Afasie is een slopende aandoening, die doorgaans het gevolg is van schade aan de linkerhersenhelft, en die een scala aan communicatieve vaardigheden kan aantasten, waaronder taalproductie en -begrip, lezen en schrijven. Jaarlijks worden ongeveer 180.000 nieuwe gevallen van afasie geïdentificeerd, en in de Verenigde Staten leven ongeveer 1 miljoen of 1 op de 250 gevallen met afasie. Behandelingen zijn beperkt en bieden op zijn best bescheiden voordelen. De huidige nadruk bij afasierehabilitatie ligt op het formuleren van intensieve spraak- en taaltherapieën en het vergroten van de therapeutische voordelen, mogelijk met hersenstimulatie in combinatie met therapieën.
De huidige studie zal de werkzaamheid onderzoeken van high-definition tACS (HD-tACS) om de neurale oscillerende activiteit te helpen herstellen bij overlevenden van een beroerte met afasie. TACS verschilt van trancraniële gelijkstroomstimulatie (tDCS), een veelgebruikt paradigma voor hersenstimulatie, doordat sinusoïdale of wisselstromen worden afgegeven in plaats van gelijkstromen. Er is aangetoond dat TACS de voortdurende oscillerende hersenactiviteit manipuleert en ook de synchronisatie (of connectiviteit) tussen beoogde hersengebieden moduleert. Dit kenmerk van tACS is behoorlijk aantrekkelijk, gezien de nieuwe hoeveelheid bewijsmateriaal dat suggereert dat taalstoornissen voortkomen uit verminderde hersenconnectiviteit en daaruit voortvloeiende verstoringen in het taalnetwerk als gevolg van een beroerte.
De studie zal gebruik maken van high-definition tACS (HD-tACS) in een parallel, dubbelblind, schijngecontroleerd ontwerp, gecombineerd met taaltherapie gericht op de fonologische kortetermijngeheugenfunctie (STM) bij overlevenden van een beroerte met afasie. Magneto-encefalografie (MEG) en fMRI BOLD-gegevensverzameling zullen plaatsvinden om respectievelijk tACS-parameters te bepalen en door stimulatie geïnduceerde neurale veranderingen te evalueren. De onderzoekers zijn van plan om 120 overlevenden van een beroerte met afasie te rekruteren in een tACS-studieontwerp met twee groepen.

Beschrijving
Inclusiecriteria:
- Gediagnosticeerd met beroerte/afasie in de linkerhersenhelft
- Toestemmingsdatum >= 1 maand na het begin van de beroerte
- Vloeiend in het Engels
- 18 jaar of ouder
Uitsluitingscriteria:
- Ernstige cognitieve, auditieve of visuele beperkingen die cognitieve en taaltesten uitsluiten
- Aanwezigheid van een ernstige, onbehandelde of onstabiele psychiatrische ziekte
- Een chronische medische aandoening die niet wordt behandeld of die instabiel is
- De aanwezigheid van hartstimulatoren of pacemakers
- Contra-indicaties voor MRI of tACS, b.v. patiënten met metalen implantaten en/of een voorgeschiedenis van schedelfracturen, zwangerschap, huidziekten
- Geschiedenis van aanhoudende of onbeheerde aanvallen
- Geschiedenis van dyslexie of andere ontwikkelingsstoornissen

Experimenteel: High Definition tACS met kortetermijngeheugengerichte logopedie
High-Definition-tACS wordt geleverd via een op batterijen werkende wisselstroomstimulator (Soterix) met behulp van twee 3x1 center-surround montages. De stroom wordt ingeschakeld en op een hellingachtige manier verhoogd gedurende ongeveer 30 seconden.
Deelnemers ondergaan gedurende 20 minuten tACS-stimulatie met een piek-tot-piek-intensiteit van 2 milliampère (mA).
De stimulatie zal niet langer dan 20 minuten worden volgehouden.
Dit zal gepaard gaan met kortetermijngeheugengerichte logopedie.

Tijdens logopedie zal High Definition tACS worden toegepast.

Sham-vergelijker: Sham-High Definition tACS met kortetermijngeheugengerichte logopedie
High-Definition-tACS wordt geleverd via een op batterijen werkende wisselstroomstimulator (Soterix) met behulp van twee 3x1 center-surround montages.
De stroom wordt ingeschakeld en op een hellingachtige manier gedurende 10 tot 30 seconden verhoogd en vervolgens afgebouwd.
Op deze manier ervaren de deelnemers dezelfde initiële sensaties (milde tintelingen) als de actieve tACS-groepen.
Dit zal gepaard gaan met kortetermijngeheugengerichte logopedie.
