Vroege palliatieve zorg voor patiënten met mondkanker in Sri Lanka
De prevalentie en daarmee samenhangende factoren van psychische problemen en de effectiviteit van een 'levenssituatie die de interventie verbetert' onder mondkankerpatiënten die ziekenhuizen voor tertiaire zorg in het district Colombo bezoeken
Het doel van deze quasi-experimentele studie is het ontwikkelen en beoordelen van de effectiviteit van een vroege palliatieve zorginterventie bij patiënten met mondkanker in Sri Lanka. De belangrijkste vraag die het wil beantwoorden is of de vroege palliatieve zorginterventie de kwaliteit van leven en psychische problemen van patiënten met kanker in Sri Lanka verbetert.
De primaire uitkomstmaten waren de kwaliteit van leven van de patiënt en de mate van psychologische problemen. Als er een vergelijkingsgroep is: Onderzoekers vergeleken met een controlegroep die de standaardzorg ontving om te zien of de nieuwe vroege palliatieve zorginterventie hielp de primaire uitkomsten te verbeteren.
Deelnemers aan de interventie-arm ontvingen het vroege palliatieve zorgpakket dat bestond uit 6 componenten, waaronder: 1) het verstrekken van informatie, 2) het aanpakken van acute en functionele problemen, 3) voedingszorg, 4) psychologische ondersteuning, 5) mindfulness-therapie en 6) coördinatie van de financiële tegemoetkoming. De interventie werd uitgevoerd door de hoofdonderzoeker en de getrainde verpleegkundigen van de openbare gezondheidszorg.

Het onderzoek vond plaats op drie tertiaire zorgafdelingen die mondkankerbehandeling aanboden in Sri Lanka: de orale en maxillofaciale afdelingen van het National Dental Hospital, Colombo (Teaching); de orale en maxillofaciale afdelingen van het Colombo South (Teaching) Hospital, Karapitiya; en de onco-chirurgieafdelingen van het National Cancer Institute Maharagama (Apeksha Hospital).
Deelnemers De deelnemers aan de studie waren patiënten met mondkanker van wie de definitieve diagnose aan de patiënt was meegedeeld en die psychische klachten hadden (score van ≥ 4 na screening met de Singalese versie van de Distress Thermometer); wachten op een operatie als eerste behandelingsmodaliteit; en getrouwd en heeft kinderen, waaronder ten minste één gezinsverzorger die goed kan communiceren en lezen in het Singalees. Uitsluitingscriteria waren terugkerende mondkanker; een formele psychiatrische diagnose; en het ontvangen of hebben ontvangen van een vroege palliatieve zorginterventie. Ze werden verdeeld over 55 controles en 55 gevallen op basis van de beschikbaarheid van een toegankelijke verpleegkundige op het gebied van de volksgezondheid.
Interventies Het pakket voor vroege palliatieve zorg, bestaande uit zes componenten, werd in drie sessies gegeven door de hoofdonderzoeker (PI) en verpleegkundigen in de volksgezondheid.
Uitkomstmaten De effectiviteit werd beoordeeld aan de hand van de mate van psychisch lijden met behulp van de Singalese versie van de Distress Thermometer en het niveau van kwaliteit van leven met behulp van EORTC QLQ 30 met module H&N 35 bij aanvang (T0), na de interventie (T1), één maand ( T2) en drie maanden (T3).

Het interventiepakket voor vroege palliatieve zorg is ontwikkeld volgens de richtlijnen van de UK Medical Research Council voor de ontwikkeling van complexe interventies.
De vroege palliatieve zorginterventie voor patiënten met mondkanker bestond uit zes componenten: 1) het verstrekken van informatie, 2) het aanpakken van acute en functionele problemen, 3) voedingszorg, 4) psychologische ondersteuning, 5) mindfulness-therapie en 6) coördinatie van de financiële vergoeding. .
Het pakket werd samengesteld door de bevindingen van literatuuronderzoek en verschillende onafhankelijke onderzoeken te trianguleren: een patiënt-controleonderzoek; diepte-interviews met deskundigen op het gebied van oncologie en palliatieve zorg in Sri Lanka; interviews met belangrijke informanten met patiënten met mondkanker en hun zorgverleners; en een observatiestudie bij het National Cancer Institute Sri Lanka-Apeksha Hospital).

Het niveau van psychische nood van de patiënt

Het niveau van psychische nood werd gemeten aan de hand van de gemiddelde scores van de Singalese versie van de Distress Thermometer.
De Distress Thermometer is een ultrakort, visueel analoog screeningsinstrument, oorspronkelijk ontwikkeld in Amerika voor prostaatkankerpatiënten in 1998.
De noodthermometer wordt geleverd met een probleemlijst.
Deze bestaat uit 35 items en bestrijkt vijf levensdomeinen (praktische, familiale/sociale, emotionele, spirituele en fysieke problemen).
De Noodthermometer en de Problemenlijst zijn vertaald in het Singalees (de primaire taal in Sri Lanka) en intercultureel aangepast aan Sri Lanka.

Dit werd gemeten met behulp van de Head & Neck-35-module (H&N-35) van de Europese Organisatie voor Onderzoek en Behandeling van Kanker.
