Een fase 2, gerandomiseerde, dubbelblinde, placebogecontroleerde, parallelle groepsstudie (TRANSFORM) om de werkzaamheid en veiligheid van GSK3915393 te evalueren bij deelnemers met idiopathische longfibrose (IPF)
Idiopathische longfibrose is een chronische longziekte die littekenvorming op de longen en ademhalingsmoeilijkheden veroorzaakt.
GSK3915393 is een nieuw medicijn, dat voor het eerst wordt getest bij deelnemers met IPF.
De studie zal de veiligheid en effectiviteit van GSK3915393 bij IPF-deelnemers beoordelen.

Inclusiecriteria:
- Deelnemers bij wie IPF binnen 5 jaar voorafgaand aan de screening is gediagnosticeerd op basis van de toepasselijke richtlijnen van de American Thoracic Society (ATS)/European Respiratory Society (ERS)/Japans Respiratory Society (JRS)/Latijns-Amerikaanse Thoracic Society (ALAT) op het moment van de diagnose.
- Centraal uitgelezen borstkas Hoge Resolutie Computertomografie (HRCT) verkregen bij screening of historische HRCT verkregen binnen 12 maanden na screening die consistent is met gebruikelijke interstitiële pneumonie (UIP) of waarschijnlijke UIP (bij onbepaalde HRCT-bevinding kan IPF lokaal worden bevestigd door historische biopsie) .
- FVC groter dan of gelijk aan (>=) 45 procent (%) van de voorspelde normaalwaarde.
- Verspreidingscapaciteit (van de longen) voor koolmonoxide (DLCO) >=25% van de voorspelde normaalwaarde, gecorrigeerd voor hemoglobine (Hb).
- Prebronchodilatator Geforceerd expiratoir volume in 1 seconde (FEV1)/FVC ≥ 0,7.
- Als u antifibrotica krijgt, moet u gedurende ten minste 12 weken vóór de screening een stabiele dosis nintedanib of pirfenidon gebruiken.
- Als de deelnemer momenteel geen pirfenidon of nintedanib krijgt, moet de deelnemer minimaal 4 weken vóór de screening gestopt zijn met pirfenidon of nintedanib.
- Lichaamsgewicht ≥40 kilogram (kg) en body mass index binnen het bereik van 18,5-35 kilogram per vierkante meter (kg/m2) (inclusief).
- Een vrouwelijke deelnemer komt in aanmerking voor deelname als een vrouw die niet zwanger kan worden (WONCBP)
- In staat om ondertekende geïnformeerde toestemming te geven
Uitsluitingscriteria:
- Deelnemers met interstitiële longziekte (ILD) geassocieerd met andere bekende oorzaken.
- Diagnose van sarcoïdose of een systemische auto-immuunziekte (inclusief maar niet beperkt tot sclerodermie, polymyositis/dermatomyositis, systemische lupus erythematosus en reumatoïde artritis).
- Acute IPF-exacerbatie binnen 6 maanden voorafgaand aan de screening en/of tijdens de screeningsperiode (bepaald door de onderzoeker).
- Klinisch significante niet-parenchymale longziekte (bijv. astma, chronische obstructieve longziekte, cavitaire of pleurale ziekten) bij screening.
- Diagnose van ernstige pulmonale hypertensie (door onderzoeker bepaald)
- Volgens de gerapporteerde resultaten van de meest recente HRCT is de omvang van het emfyseem groter dan de omvang van de fibrose.
- Geschiedenis van een eerdere longtransplantatie of recente grote operatie (door de onderzoeker bepaald) binnen 12 weken voorafgaand aan de screening of gepland tijdens de proefperiode. Registratie op een wachtlijst voor transplantatie is toegestaan.
- Klinisch significante luchtweginfectie (bijv. actieve tuberculose, infectieuze longontsteking, Corona-virusziekte 2019 [COVID-19]) die behandeling vereist binnen 4 weken voorafgaand aan en/of tijdens de screeningperiode.
- Het roken van sigaretten (inclusief e-sigaretten) op dit moment of binnen 3 maanden vóór de screening.
- Huidige of chronische leverziekte of bekende lever- of galafwijkingen (met uitzondering van het Gilbert-syndroom of asymptomatische galstenen).
- Alaninetransaminase (ALT), aspartaattransaminase (AST), alkalische fosfatase (ALP) >2x bovengrens van normaal (ULN) en bilirubine >1,5x ULN (geïsoleerd bilirubine >1,5xULN is acceptabel als bilirubine gefractioneerd is en direct bilirubine minder dan <) 35% bij screening).
- Klinisch significante afwijkingen gedetecteerd op een ECG van ritme of geleiding, een gecorrigeerd QT-interval (QTc) >450 milliseconde (msec) of QTc > 480 msec voor deelnemers met een bundeltakblok en/of een pacemaker die actief ventriculair pacing tijdens het screening-ECG .
- Deelnemers met pacemakers die op het moment van het screening-ECG geen pacemaker zijn, moeten een niet-gestimuleerde QTc <450 msec hebben.
Voorafgaande/gelijktijdige therapie-
- Gelijktijdig gebruik van pirfenidon en nintedanib bij screening.
- Ontvangen systemische corticosteroïden equivalent aan prednison >10 mg/dag of equivalent binnen 2 weken na de screeningsperiode.
- Gebruik van een van de volgende therapieën binnen 4 weken voorafgaand aan de screening en tijdens de screeningperiode of gepland tijdens het onderzoek:
- Immunomodulerende therapieën, waaronder maar niet beperkt tot azathioprine, mycofenolaatmofetil, methotrexaat, tacrolimus, cyclofosfamide, imatinib, tumornecrosefactor-alfa (TNF-α)-remmers.
- Medicijnen die worden onderzocht voor de behandeling van IPF, waaronder geïnhaleerde treprostinil en fosfodiësterase-4 (PDE-4) -remmers. Symptomatische hoesttherapieën zijn toegestaan.
- Huidig gebruik van systemische sterke en matige inductoren of remmers van Cytochroom P450 3A4 (CYP3A4) (zie de sectie verboden medicatie voor meer informatie) die niet veilig kan worden stopgezet of minstens 14 dagen vóór randomisatie kan worden overgezet op een alternatief middel.
- Huidig gebruik van systemische CYP3A4-substraten met een smalle therapeutische index die niet veilig kan worden stopgezet of ten minste 14 dagen vóór randomisatie kan worden overgezet op een alternatief middel.

