Descrizione
Criterio di inclusione:
- Partecipanti con IPF diagnosticata entro 5 anni prima dello screening in base alle linee guida applicabili dell'American Thoracic Society (ATS)/European Respiratory Society (ERS)/Japanese Respiratory Society (JRS)/Latin American Thoracic Society (ALAT) al momento della diagnosi.
- Tomografia computerizzata ad alta risoluzione (HRCT) del torace letta centralmente ottenuta allo screening o HRCT storica ottenuta entro 12 mesi dallo screening che sia coerente con polmonite interstiziale usuale (UIP) o UIP probabile (se il risultato della HRCT è indeterminato, l'IPF può essere confermata localmente mediante biopsia storica) .
- FVC maggiore o uguale a (>=) 45% (%) del normale previsto.
- Capacità di diffusione (del polmone) per il monossido di carbonio (DLCO) >= 25% del normale previsto corretto per l'emoglobina (Hb).
- Volume espiratorio forzato prebroncodilatatore in 1 secondo (FEV1)/FVC ≥ 0,7.
- I pazienti in trattamento con antifibrotici devono assumere una dose stabile di nintedanib o pirfenidone per almeno 12 settimane prima dello screening.
- Se attualmente non riceve pirfenidone o nintedanib, il partecipante deve aver interrotto pirfenidone o nintedanib per almeno 4 settimane prima dello screening.
- Peso corporeo ≥40 chilogrammi (kg) e indice di massa corporea compreso tra 18,5 e 35 chilogrammi per metro quadrato (kg/m2) (incluso).
- Una partecipante di sesso femminile può partecipare se è una donna in età non fertile (WONCBP)
- In grado di dare il consenso informato firmato
Criteri di esclusione:
- Partecipanti con malattia polmonare interstiziale (ILD) associata ad altre cause note.
- Diagnosi di sarcoidosi o di qualsiasi malattia autoimmune sistemica (inclusi ma non limitati a sclerodermia, polimiosite/dermatomiosite, lupus eritematoso sistemico e artrite reumatoide).
- Esacerbazione acuta dell'IPF entro 6 mesi prima dello screening e/o durante il periodo di screening (determinato dallo sperimentatore).
- Malattia polmonare non parenchimale clinicamente significativa (ad es. Asma, malattia polmonare ostruttiva cronica, malattie cavitarie o pleuriche) allo screening.
- Diagnosi di ipertensione polmonare grave (determinata dallo sperimentatore)
- Secondo i risultati riportati dalla più recente HRCT, l'entità dell'enfisema è maggiore dell'entità della fibrosi.
- Anamnesi di precedente trapianto polmonare o recente intervento chirurgico importante (determinato dallo sperimentatore) nelle 12 settimane precedenti lo screening o pianificato durante il periodo di sperimentazione. È consentita l'iscrizione in una lista d'attesa per il trapianto.
- Infezione del tratto respiratorio clinicamente significativa (ad es. tubercolosi attiva, polmonite infettiva, malattia da Corona virus 2019 [COVID-19]) che richiede trattamento entro 4 settimane prima e/o durante il periodo di screening.
- Fumo di sigaretta (comprese le sigarette elettroniche) attuale o entro 3 mesi prima dello screening.
- Malattia epatica attuale o cronica o anomalie epatiche o biliari note (ad eccezione della sindrome di Gilbert o dei calcoli biliari asintomatici).
- Alanina transaminasi (ALT), aspartato transaminasi (AST), fosfatasi alcalina (ALP) >2 volte il limite superiore della norma (ULN) e bilirubina >1,5x ULN (la bilirubina isolata >1,5xULN è accettabile se la bilirubina è frazionata e la bilirubina diretta inferiore a ( <) 35% allo screening).
- Anomalie clinicamente significative rilevate sull'ECG del ritmo o della conduzione, un intervallo QT corretto (QTc) > 450 millisecondi (msec) o QTc > 480 msec per i partecipanti con un blocco di branca e/o un pacemaker che stanno stimolando attivamente il ventricolo durante l'ECG di screening .
- I partecipanti con pacemaker che non stimolano al momento dell'ECG di screening devono avere un QTc non stimolato <450 msec.
Terapia precedente/concomitante-
- Uso simultaneo di pirfenidone e nintedanib allo screening.
- Hanno ricevuto corticosteroidi sistemici equivalenti al prednisone >10 mg/die o equivalenti entro 2 settimane dal periodo di screening.
- Utilizzo di una qualsiasi delle seguenti terapie entro 4 settimane prima dello screening e durante il periodo di screening o pianificata durante lo studio:
- Terapie immunomodulanti, inclusi ma non limitati a azatioprina, micofenolato mofetile, metotrexato, tacrolimus, ciclofosfamide, imatinib, inibitori del fattore di necrosi tumorale alfa (TNF-α).
- Farmaci in fase di studio per il trattamento dell’IPF, inclusi treprostinil per via inalatoria e inibitori della fosfodiesterasi-4 (PDE-4). Sono consentite terapie sintomatiche per la tosse.
- Uso attuale di induttori o inibitori sistemici forti e moderati del citocromo P450 3A4 (CYP3A4) (vedere la sezione sui farmaci proibiti per ulteriori informazioni) che non possono essere interrotti in sicurezza o passati a un agente alternativo almeno 14 giorni prima della randomizzazione.
- Uso attuale di substrati sistemici del CYP3A4 che hanno un indice terapeutico ristretto che non può essere interrotto in sicurezza o passato a un agente alternativo almeno 14 giorni prima della randomizzazione.

