Cervicale radiculopathie (CR) is een aandoening waarbij de zenuwwortel van een spinale zenuw wordt samengedrukt of aangetast, waardoor de pijn en symptomen zich voorbij de nek verspreiden en uitstralen naar de schouder en het bovenste ledemaat. Cervicale radiculopathie is voornamelijk het gevolg van een botsing en ontsteking van een zenuwwortel, veroorzaakt door een ruimte-innemende laesie die de grootte van het foramen tussen de wervels verkleint, als een degeneratieve laesie van het zygapophyseale gewricht, of het is geassocieerd met een verstoring van de cervicale schijf.
Patiënten met CR klagen over nek-, periscapulaire en radiculaire pijn in de hand en arm. Evenals neurologische symptomen zoals sensorische stoornissen (paresthesie of gevoelloosheid), spierzwakte met verminderde peesreflexen in de aangetaste zenuwwortel of een combinatie van deze klachten en symptomen.
De gerapporteerde jaarlijkse incidentie van CR bedraagt 85 gevallen per 100.000 mensen in de bevolking, terwijl de prevalentie 3,5/1000 personen bedraagt. De zenuwwortel C7 wordt het vaakst getroffen, waarbij meer dan de helft van alle gevallen dit niveau treft.
Verschillende onderzoeken maakten gebruik van therapeutische oefeningen, manuele therapie, andere modaliteiten halskraag, cervicale tractie, houdingseducatie en verschillende medicijnen zoals medicijnen en steroïde-injecties bij de conservatieve behandeling van cervicale radiculopathie in de verschillende stadia.
De neurale mobilisatie (NM) is een manuele therapiemethode die de neurale flexibiliteit verbetert, de dynamische gevoeligheid van het zenuwstelsel verlaagt, de bloedstroom verhoogt en pijn verlicht; daarvoor vergroten verbeterde neurale mobiliteit en verlichte pijn het bewegingsbereik van de gewrichten (ROM).
De neurale mobilisatietechnieken (NM) worden geleverd door twee technieken, "glijden/glijden" en "spannen". De techniek van de spanner genereert spanning aan beide uiteinden van de zenuw, terwijl bij sliders de zenuw ten opzichte van de omliggende structuren glijdt door gewrichtsbewegingen uit te voeren die het zenuwbed met minimale spanning verlengen. Bovendien zijn sliders gewoonlijk minder agressief dan spanners en kan het gebruik ervan geïndiceerd zijn in vroege ziektestadia.
Beide technieken helpen bij het voorkomen van de vorming van verklevingen, verminderen de endoneuriële druk, verminderen intraneuraal oedeem, verhogen de zuurstofvoorziening van de zenuwen en verminderen de ischemische pijn.
Studies waarin NM-technieken met sliders en spanners in hun klinische onderzoeken werden gebruikt, lieten significante veranderingen zien in biomechanische factoren, zoals de zelfrapportages van patiënten over pijn, invaliditeit, bewegingsvrijheid, uithoudingsvermogen en spierkracht bij de behandeling van nek- en skeletaandoeningen met zenuwgerelateerde symptomen.
Wat zijn daarom de mogelijke effecten van het toevoegen van de neurale mobilisatietechniek met spanner versus neurale mobilisatietechniek met slider aan conventionele fysieke behandeling bij de behandeling van patiënten met cervicale radiculopathie?

Beschrijving
Inclusiecriteria:
- Huidige aanhoudende of intermitterende pijn die langer dan 3 maanden aanhoudt.
- Motorische, reflex- en/of sensorische veranderingen in het bovenste lidmaat.
- Verminderde diepe peesreflexen in de aangedane arm.
- Paresthesie of gevoelloosheid langs de loop van de zenuw.
- Een positieve Upper Limb Neural Tension 1 (ULNT)-test voor de medianuszenuw, Spurlings-test, compressietest, cervicale distractietest, cervicale rotatie van minder dan 60° naar de symptomatische kant en Valsalva-manoeuvre.
- Eenzijdige aandoening in het bovenste lidmaat.
Uitsluitingscriteria:
- Geschiedenis van chirurgische procedures voor pathologieën die aanleiding geven tot nekpijn of CTS.
- Klinische tekenen of symptomen van medische alarmsignalen; (infectie, kanker en hartbetrokkenheid).
- Patiënten met nekpijn vanwege tekenen en symptomen van ernstige pathologie, insufficiëntie van de bovenste cervicale ligamenten, onverklaarde disfunctie van de hersenzenuwen en fracturen.
- Elke systematische ziekte zoals reuma en tuberculose, cervicale myelopathie of multiple sclerose.
- Het is bekend dat systemische ziekten gegeneraliseerde perifere neuropathie veroorzaken, zoals diabetes mellitus.
- Ziekten van de bovenste motorneuronen, zoals beroerte en amyotrofische laterale sclerose (ALS).
- Volledig verlies van gevoel langs de betrokken zenuwwortel.
- Primair rapport van bilaterale uitstralende armpijn.
- Onvermogen om de geplande interventie te tolereren.
- Zwangere vrouw.
- Thoracic outlet-syndroom
- Pronator teres-syndroom
