Effecten van donor-ontvanger geslachtsafhankelijke bloedtransfusie op de resultaten van de patiënt (SexMATTERS RCT)
Effecten van donor-ontvanger seks-matched versus geslachtsgebonden rode bloedceltransfusie op de resultaten bij kritisch zieke volwassen patiënten
Transfusies van rode bloedcellen (RBC) worden geselecteerd op basis van overeenkomende donor- en ontvangerbloedgroep: donor en ontvanger -seks worden niet in aanmerking genomen bij het selecteren van bloed voor transfusie. Daarom kunnen getransfuseerde patiënten momenteel op geslacht gematchte en/of ongeëvenaarde RBC's ontvangen wanneer transfusies worden gegeven. Seks-afgestemde stamceltransplantaties, en sommige solide orgaantransplantaties, hebben aangetoond dat seksuele donor voor ontvanger de resultaten van de patiënt verbetert. Recente verkennende studies hebben ook gesuggereerd dat de resultaten van de patiënt kunnen worden verbeterd door geslachtsafwijking voor RBC-transfusie. Er is opkomend bewijs van onderliggende biologische mechanisme (s) om deze waarnemingen te ondersteunen. Deze studie is ontworpen als een gerandomiseerde gecontroleerde studie en zal de impact onderzoeken op patiënten die RBC-transfusies ontvangen van donoren van hetzelfde geslacht ("geslachtsgebonden") vergeleken met donoren van het andere geslacht ("geslachtsgewijs").
De studie zal volwassen patiënten bestuderen die zijn opgenomen op de intensive care die een RBC -transfusie nodig heeft. Patiënten zullen worden toegewezen (via een proces dat randomisatie wordt genoemd) om geslachtsgebonden RBC's of geslachtsgebonden RBC's te ontvangen om te bepalen of er een verschil in mortaliteit is tussen degenen die gematchte versus niet-overeenkomende RBC's ontvangen. De resultaten van deze proef kunnen directe implicaties hebben voor middelen, bloedinventarisatie en RBC -transfusie -bestelpraktijken.

Achtergrond: Rode bloedcel (RBC) transfusies worden geselecteerd op basis van donor- en ontvanger -compatibiliteit met bloedgroep zonder rekening te houden met donor en ontvanger seks. Stamceltransplantaties, en sommige vaste orgaantransplantaties, hebben aangetoond dat seksuele donor naar ontvanger de resultaten van de patiënt verbetert. De hoofdonderzoeker heeft een systematische review/meta-analyse en een verkennende analyse van 40.000 patiënt voltooid; Beiden vertoonden een signaal van voordeel van geslachtsgebonden RBC-transfusie. Biologische bevindingen ter ondersteuning van de hypothese dat geslachtsgebonden RBC-transfusies de overleving bij ontvangers verbeteren, zijn ook in opkomst.
Controversiële resultaten zijn voortgekomen uit observationele studies en een gerandomiseerde gecontroleerde studie (RCT) met betrekking tot de mortaliteit van ontvangers van geslachtsgebonden RBC-transfusies; Daarom bestaat Equipoise.
De hoofdonderzoeker voltooide een multisite piloot RCT die de haalbaarheid aantoonde voor het uitvoeren van een grootschalige RCT en heeft geholpen om het ontwerp voor dit onderzoeksvoorstel te informeren. Het doel van dit onderzoeksproject is om een precisietransfusiestrategie te ontwikkelen op basis van seksuele donor en ontvanger om nadelige effecten te minimaliseren en de patiëntuitkomsten na transfusie te verbeteren.
Algemene onderzoeksvraag: bij getransfuseerde volwassen patiënten die zijn opgenomen op de intensive care (ICU), resulteren donor-recipiënt geslachtsafhankelijke RBC-transfusies in verbeterde mortaliteit in vergelijking met geslachtsgemaakte RBC-transfusies?
Methoden: Deze studie is een superioriteit RCT met pragmatische kenmerken die zich op deelnemende sites in Canada zullen voordoen.
Studiepopulatie: volwassen patiënten (leeftijd ≥18) die in het ziekenhuis worden opgenomen met opname bij een in aanmerking komende ICU en RBC -transfusie vereisen.
Interventie: Donor-ontvanger seks-matched RBC-transfusies. Vergelijking: donor-ontvanger-geslachtsgebonden RBC-transfusies.
Primaire uitkomst: 30 -daagse sterfte uit de tijd van randomisatie. Secundaire resultaten: tijd tot dood, 30-daagse sterfte in het ziekenhuis, 90 dagen sterfte, hemoglobine en creatinine-toename, ICU en ziekenhuislengtes van verblijf, aantal/type getransfuseerde producten en analyse van de kosteneffectiviteit. Subgroepanalyses zullen rekening houden met het feit dat de ontvanger geslacht, leeftijd, blootstelling aan transfusie, bloedgroep, toelating diagnose, baseline hemoglobine/creatinine.
Expertise: het studieteam is uniek gepositioneerd met uitgebreide kennis en expertise in transfusiemens, onderzoeksmethodologie, uitvoering van grote pragmatische proeven en biostatistiek. Het studieteam zal samenwerken met Canadese bloeddiensten en experts in bloedgebruik en operationele modellering. Deze studie wordt ondersteund en onderschreven door de Canadian Transfusion Trials Group.
Belang en resultaten: bloedarmoede komt veel voor bij ernstig zieke patiënten; Maar liefst 90% van de patiënten zal anemisch worden tegen de derde dag van hun ICU -opname. Tussen 20% en 40% van de ernstig zieke patiënten die op de IC zijn opgenomen, vereist RBC -transfusie; Elke patiënt ontvangt gemiddeld 2 tot 5 RBC -eenheden. De Age of Blood Evaluation (ABLE) -studie toonde een 90 dagen aan dat alle sterftecijfer van 35-37% veroorzaakt in de bestudeerde getransfuseerde ICU-populatie. De pilotstudie weerspiegelde vergelijkbare sterftecijfers. Als het aanbieden van geslachtsgebonden transfusies de sterfte kan verminderen, kan dit zich vertalen in een hoge aantallen voorkomende sterfgevallen, aangezien 85 miljoen RBC-eenheden elk jaar wereldwijd worden getransfuseerd. Als deze studie constateert dat donor-ontvanger seksuele matching levens redt, zou dit veranderingen op het niveau van de bloedleverancier vereisen om seks te identificeren op elke RBC-eenheid en veranderingen op ziekenhuisniveau die seksuele matching in de selectie van compatibel zouden omvatten bloed voor elke patiënt.

Inclusiecriteria:
- Volwassenen (leeftijd ≥18);
- Toegang tot een deelnemende ICU;
- Vereist RBC -transfusie.
Uitsluitingscriteria:
- Vereiste voor een gespecialiseerd RBC -product of eenheid die niet direct beschikbaar is in de inventaris (bijv. Zeldzame bloedgroep, gewassen RBC's, complexe RBC -antilichamen, enz.);
- Massaal bloedende patiënt (d.w.z. ≥4 eenheden bloed geordend in één keer geordend, of massief bloedingsprotocol geïnitieerd, of een dringend bloedverzoek gedaan);
- Seks onbekend of anders dan mannelijk of vrouwelijk (d.w.z. intersekse);
- Heeft geen geldig Ontario Health Insurance Plan (OHIP) gezondheidskaartnummer.
