Atriale fibrillatie (AF), ook bekend als atriale flutter, is een van de meest voorkomende hartritmestoornissen en vormt een ernstige bedreiging voor de gezondheid en het welzijn van patiënten. Het mechanisme van AF is complex, waarbij het autonome zenuwstelsel een cruciale rol speelt bij het ontstaan en in stand houden ervan. Momenteel zijn medicatie en ablatie de belangrijkste behandelmethoden om AF-episodes te verminderen, maar de effectiviteit van medicamenteuze therapie is beperkt en herhaling na ablatie komt nog steeds voor. Recente onderzoeken hebben aangetoond dat veranderingen in de activiteit van het autonome zenuwstelsel een van de belangrijkste factoren zijn die bijdragen aan het terugkeren van AF na ablatie. Daarom wordt verwacht dat, door veranderingen in de activiteit van het autonome zenuwstelsel objectief te beoordelen, het succespercentage van AF-ablatie zal verbeteren en een potentiële remedie voor AF zal worden geboden, met aanzienlijke sociale en economische waarde.
Studies hebben aangetoond dat ablatie van longaderisolatie (PVI) de reguliere methode is voor AF-ablatie en dat dit de functie van het autonome zenuwstelsel beïnvloedt. Patiënten die na AF-ablatie significante veranderingen in de functie van het autonome zenuwstelsel vertonen, hebben hogere succespercentages. Bovendien zijn vroegtijdig optredende atriale aritmieën nauw verwant aan het terugkeren van AF op de lange termijn na ablatie. De mechanismen die ten grondslag liggen aan vroegtijdig recidief vereisen echter nog verder onderzoek.
Ons onderzoeksteam heeft diepgaande onderzoeken uitgevoerd naar de regulatie van de functie van het autonome zenuwstelsel in het hart en naar niet-invasieve beoordelingstechnieken, waarmee baanbrekende vooruitgang is geboekt. Het ganglion van het linker atrium speelt een cruciale rol bij het initiëren en in stand houden van AF, en ablatie van het ganglion kan het succespercentage van AF-ablatie verbeteren. Niet-invasieve beoordelingstechnieken zoals hartslagvertraging kunnen veranderingen in de activiteit van de nervus vagus in het hart kwantitatief evalueren, wat kan worden gebruikt om de werkzaamheid van autonome ganglioninterventietherapie te voorspellen.
Op basis van het bovenstaande onderzoek veronderstelt ons onderzoeksteam dat nauwkeurige en kwantitatieve beoordeling van de functie van het autonome zenuwstelsel met behulp van hartslagvertraging herhaling van AF na PVI-ablatie kan voorspellen. Om deze hypothese te valideren, zijn we van plan een prospectieve observationele studie uit te voeren, waarbij we patiënten met paroxysmale AF rekruteren, veranderingen in de functie van het autonome zenuwstelsel beoordelen, vroege atriale aritmieën en herhaling van AF monitoren, en theoretische ondersteuning bieden voor de screening en interventie van hoog- patiënten die een risico lopen op een recidief, waardoor een basis wordt gelegd voor klinische toepassing.

Beschrijving
Inclusiecriteria:
- Onderteken het geïnformeerde toestemmingsformulier;
- Duidelijk gediagnosticeerd als paroxismale AF, bereid om CPVI-behandeling te ondergaan;
- Een klasse I of klasse III anti-aritmicum met slechte werkzaamheid of intolerantie voor geneesmiddelen.
Uitsluitingscriteria:
- Diagnose van sinusritme bij rekrutering;
- Leeftijd is <18 jaar of> 75 jaar;
- Transthoracale echocardiografie suggereerde een anterieure en posterieure diameter van het linker atrium van> 55 mm;
- Voorgeschiedenis van katheterablatie of chirurgische ablatie voor AF;
- Linker atriale trombus vastgelegd door middel van echografie of CT;
- Met ernstige longziekten;
- Voorgeschiedenis van hartchirurgie;
- Patiënten met hyperthyreoïdie, atriumseptumdefect, mitralisklepstenose of ernstige coronaire hartziekte die verdere behandeling nodig hebben;
- Tijdens zwangerschap of borstvoeding.