Forced vital capacity (FVC) is defined as the amount of air that can be forcibly exhaled from the lungs after taking the deepest breath possible.
It was measured by spirometry test.
The maximum of triplicate FVC for each participant was used for calculations.
Baseline was defined as the last non-missing value/assessment prior to the first dose of study treatment on Day 1. Change from Baseline (CFB) in FVC at Week 26 was calculated for each participant using the FVC Week 26 result minus the Baseline FVC result.
Posterior median CFB and the 95% highest posterior density (HPD) interval were derived using a Bayesian repeated measures model.
The data presented as "Median" refers to the 'posterior median' and "95% confidence interval" to '95% HPD interval'.

Participants with relative decline of <=5% at Week 26 were classified as responders; those with greater than (>) 5% decline were non-responders.
Participants who died due to disease progression prior to Week 26 were imputed as non-responders.

Triplicate 12-lead ECGs were obtained after participant has rested in supine position for 5 minutes, using an ECG machine that automatically calculates heart rate and measures PR, QRS, QT, and QTc intervals.
ECG parameters with PCI ranges were: PR Interval (low: <110 milliseconds[msec], high: >220 msec), QRS Duration (low: <75 msec, high: >120 msec) and QTcF Interval (<=450 msec, >450 msec to <=480 msec, >480 msec to <=500 msec and >500 msec).
Participants were counted in the worst-case category if their value changed 'To Low' or 'To High', unless there was no change in their category.
Participants whose value category was unchanged (e.g., High to High), or whose value became within range, were recorded in 'To Within Range or No Change' category.

Periodo di condivisione IPD
Gli IPD anonimizzati saranno resi disponibili entro 6 mesi dalla pubblicazione dei risultati primari, secondari chiave e di sicurezza per gli studi su prodotti con indicazioni approvate o risorse cessate per tutte le indicazioni.
Criteri di accesso alla condivisione IPD
L'IPD anonimizzato viene condiviso con i ricercatori le cui proposte sono approvate da un gruppo di revisione indipendente e dopo la stipula di un accordo di condivisione dei dati.
L'accesso è previsto per un periodo iniziale di 12 mesi ma può essere concessa una proroga, ove giustificato, fino a 6 mesi.
