De effectiviteit van trillingstraining van het hele lichaam bij het verbeteren van hersenactiviteit, botmineraaldichtheid, kwaliteit van leven en biomarkers voor de oudere mensen met obesitas van dynapenische buik
Deze studie richt zich op trillingstraining van het hele lichaam (WBVT), een vorm van neuromusculaire training die mechanische trillingen gebruikt. Van WBVT is aangetoond dat het de spierfunctie, botmineraaldichtheid en kwaliteit van leven verbetert. Hoewel sommige voordelen zijn waargenomen in de oudere bevolking, blijft er een onderzoekskloof over de effectiviteit ervan bij oudere volwassenen met obesitas van dynapense buik. Daarom zullen de bevindingen van deze studie nieuw bewijs leveren voor gezondheidsinterventies die zich richten op deze specifieke groep.
Dit onderzoek is gericht op poliklinische patiënten van 65 jaar en ouder die voldoen aan de criteria voor obesitas van de dynapense buik. Het beoogt de effecten van WBVT op hersenactiviteit, osteoporose, kwaliteit van leven en biologische markers te evalueren. De studieperiode is van 1 februari 2025 tot 31 januari 2027, met een geplande steekproefgrootte van 120 deelnemers, gelijk verdeeld in een experimentele groep en een controlegroep (elk 60 deelnemers).

Het doel van deze studie is om de effecten van trillingstraining op het hele lichaam op oudere volwassenen met dynapenische buikobesitas in de volgende gebieden te onderzoeken:
Hersenactiviteit: veranderingen in serumspiegels van van hersenen afgeleide neurotrofe factor (BDNF).
Botminerale dichtheid: gemeten met behulp van dual-energie röntgenabsorptiometrie (DXA).
Kwaliteit van leven: beoordeeld door vragenlijsten die betrekking hebben op verbeteringen in lichamelijke gezondheid, geestelijk welzijn en sociaal functioneren.
Biomarkers: veranderingen in niveaus van totaal cholesterol, lipoproteïne cholesterol met hoge en lage dichtheid, C-reactief eiwit (CRP) en geglycated hemoglobine (HbA1c).

Poliklinische patiënten van 65 jaar of ouder
Gediagnosticeerd met dynapenische buik obesitas
In staat om eenvoudige instructies te begrijpen en te volgen
In staat om duidelijk te communiceren en hun eigen wil uit te drukken
Bereid om het geïnformeerde toestemmingsformulier te ondertekenen
Uitsluitingscriteria:
Onvermogen om te communiceren in Mandarijn of Taiwanese, wat resulteert in het onvermogen om deel te nemen aan onderzoeksactiviteiten
Ernstige visuele of gehoorbeschadiging die de deelname van de studie belemmert
Aanwezigheid van cognitieve stoornissen, musculoskeletale of ernstige osteoporose-gerelateerde aandoeningen

Experimenteel: Trainingsgroep voor trillingen van het hele lichaam
Deelnemers aan de experimentele groep hebben drie keer per week gedurende een totale duur van 24 weken driemaal per week getraind (WBV) training (WBV).
Tijdens elke sessie stonden de deelnemers op blote voeten op een trillingsplatform, waarbij een statische squatpositie werd gehandhaafd met knieën gebogen op 30 graden en voeten schouderbreedte uit elkaar.
Het trainingsprotocol nam geleidelijk toe in duur, het aantal sets en de totale trainingstijd van een eerste 10 minuten tot 30 minuten gedurende de periode van 24 weken.
De trillingsfrequentie werd geleidelijk verhoogd van 20 Hz tot 40 Hz, met een constante amplitude van 2 mm.
Elke sessie van 10 minuten bevatte een rustinterval van 1 minuten.
Bovendien kregen alle deelnemers het handboek "Moving for Health" uitgegeven door de Health Promotion Administration, Ministerie van Volksgezondheid en Welzijn, Taiwan, als onderdeel van de begeleiding van gezondheidseducatie.
Gegevensverzameling na interventie werd uitgevoerd op weken 18 en 25 na het interventieprogramma van 24 weken.

Placebo-vergelijker: controlegroep
De controlegroep ontving het handboek "Moving for Health" uitgegeven door de Health Promotion Administration, Ministerie van Volksgezondheid en Welzijn, Taiwan, en nam deel aan 30 minuten durende sessies voor gezondheidseducatie gedurende een periode van 24 weken.
De educatieve inhoud omvatte inspanningsgerelateerde informatie, een inleiding tot dynapenische buikobesitas (DAO), bijbehorende risicofactoren en mogelijke gevolgen voor de gezondheid.
Gegevensverzameling na interventie werd uitgevoerd op weken 18 en 25 na het interventieprogramma van 24 weken.

Dual-Energy röntgenabsorptiometrie (DXA)

Deze studie gebruikte dual-energy röntgenabsorptiometrie (DXA), een veelgebruikte en klinisch veilige methode voor het beoordelen van de gezondheid van de bot.
Het DXA-systeem (merk waarnaar wordt verwezen in eerdere publicaties) werkt door een röntgenstraling met lage straling (± 10 μSV) uit te stoten om de mate van röntgendraging door lichaamsweefsels te meten.
Op basis van de absorptiegegevens berekent het apparaat de botmineraaldichtheid (BMD).
Aangezien botten meer röntgenfoto's absorberen en een hoge doordringbaarheid hebben, terwijl vet minder absorbeert en een lage doordringendheid heeft, onderscheidt DXA effectief tussen weefseltypen.
DXA-resultaten worden meestal geïnterpreteerd met behulp van T-scores voor BMD.
Een T -score ≥ -1,0 duidt op de normale botdichtheid en een goede skeletgezondheid.
Een T -score tussen -2,5 en -1.0 suggereert een lage botmassa (osteopenie), wat duidt op een verminderde BMD maar nog niet op de drempel voor osteoporose.
Een T -score ≤ -2,5 definieert osteoporose, hetgeen een significant verminderde BMD en een hoger risico op fracturen aangeeft.
Ernstige osteoporose wordt gediagnosticeerd wanneer een T -score ≤ -2,5.

De Taiwan-versie van de vragenlijst van de Wereldgezondheidsorganisatie Quality of Life is een verkorte versie afgeleid van de originele 100-Item Whoqol-100 ontwikkeld door de Wereldgezondheidsorganisatie.
Verkennende en bevestigende factoranalyses van de vragenlijst ondersteunen een vierdomeinmodel bestaande uit lichamelijke gezondheid, psychologisch welzijn, sociale relaties en milieu.
De interne consistentie (Cronbach's α) van deze vier domeinen varieert van 0,70 tot 0,77.
De vragenlijst bevat 28 items: één item voor de algehele kwaliteit van leven, één voor algemene gezondheid, zeven voor lichamelijke gezondheid, zes voor psychologisch welzijn, drie voor sociale relaties en acht voor omgevingsfactoren.
De schaal maakt gebruik van een 5-punts Likert-indeling op basis van frequentie, intensiteit, mogelijkheden en evaluatie.
Hogere scores duiden op een betere kwaliteit van leven.
