La radiculopatía cervical (CR) es una afección en la que la raíz nerviosa de un nervio espinal se comprime o daña, lo que hace que el dolor y los síntomas se extiendan más allá del cuello, irradiando al hombro y las extremidades superiores. La radiculopatía cervical resulta principalmente de un pinzamiento e inflamación de una raíz nerviosa inducida por una lesión que ocupa espacio y que reduce el tamaño del agujero intervertebral, como una lesión degenerativa de la articulación cigapofisaria o está asociada con un trastorno del disco cervical.
Los pacientes que presentan RC se quejan de dolor en el cuello, periescapular y radicular en la mano y el brazo. Además de síntomas neurológicos como alteraciones sensoriales (parestesia o entumecimiento), debilidad muscular con reflejos tendinosos reducidos en la raíz nerviosa afectada o una combinación de estos signos y síntomas.
La incidencia anual informada de RC es de 85 casos por 100.000 personas de la población, mientras que la prevalencia es de 3,5/1.000 personas. La raíz nerviosa C7 es la que se afecta con mayor frecuencia y más de la mitad de todos los casos afectan a este nivel.
Varios estudios utilizaron ejercicios terapéuticos, terapia manual, otras modalidades de collarín cervical, tracción cervical, educación postural y diferentes medicamentos como drogas e inyecciones de esteroides en el manejo conservador de la radiculopatía cervical en sus diferentes etapas.
La movilización neuronal (NM) es un método de terapia manual que mejora la flexibilidad neuronal, reduce la sensibilidad dinámica del sistema nervioso, aumenta el flujo sanguíneo y alivia el dolor; por eso, la mejora de la movilidad neuronal y el alivio del dolor aumentan el rango de movimiento (ROM) de las articulaciones.
Las técnicas de movilización neuronal (NM) se administran mediante dos técnicas, "deslizamiento/deslizamiento" y "tensión". La técnica del tensor genera tensión desde ambos extremos del nervio, mientras que los controles deslizantes implican el deslizamiento del nervio en relación con las estructuras circundantes mediante la realización de movimientos articulares que alargan el lecho del nervio con una tensión mínima. Además, los controles deslizantes suelen ser menos agresivos que los tensores y su uso podría estar indicado en las primeras etapas de la enfermedad.
Ambas técnicas ayudan a prevenir la formación de adherencias, reducir la presión endoneural, reducir el edema intraneural, aumentar la oxigenación de los nervios y disminuir el dolor isquémico.
Los estudios que utilizaron técnicas de NM con deslizadores y tensores en sus estudios clínicos mostraron cambios significativos en factores biomecánicos, como los autoinformes de los pacientes sobre dolor, discapacidad, ROM, resistencia y fuerza muscular en el tratamiento de trastornos musculoesqueléticos del cuello con síntomas relacionados con los nervios.
Por lo tanto, ¿cuáles son los posibles efectos de agregar la técnica de movilización neural con tensor versus la técnica de movilización neural deslizante al tratamiento físico convencional en el tratamiento de pacientes con radiculopatía cervical?

Descripción
Criterios de inclusión:
- Dolor actual continuo o intermitente que ha persistido durante más de 3 meses.
- Cambios motores, reflejos y/o sensoriales en el miembro superior.
- Disminución de los reflejos tendinosos profundos en el brazo afectado.
- Parestesia o entumecimiento a lo largo del trayecto del nervio.
- Una prueba de tensión neural del miembro superior 1 (ULNT) positiva para el nervio mediano, prueba de Spurlings, prueba de compresión, prueba de distracción cervical, rotación cervical de menos de 60 ° hacia el lado sintomático y maniobra de Valsalva.
- Afectación unilateral en el miembro superior.
Criterios de exclusión:
- Historia de procedimientos quirúrgicos por patologías que dan lugar a dolor de cuello o STC.
- Signos o síntomas clínicos de señales de alerta médicas; (infección, cáncer y afectación cardíaca).
- Pacientes con dolor de cuello por signos y síntomas de patología grave, insuficiencia del ligamento cervical superior, disfunción inexplicable de los nervios craneales y fractura.
- Cualquier enfermedad sistemática como reumatismo y tuberculosis, mielopatía cervical o esclerosis múltiple.
- Se sabe que la enfermedad sistémica causa neuropatía periférica generalizada como la diabetes mellitus.
- Enfermedad de la neurona motora superior, como accidente cerebrovascular y esclerosis lateral amiotrófica (ELA).
- Pérdida completa de sensación a lo largo de la raíz nerviosa afectada.
- Informe primario de dolor bilateral irradiado en el brazo.
- Incapacidad para tolerar la intervención planificada.
- Mujer embarazada.
- Síndrome de salida torácica
- Síndrome del pronador redondo
