandeld atriumfibrilleren is een potentieel verwaarloosde onderliggende oorzaak van MINOCA. Aangezien implanteerbare hartmonitors (ICM) boezemfibrilleren met hoge nauwkeurigheid kunnen detecteren, is het doel van deze studie om voor de eerste keer het optreden van de eerste gediagnosticeerde atriumfibrilleren te beoordelen met behulp van ICM bij patiënten met MINOCA.
Om gegevensverzameling voor iedereen mogelijk te maken, zullen patiënten met contra-indicatie(s) voor ICM-implantatie worden opgenomen in de niet-ICM-groep om de frequentie van onderliggende oorzaken van MINOCA en klinische resultaten gedurende 5 jaar te beoordelen.

Inclusiecriteria ICM-groep
- ≥18 jaar
- Schriftelijke geïnformeerde toestemming
- Acuut myocardinfarct (AMI) type 1 volgens de 4e universele definitie van myocardinfarct
- Niet-obstructieve kransslagaders op angiografie gedefinieerd als de afwezigheid van kransslagaderstenoses ≥50% in een mogelijke infarctgerelateerde slagader
- Geen klinisch openlijke specifieke oorzaak voor de acute presentatie
- Subendocardiale of transmurale late gadolinum enhancement (LGE) consistent met een ischemische etiologie op cardiale magnetische resonantie beeldvorming (CMR)
- Geen duidelijke onderliggende oorzaak van MINOCA en daardoor verhoogde kans op boezemfibrilleren
Uitsluitingscriteria ICM-groep:
- Bekende atriale fibrillatie of atriale flutter
- Geschiedenis van atriale fibrillatie of atriale flutter-ablatie
- Bekende coronaire hartziekte
- Vorige MI
- Eerdere percutane coronaire interventie (PCI)
- Eerdere coronaire bypassoperatie (CABG)
- Contra-indicaties voor CMR (d.w.z. niet-MR-compatibel implanteerbaar hartapparaat, glomerulaire filtratiesnelheid (GFR) <30 ml/min)
- Contra-indicaties voor ICM-implantatie
- Duidelijke onderliggende oorzaak van MINOCA vóór ICM-implantatie
Opnamecriteria niet-ICM-groep:
- ≥18 jaar
- Schriftelijke geïnformeerde toestemming
- AMI type 1 volgens de 4e universele definitie van myocardinfarct
- Niet-obstructieve kransslagaders op angiografie gedefinieerd als de afwezigheid van kransslagaderstenoses ≥50% in een mogelijke infarctgerelateerde slagader
- Geen klinisch openlijke specifieke oorzaak voor de acute presentatie
- Subendocardiaal of transmuraal LGE consistent met een ischemische etiologie op CMR

- Bekende coronaire hartziekte
- Vorige MI
- Vorige PGB
- Vorige CABG