Baseline was defined as the last non-missing value/assessment prior to the first dose of study treatment on Day 1. Change from Baseline (CFB) in FVC at Week 26 was calculated for each participant using the FVC Week 26 result minus the Baseline FVC result.
Posterior median CFB and the 95% highest posterior density (HPD) interval were derived using a Bayesian repeated measures model.

Gekwalificeerde onderzoekers kunnen via het Data Sharing Portal toegang vragen tot geanonimiseerde gegevens op patiëntniveau (IPD) en gerelateerde onderzoeksdocumenten van de in aanmerking komende onderzoeken.

IPD-tijdsbestek voor delen
Geanonimiseerde IPD zal beschikbaar worden gemaakt binnen zes maanden na publicatie van de primaire, belangrijke secundaire en veiligheidsresultaten voor onderzoeken naar producten met goedgekeurde indicatie(s) of beëindigde middelen voor alle indicaties.
IPD-toegangscriteria voor delen
Geanonimiseerde IPD wordt gedeeld met onderzoekers wier voorstellen zijn goedgekeurd door een onafhankelijk beoordelingspanel en nadat er een overeenkomst voor het delen van gegevens is gesloten.
Toegang wordt verleend voor een initiële periode van twaalf maanden, maar indien gerechtvaardigd kan een verlenging worden verleend tot maximaal zes maanden.
